<strong>com</strong>posite shade should not be selected by plac<strong>in</strong>g a layer of<br />
uncured <strong>com</strong>posite res<strong>in</strong> aga<strong>in</strong>st the teeth or aga<strong>in</strong>st the shade<br />
guide, s<strong>in</strong>ce the <strong>com</strong>posite will change after cur<strong>in</strong>g. On occasion<br />
it may be useful to place a very th<strong>in</strong> layer on the tooth <strong>and</strong><br />
cure it to view before easily remov<strong>in</strong>g it, provided no etch<strong>in</strong>g<br />
or bond<strong>in</strong>g preceded this. The <strong>com</strong>posite material must also be<br />
color stable to avoid <strong>com</strong>promised esthetics over time. 25 When<br />
perform<strong>in</strong>g shade selection, first the hue, then the chroma <strong>and</strong><br />
then the value are chosen. If us<strong>in</strong>g a Vitapan shade guide, this<br />
order will result first <strong>in</strong> choos<strong>in</strong>g from A-D for the hue, then<br />
select<strong>in</strong>g from with<strong>in</strong> that group for the chroma. Lastly, the<br />
value is selected based on degree of lightness/darkness <strong>and</strong><br />
may result <strong>in</strong> a different shade be<strong>in</strong>g viewed as an alternative.<br />

Note that with the <strong>in</strong>crease <strong>in</strong> the number of patients<br />
who have had tooth whiten<strong>in</strong>g, many <strong>com</strong>posite systems<br />
now offer two or three bleach<strong>in</strong>g shades – <strong>in</strong> the case of<br />
patients who regularly <strong>and</strong> heavily whiten their teeth, the<br />
bleach<strong>in</strong>g shades will probably be the best (<strong>and</strong> sometimes<br />
only) place to start.<br />
When us<strong>in</strong>g a <strong>com</strong>posite res<strong>in</strong>, the manufacturer’s<br />
re<strong>com</strong>mended shade guide must be used to ensure the<br />
best match possible of the restoration with the teeth.<br />
Physical Attributes<br />
Key physical attributes for a <strong>com</strong>posite res<strong>in</strong> <strong>in</strong>clude<br />
its <strong>com</strong>pressive strength, flexural strength, modulus of<br />
elasticity, resistance to shr<strong>in</strong>kage, surface hardness, wear<br />
resistance <strong>and</strong> low porosity. These impact the strength of<br />
the material <strong>and</strong> its longevity <strong>and</strong> are important for anterior<br />
<strong>and</strong> posterior restorations, with <strong>com</strong>pressive strength <strong>and</strong><br />
wear resistance be<strong>in</strong>g particularly important <strong>in</strong> posterior<br />
stress-bear<strong>in</strong>g areas. The ability to achieve a high polish, as<br />
well as sta<strong>in</strong> resistance, wear resistance <strong>and</strong> low porosity, are<br />
essential for <strong>in</strong>itial <strong>and</strong> long-term esthetics. A high polish<br />
results <strong>in</strong> a smooth surface that not only feels better to the<br />
patient but also reflects <strong>and</strong> scatters light well for an optimal<br />
appearance (<strong>and</strong> prevents <strong>in</strong>cremental plaque buildup on<br />
rough surfaces). Radiopacity is also important to enable<br />
visualization of restorations on radiographs, <strong>and</strong>, of course,<br />
materials should be bio<strong>com</strong>patible. Key h<strong>and</strong>l<strong>in</strong>g characteristics<br />
are the h<strong>and</strong>l<strong>in</strong>g time, sett<strong>in</strong>g time <strong>and</strong> stick<strong>in</strong>ess of<br />
the material, as well as the ability to pack it (particularly for<br />
posterior restorations), sculpt it <strong>and</strong> polish it to a f<strong>in</strong>e gloss.<br />
All the above must be considered <strong>in</strong> an optimal blend of<br />
physical attributes available <strong>in</strong> the chosen <strong>com</strong>posite.<br />
Simplification<br />
Considerable effort has been made to provide simpler, reliable<br />
techniques with esthetics <strong>and</strong> strength for cl<strong>in</strong>ician <strong>and</strong><br />
patient acceptance.<br />
Adhesive Systems<br />
Bond<strong>in</strong>g systems have be<strong>com</strong>e simpler to use <strong>and</strong> with<br />
reduced chairside time. This has resulted <strong>in</strong> the development<br />
of self-etch bond<strong>in</strong>g systems that will etch <strong>and</strong> bond<br />
<strong>in</strong> one step versus the separate etch<strong>in</strong>g, r<strong>in</strong>s<strong>in</strong>g, dry<strong>in</strong>g<br />
<strong>and</strong> bond<strong>in</strong>g steps of traditional etch-<strong>and</strong>-bond systems<br />
(three-step <strong>and</strong> two-step bond<strong>in</strong>g systems). The choice of<br />
adhesive agent (number of steps/technique) depends on the<br />
situation <strong>and</strong> the <strong>com</strong>posite. Many <strong>com</strong>posite systems have<br />
an ac<strong>com</strong>pany<strong>in</strong>g bond<strong>in</strong>g system that must be used with<br />
that system – if the manufacturer re<strong>com</strong>mends a custom<br />
bond<strong>in</strong>g system, this must be used for reliable results, as<br />
the chemistry of the <strong>com</strong>posite may be <strong>in</strong><strong>com</strong>patible with<br />
generic bond<strong>in</strong>g systems.<br />
