How can I keep acne
under control?

Certain skins are genetically
predisposed to acne,
whereas for others, stress,
environmental agents and
hormonal changes are responsible
for if. When the
skin is not cleansed properly,
sebum becomes oxidized,
hardens and prevents the
normal elimination of dead
skin cells. This is why proper
hygiene is so important. In
a beauty institute, aestheticians
can help these afflicted
skins by using a non-foaming
deep-cleansing gel
and a soothing lotion. To
exfoliate, I recommend using
an enzyme-based exfoliant
that biologically digests
dead skin cells without irritating
the skin. In cases of
active acne, avoid extracting
pustules and leave the skin
alone. Treat inflammation
with specialized topical solutions.
It is also preferable to
avoid facial massages; you
can perform lymphatic drainage
instead, which will eliminate
toxins from the body.
Then, choose a purifying and
balancing mask and finish
off with the appropriate protection
cream. (To find out

Transsexualism is a condition in which a person feels that he/she is a member of
the gender opposite to the one assigned to them at birth. To reach full acceptation
of one’s gender, the transsexual person needs to wear clothes typical of the
gender to which he/she self-identifies, to undergo surgery (gender reassignment,
etc.), and, quite often, to undergo permanent hair removal. Facial hair removal
thus becomes a critical phase in the transformation of a transsexual woman,
meaning a biological man that self-identifies as a member of the female gender.
But how do we treat these cases, and which techniques should we choose to
reach better results?

First of all, since beard hair is fairly stubborn,
you need a powerful device, able
to concentrate current at the base of the
follicle to prevent cutaneous reactions
and improve comfort. Today, with the
new ultrarapid 27 MHz technology of the
Apilus ®
Platinum, all of this is possible.
This device delivers peak energy concentration,
excellent results and a high level
of comfort, allowing you to treat more
hair per session.
Second, “the treatment per se must be
fairly quick. For this reason, thermolysis
techniques will be preferred over the
Blend”, confirms Mrs. Kelly Morrissy.
Today, the most appropriate thermolysis
technique for beard hair is without a
doubt the Synchro. This new technique,
available on the Apilus ®
Platinum, consists
in the synchronized application of a
hundred or so micropulses of ultrarapid
PicoFlash current (0.002 second each),
combined with a movement from the
bulge to the bulb. It is extremely effective
for deeply rooted, stubborn hair,
and delivers quick results since it is able
to completely destroy germinative cells
from the bulge to the bulb. Moreover,
given the extremely quick micropulses,
the method is safe and comfortable.
For superficial hair or hair weakened by
the Synchro technique, it is recommended
to choose the PicoFlash technique
(ultrarapid 27 MHz pulse – twice
as rapid as MicroFlash). Moreover, the
dual-pulse technique with PicoFlash
may prove very useful for stubborn hair.
According to Mrs. Johanne Fortier, it is
best to deliver the first pulse in the bulge
area and the second in the bulb area.
However, if the insertion is superficial,
then both pulses should be delivered at
the base of the follicle. It is important
to remember to use insulated probes,
and to make sure the insulated portion
remains in the follicle despite the movement
in order to protect the skin surface.
Moreover, to guarantee better comfort
and appropriate energy level when using
this technique, the Apilus ®
Platinum
offers a "proportional pulsing" function,
which automatically reduces the length
of each pulse.
Finally, the MultiPlex is the ideal
thermolysis technique to treat distorted
hair. Thanks to its warming process
(low-intensity slow thermolysis), the final
PicoFlash pulse will achieve greater
destruction, covering a wider zone at
the base of the follicle. The MultiPlex
can also be used with the dual-pulse
technique.
For those of you who prefer the Blend,
we suggest the MultiBlend technique,
also available on the Apilus ®
Platinum.
The MultiBlend combines a continuous
galvanic and thermolysis current, with a
final PicoFlash pulse, for superior effec-

