the Scene on <strong>Infant</strong> <strong>and</strong> Young<br />

Child Feed<strong>in</strong>g <strong>in</strong> Emergencies (Day 1)<br />

3.1 Overview<br />

Day 1 comprised a series of presentations <strong>and</strong> plenary<br />

discussions <strong>in</strong>tended to set the scene <strong>in</strong> relation to the<br />

policy guidance that exists for IFE, the extent of the<br />

problem of IFE <strong>and</strong> to share <strong>in</strong>formation on some<br />

<strong>in</strong><strong>it</strong>iatives that are be<strong>in</strong>g implemented which should<br />

contribute to improved support for IFE. Attention was<br />

drawn to the Operational Guidance on IFE that formed<br />

the basis for <strong>and</strong> structure of the meet<strong>in</strong>g.<br />

3.2 Open<strong>in</strong>g comments<br />

The workshop was opened by Bruce Cogill as the Global<br />

Nutr<strong>it</strong>ion Cluster Coord<strong>in</strong>ator, Dr. Ina Hernawati,<br />

Director of Commun<strong>it</strong>y Nutr<strong>it</strong>ion of the M<strong>in</strong>istry of<br />

Health (Indonesia), Dr. Gianfranco Rotigliano,<br />

Representative of UNICEF Indonesia, <strong>and</strong> Dr. Nugroho<br />

Abikusno, Regional Advisor for Nutr<strong>it</strong>ion of WHO South-<br />

East Asia Regional Office (SEARO). All the speakers<br />

emphasised the importance of IFE as an essential<br />

component of an adequate human<strong>it</strong>arian response. They<br />

also, however, lamented the fact that to date, the response<br />

of national governments <strong>and</strong> <strong>in</strong>ternational agencies has<br />

seldom been optimal. Dr. Hernawati <strong>and</strong> Dr. Rotigliano<br />

both c<strong>it</strong>ed the experiences of Indonesia follow<strong>in</strong>g the<br />

tsunami <strong>in</strong> 2005 <strong>and</strong> the Yogjakarta earthquake, when<br />

large amounts of unsolic<strong>it</strong>ed donations of <strong>in</strong>fant formula<br />

<strong>and</strong> baby milks were sent <strong>in</strong> <strong>and</strong> distributed, w<strong>it</strong>h<br />

negative results on <strong>child</strong> health <strong>and</strong> nutr<strong>it</strong>ion.<br />

3.3 Session 1: Policy guidance <strong>and</strong><br />

coord<strong>in</strong>ation<br />

The first session focused on policy guidance <strong>and</strong><br />

coord<strong>in</strong>ation <strong>in</strong> <strong>emergencies</strong>. Karen Codl<strong>in</strong>g, an<br />

<strong>in</strong>dependent consultant work<strong>in</strong>g for the ENN, gave an<br />

<strong>in</strong>troductory presentation on the importance of IFE <strong>and</strong><br />

how this is reflected <strong>in</strong> several global declarations <strong>and</strong><br />

strategies. She then went on to provide an overview of the<br />

Operational Guidance on IFE, which provides the best<br />

available guidance on what to do <strong>and</strong> not do on IFE. Flora<br />

Sib<strong>and</strong>a-Mulder, UNICEF Senior Advisor for Nutr<strong>it</strong>ion <strong>in</strong><br />

Emergencies, then reviewed the experiences to date on<br />

<strong>in</strong>fant <strong>and</strong> <strong>young</strong> <strong>child</strong> <strong>feed<strong>in</strong>g</strong> <strong>in</strong> <strong>emergencies</strong> <strong>and</strong> <strong>in</strong><br />

particular, laid out the responsibil<strong>it</strong>ies of UNICEF, as lead<br />

agency for the Nutr<strong>it</strong>ion Cluster, on IFE.<br />

A key issue that was raised by both presentations was<br />

the l<strong>in</strong>kage between <strong>in</strong>fant <strong>and</strong> <strong>young</strong> <strong>child</strong> <strong>feed<strong>in</strong>g</strong> <strong>in</strong><br />

