Notes for Applicants
(i) 請 注 意 , 你 須 為 每 一 具 你 管 有 的 輻 照 儀 器 申 領 一 個 牌 照 。
Please note that for possession of an irradiating apparatus, you need one possession licence for each apparatus.
(ii) 請 你 為 擬 管 有 而 尚 未 領 有 牌 照 的 每 具 輻 照 儀 器 申 領 牌 照 , 每 份 申 請 只 供 一 具 儀 器 之 用 。
Please submit a separate application for each irradiating apparatus which you intend to possess but is not covered by a
licence.
(iii) 請 注 意 , 在 輻 射 管 理 局 發 出 使 用 輻 照 儀 器 牌 照 之 前 , 申 請 人 不 得 使 用 , 或 容 許 任 何 其 他 人 士 使 用 輻 照 儀 器 。 如 果 申 請 人 所 申
請 的 牌 照 只 供 貯 存 儀 器 , 或 有 關 裝 置 的 輻 射 安 全 措 施 並 不 足 夠 , 輻 射 管 理 局 督 察 可 能 會 將 輻 照 儀 器 密 封 。
You are not permitted to use the irradiating apparatus, or to allow any other persons to use it before a licence for such
purposes is issued by the Radiation Board. An Inspector of the Radiation Board may seal the irradiating apparatus if you
are applying for a licence to possess the apparatus for storage only, or if the radiation safety of the installation is found to be
inadequate.

假 如 申 請 最 終 被 否 決 , 已 繳 交 的 款 項 將 盡 快 全 數 退 還 。

Licence in the name of a person - Copy of Identity Card/Document of the applicant,
2 recent photographs of size 45 x 60(mm).
用 公 司 名 義 申 請 牌 照 : 商 業 登 記 證 及 獲 授 權 人 士 身 分 證 / 證 明 文 件 的 副 本 。
Licence in the name of a company - Copy of the Business Registration Certificate
Copy of Identity Card/Document of the authorized person.
4. 牌 照 期 限
Licence period
(i) (a) 類 牌 照 , 有 效 期 至 批 出 持 有 供 使 用 的 輻 照 儀 器 牌 照 為 止 , 或 為 期 一 年 , 兩 者 以 較 早 的 日 期 為 準 。
Type (a) Licence is valid for one year, or until a licence to possess the irradiating apparatus in a functional state is granted,
whichever is the earlier.
(ii) (b) 類 牌 照 有 效 期 為 一 年 。
Type (b) licence (for storage) is valid for one year.
(iii)
表 格 中 所 列 的 所 有 有 關 牌 照 必 須 於 呈 交 本 申 請 書 時 仍 為 有 效 。 有 關 牌 照 持 有 人 請 於 牌 照 屆 滿 前 八 個 星 期 把 填 妥 之 申 請 書 及 所
需 文 件 交 到 輻 射 管 理 局 。 否 則 , 本 局 有 可 能 在 牌 照 屆 滿 前 未 能 完 成 處 理 有 關 牌 照 的 續 牌 申 請 。
Any current licence mentioned in this application must remain valid on the date of submission of this application. Licensee
shall submit the duly completed application together with the required documents 8 weeks before expiry of the licence.
Otherwise, the Board may not be able to complete processing of the licence renewal application before its expiry..
