Plann<strong>in</strong>g<br />

<strong>and</strong> Design<strong>in</strong>g Capac<strong>it</strong>y<br />

Build<strong>in</strong>g <strong>in</strong> IFE (Day 4)<br />

6.1 Overview<br />

Day 4 of the workshop centred on capac<strong>it</strong>y build<strong>in</strong>g <strong>and</strong><br />

was run largely as <strong>in</strong>teractive work<strong>in</strong>g groups assess<strong>in</strong>g<br />

capac<strong>it</strong>y build<strong>in</strong>g needs <strong>in</strong> different scenarios 25 . The focus<br />

was on how to plan <strong>and</strong> undertake tra<strong>in</strong><strong>in</strong>g of front l<strong>in</strong>e<br />

workers to enable them to provide appropriate IYCF<br />

support to mothers/caregivers <strong>and</strong> their <strong>child</strong>ren <strong>in</strong><br />

<strong>emergencies</strong>. The day was faciltiated by Felic<strong>it</strong>y Savage<br />

(IFC) <strong>and</strong> Rebecca Norton (Independent/IFE Core Group).<br />

Build<strong>in</strong>g upon the capac<strong>it</strong>y build<strong>in</strong>g work<strong>in</strong>g group of<br />

Day 3, Day 4 started out w<strong>it</strong>h agreement that capac<strong>it</strong>y<br />

build<strong>in</strong>g was about systems <strong>and</strong> structures as well as<br />

tra<strong>in</strong><strong>in</strong>g. Tra<strong>in</strong><strong>in</strong>g has to be properly planned <strong>and</strong><br />

<strong>in</strong>tegrated w<strong>it</strong>h<strong>in</strong> exist<strong>in</strong>g systems <strong>and</strong> supervision <strong>and</strong><br />

motivation of staff is an <strong>in</strong>tegral component. As discussed<br />

on Day 3, policy, guidel<strong>in</strong>es, coord<strong>in</strong>ation, advocacy <strong>and</strong><br />

orientation of decision makers has to be underway before<br />

effective tra<strong>in</strong><strong>in</strong>g of front l<strong>in</strong>e workers can take place.<br />

The session was <strong>in</strong>troduced w<strong>it</strong>h a sketch performed<br />

w<strong>it</strong>h the aim of highlight<strong>in</strong>g the importance of <strong>in</strong>fant <strong>and</strong><br />

<strong>young</strong> <strong>child</strong> <strong>feed<strong>in</strong>g</strong> counsell<strong>in</strong>g. The facil<strong>it</strong>ators po<strong>in</strong>ted<br />

out the need to provide qual<strong>it</strong>y counsell<strong>in</strong>g dur<strong>in</strong>g<br />

<strong>emergencies</strong>. Too often ‘counsell<strong>in</strong>g’ consists of<br />

‘messages’ on <strong>in</strong>fant <strong>feed<strong>in</strong>g</strong>, w<strong>it</strong>h the result that health<br />

workers overload mothers w<strong>it</strong>h <strong>in</strong>formation <strong>and</strong> tell them<br />

what to do, rather than listen<strong>in</strong>g, build<strong>in</strong>g confidence, <strong>and</strong><br />

negotiat<strong>in</strong>g behaviour change.<br />

6.2 Tra<strong>in</strong><strong>in</strong>g materials/resources<br />

Two ma<strong>in</strong> tra<strong>in</strong><strong>in</strong>g courses were presented <strong>and</strong> proposed<br />

for consideration. These were:<br />

1. WHO/UNICEF Breast<strong>feed<strong>in</strong>g</strong> Counsell<strong>in</strong>g: A Tra<strong>in</strong><strong>in</strong>g<br />

Course (1993) (BFC) 26 . This is sometimes called the 40-<br />

hour course <strong>and</strong> is available <strong>in</strong> a number of local<br />

languages, <strong>and</strong> several adapted forms. Some participants<br />

had been tra<strong>in</strong>ed us<strong>in</strong>g the course, but others were not<br />

familiar w<strong>it</strong>h <strong>it</strong>.<br />

2. Module 2 <strong>Infant</strong> Feed<strong>in</strong>g <strong>in</strong> Emergencies for health<br />

<strong>and</strong> nutr<strong>it</strong>ion workers <strong>in</strong> <strong>emergencies</strong> (version 1.1,<br />