<strong>emergencies</strong> <strong>and</strong> ‘non-<strong>emergencies</strong>’. IFE often reflects poor<br />

<strong>in</strong>fant <strong>and</strong> <strong>young</strong> <strong>child</strong> <strong>feed<strong>in</strong>g</strong> (IYCF) <strong>in</strong> non-<strong>emergencies</strong>.<br />

Correct<strong>in</strong>g IYCF dur<strong>in</strong>g <strong>emergencies</strong> is therefore doubly<br />

hard because of the difficult cond<strong>it</strong>ions result<strong>in</strong>g from the<br />

emergency <strong>and</strong> the lack of underst<strong>and</strong><strong>in</strong>g, awareness <strong>and</strong><br />

capac<strong>it</strong>y w<strong>it</strong>h<strong>in</strong> the commun<strong>it</strong>y <strong>and</strong> government, for<br />

example, to adequately protect, promote <strong>and</strong> support<br />

breast<strong>feed<strong>in</strong>g</strong> that existed even pre-emergency. Much<br />

work, therefore, needs to be done to improve IYCF <strong>in</strong><br />

general, not only <strong>in</strong> <strong>emergencies</strong>. The po<strong>in</strong>t was made<br />

however, that the IFE response can often raise awareness<br />

<strong>and</strong> pol<strong>it</strong>ical comm<strong>it</strong>ment of the need to do more dur<strong>in</strong>g<br />

non-<strong>emergencies</strong> – <strong>and</strong> may even strengthen national IYCF<br />

programm<strong>in</strong>g as a result. On a related issue, <strong>it</strong> was agreed<br />

that while policies on IFE are needed, <strong>it</strong> was preferable for<br />

them to be <strong>in</strong>tegrated <strong>in</strong>to national IYCF policy or<br />

emergency response policy, or both.<br />

3.4 Session 2: Regional challenges<br />

regard<strong>in</strong>g donations <strong>in</strong><br />

<strong>emergencies</strong><br />

The second session addressed the important issue of<br />

deal<strong>in</strong>g w<strong>it</strong>h donations of <strong>in</strong>fant formula <strong>and</strong> other milk<br />

products dur<strong>in</strong>g the emergency. David Clark, Legal<br />

Advisor <strong>in</strong> the Nutr<strong>it</strong>ion Section <strong>in</strong> UNICEF Headquarters<br />

(HQ), provided <strong>in</strong>formation on the International Code of<br />

Market<strong>in</strong>g of Breastmilk Subst<strong>it</strong>utes (BMS) <strong>and</strong><br />

subsequent relevant World Health Assembly (WHA)<br />

Resolutions (The International Code) 7 . The International<br />

Code is important for protect<strong>in</strong>g <strong>in</strong>fants <strong>and</strong> caregivers<br />

from <strong>in</strong>appropriate market<strong>in</strong>g of breastmilk subst<strong>it</strong>utes<br />

(BMS) <strong>and</strong>, <strong>in</strong> emergency s<strong>it</strong>uations, is especially<br />

important for controll<strong>in</strong>g unsolic<strong>it</strong>ed donations <strong>and</strong><br />

prevent<strong>in</strong>g distribution of unsu<strong>it</strong>able products. The<br />

International Code has been <strong>in</strong>tegrated <strong>and</strong> built upon <strong>in</strong><br />

the Operational Guidance on IFE.<br />

Sri Sukotjo from UNICEF Indonesia provided valuable<br />

quant<strong>it</strong>ative data on the extent <strong>and</strong> impact of uncontrolled<br />

distribution of <strong>in</strong>fant milk <strong>in</strong> Yogjakarta after the 2006<br />

earthquake. She showed clear correlations between receipt<br />
