Federal Soldier’s and Sailor’s Civil Relief Act (SSMRA) - Non Resident Servicemen on active duty<br />
in <strong>Connecticut</strong>.<br />
<strong>Connecticut</strong> Servicemen “PA 738” with one <strong>Connecticut</strong> Registered car garaged out <strong>of</strong> state.<br />
Continuing Service - Active duty military forms must be filed each year prior to October 1 st<br />
with <strong>the</strong> Assessor’s Office.<br />
Personal Property declarations for Businesses, Farms and none-registered motor vehicles are required<br />
to be filed annually prior to November 1 st to avoid a 25% penalty.<br />
Applications for Farm & Forest land classifications “PA 490” must be filed no later than 30 days after<br />
October 1 st with <strong>the</strong> Assessor’s Office<br />
Heath Director <strong>Report</strong><br />
The responsibilities <strong>of</strong> <strong>the</strong> public health director include:<br />
Environmental Health (water quality, sewage disposal, restaurant inspections)<br />
Communicable Disease Surveillance (reportable illnesses such as Lyme Disease, tuberculosis, rabies, and<br />
sexually transmitted diseases, are reported to, and monitored by <strong>the</strong> Health Director). In addition, o<strong>the</strong>r<br />
medical diseases such as lead toxicity are also reportable to <strong>the</strong> State.<br />
Maternal and Child Health (<strong>the</strong> town <strong>of</strong> <strong>Salem</strong> contracts with <strong>the</strong> Sou<strong>the</strong>astern CT Visiting Nurse<br />
Association to provide Well Child services for physicals and immunizations from birth to age 5).<br />
Coordination with <strong>the</strong> State Health Department regarding <strong>the</strong> variety <strong>of</strong> health issues which affect <strong>the</strong><br />
town <strong>of</strong> <strong>Salem</strong>.<br />
Attending <strong>the</strong> biannual Health Directors Conferences, sponsored by <strong>the</strong> State Department <strong>of</strong> Health.<br />
Communication <strong>of</strong> town <strong>of</strong>ficials on all public health matters.<br />
<strong>Salem</strong> is a small semi-rural, middle to upper-middle class community with modest industry and farming.<br />
The town’s building department serves as <strong>the</strong> point <strong>of</strong> coordination for all health and environmental<br />
issues. The First Selectman consults <strong>the</strong> Health Director for any issue that requires his/her expertise.<br />
Health and environmental issues that were a concern for July <strong>2010</strong> – June <strong>2011</strong> were:<br />