December 2007), developed by the IFE Core Group<br />

(referred to as IFE Module 2).<br />

BFC is the generic tra<strong>in</strong><strong>in</strong>g course on breast<strong>feed<strong>in</strong>g</strong><br />

counsell<strong>in</strong>g, which is conducted over 5-6 days <strong>and</strong> is<br />

appropriate for health workers both <strong>in</strong> health facil<strong>it</strong>ies<br />

<strong>and</strong> the commun<strong>it</strong>y. It aims to provide both cl<strong>in</strong>ical <strong>and</strong><br />

counsell<strong>in</strong>g skills. It <strong>in</strong>cludes structured exercises <strong>and</strong><br />

practical sessions, <strong>and</strong> has been shown to be effective <strong>in</strong><br />

<strong>in</strong>creas<strong>in</strong>g health worker skills. It has also been shown<br />

that health workers us<strong>in</strong>g these skills to counsel mothers<br />

<strong>in</strong>crease exclusive breast<strong>feed<strong>in</strong>g</strong> rates. The course <strong>in</strong>cludes<br />

a component of tra<strong>in</strong><strong>in</strong>g of tra<strong>in</strong>ers.<br />

IFE Module 2 was developed w<strong>it</strong>h the needs of<br />

emergency relief workers <strong>in</strong> m<strong>in</strong>d. It is a resource of<br />

essential <strong>in</strong>formation for orientation <strong>and</strong> for tra<strong>in</strong><strong>in</strong>g,<br />

particularly when workers need to learn what to do quickly.<br />

It is based on the BFC materials, <strong>and</strong> so complements<br />

them, <strong>and</strong> applies the latest UN policy guidance to<br />

emergency sett<strong>in</strong>gs (e.g. WHO guidance on <strong>in</strong>fant <strong>feed<strong>in</strong>g</strong><br />

<strong>and</strong> HIV). IFE Module 2 also <strong>in</strong>cludes add<strong>it</strong>ional sections<br />

that are especially relevant to s<strong>it</strong>uations seen <strong>in</strong><br />

<strong>emergencies</strong> but where there are gaps <strong>in</strong> ‘official’ guidance<br />

(e.g. artificial <strong>feed<strong>in</strong>g</strong> <strong>in</strong> <strong>emergencies</strong>, <strong>and</strong> manag<strong>in</strong>g<br />

malnourished <strong>in</strong>fants under six months of age). It does not<br />

<strong>in</strong>clude structured exercises or practical counsell<strong>in</strong>g<br />

sessions but does <strong>in</strong>clude practical emergency scenarios to<br />

work through, case studies <strong>and</strong> presentation overheads.<br />

Other relevant materials<br />

IFE Module 1 for Emergency Relief Staff was also<br />

<strong>in</strong>troduced, but hard copies were not distributed to participants<br />

because <strong>it</strong> is outdated (2001), <strong>and</strong> is due to be<br />

updated by the IFE Core Group <strong>in</strong> 2008. However, updated<br />

IFE Module 1 overhead slides were <strong>in</strong>cluded on the<br />

participants CD Rom <strong>and</strong> are also available on the ENN<br />

webs<strong>it</strong>e 27 . IFE Module 1 was developed as a resource for<br />

advocacy <strong>and</strong> orientation of decision makers <strong>and</strong> to support<br />

implementation of the Operational Guidance on IFE.<br />

A module on <strong>in</strong>fant <strong>feed<strong>in</strong>g</strong> <strong>in</strong> <strong>emergencies</strong> is be<strong>in</strong>g<br />

f<strong>in</strong>alised by the Capac<strong>it</strong>y Development Work<strong>in</strong>g Group of<br />

the IASC Nutr<strong>it</strong>ion Cluster, as part of a harmonised<br />

tra<strong>in</strong><strong>in</strong>g package on nutr<strong>it</strong>ion <strong>in</strong> <strong>emergencies</strong>. The IFE<br />

Cluster module has been authored by IFE Core Group<br />

collaborators w<strong>it</strong>h technical review by a number of IFE<br />

Core Group members. A draft was not ready for <strong>in</strong>clusion<br />

<strong>in</strong> Day 4 of the Bali workshop but was highlighted as a key<br />

piloted at the Breast<strong>feed<strong>in</strong>g</strong>: Policy <strong>and</strong> Practice course run at the<br />