tiveness. However, although the Blend
method is highly effective for distorted
hair, it could appear longer for your
client. Remember that the Blend requires
at least 4 or 5 seconds for each hair, compared
to a few thousandths of a second
in ultrarapid thermolysis. Thus, not as
many hair can be treated per session,
which might be discouraging. However,
in the long run, the total treatment time
may indeed be lower, given the highly
destructive power of lye. If you decide
to use this technique, please make sure
your client shows no contraindications to
galvanic current. Mrs. Fortier also reminds
us that “the Blend should only be used on
deeply rooted hair in the anagen phase,
since the galvanic current needs a highmoisture
environment to generate lye.
You will need to use another thermolysis
technique such as PicoFlash to treat the
remaining hairs.”
When treating stubborn hair in the facial
area, as is the case for transsexuals, you
must imperatively use insulated probes
according to Mrs. Fortier. Why? Because
you need to concentrate a fair amount
of energy at the base of the follicle, while
protecting facial skin. For superficial
insertions, the Synchro or dual-pulse
technique, choose IsoGard probes by
Pro-Tec ® , where only the tip is bare. For
deeply rooted hair or if you opt for the
Blend method or PicoFlash, MicroFlash
or MultiPlex with one or two pulses,
then IsoBlend probes by Pro-Tec ®
are
an excellent solution, since the noninsulated
portion is longer (see “Dectro
Specials”). And please, give yourself
three minutes at the end of the session to
perform cataphoresis on your client. This
will help soothe redness, tighten pores
and prevent infection. These clients need
it more than any other!
Of course, we must also consider the
fact that nowadays, certain transsexuals
will prefer laser treatment. Mrs. Morrissy
reveals that laser or IPL (intense pulsed
light), by reducing pilosity more quickly,
can accelerate the transition from
one gender to the other. However, to
achieve permanent results, transsexuals
have no other choice but to combine the
treatment with electrolysis, since it is the
only effective method to permanently
eliminate stubborn hair and barely pigmented
hair. Moreover, if your client has
blond, white or red hair or if her skin is
dark, then laser is simply not effective;
electrolysis is the only option available.
Based on her extensive experience, Mrs.
Morrissy mentions that regardless of the
preferred option, the amount of money
and time invested are practically identical.
The most important thing is to carefully
examine your client’s skin and pilosity to
recommend the most appropriate treatment
for her.
In conclusion, Mrs. Morrissy shares some
of her trade secrets: when treating facial
hair with electrolysis, it is preferable to
start with the most sensitive areas where
the hair density is greatest, namely the
chin and the lip contour. The client’s
tolerance level is always greater at the
start of the treatment, and once this area
is treated, the rest will progress rapidly,
which is more encouraging. Moreover,
hormone therapy, which all transsexuals
must undergo, will make your work easier
since it reduces pilosity and softens the
skin. Then, once your client has undergone
gender reassignment surgery (removal of
testes and genital reconstruction), pilosity
will again be reduced, making it that much
easier to achieve permanent hair removal.
In summary, a powerful and rapid device
such as the Apilus ® Platinum, techniques
suitable for tough, stubborn hair, the use
of insulated probes, and a determined
client who faithfully attends each appointment,
are all elements that will guarantee
successful permanent hair removal for
transsexual women. And these clients
are certainly determined, since permanent
hair removal is an essential step in
achieving complete transformation. Once
all these elements are combined, all you
need to do is to keep the lines of communication
open with your client, encourage
her, and constantly observe the treated
area to adapt your technique or your
parameters as needed. ♦

Having overabundant sebaceous secretions
is an unfortunate condition that
many must learn to deal with. Those
with lipidic skin generally do not appreciate
the temperamental nature of their
skin. Unsure of how to deal with the problem,
they often try all sorts of products
that are not always suited for their skin
type. For this reason, it is important for
us beauty professionals to fully understand
the causes and consequences of
overabundant sebaceous secretions, in
order to offer adequate solutions to this
unhappy clientele.
Many systems of the human body and
external factors can affect the activity of
sebaceous glands. Here are the main causes
to consider:
The endocrine system: hormones secreted
by gonads, primarily progesterone
and androgen in women and testosterone
in men, stimulate sebaceous gland activity.
The endocrine system undergoes
many significant changes over the course
of one’s life: at puberty, during pregnancy,
at menopause in women and andropause
in men. Medication intake, namely oral
contraceptives or hormones, also modifies
the state of sebaceous glands.
The digestive system and the liver: the
digestive system and lipid management by
the liver influence the quality of sebum. A
diet rich in saturated fats negatively impacts
the chemical composition of sebum, made
up of triglycerids, wax, free fatty acids, scalene,
and cholesterol. When the compositional
balance is disrupted, then all the skin
barriers become disorganized. The hydrolipid
layer, the hydrogen potential (pH) and
the lipid-protein cenapse are all affected by
variations in the quality of sebum. Without
these barriers, the skin becomes vulnerable
to external aggressions and to unwanted
micro-organisms. Sufficient water intake is
also necessary to maintain proper hydration
for adequate cellular activity. Certain vitamin
deficiencies can also have an impact,
for example vitamin A deficiency may cause
hyperkeratinization, which ultimately leads
to obstruction of pilo-sebaceous orifices,
or vitamins B2, B6 and H, which may lead
to seborrhoea, whereas iodine stimulates
sebaceous gland activity.
The nervous system: the nervous system
indirectly influences sebaceous gland output.
Situations that generate permanent
or excessive stress often lead to ailments of
the follicle.
Retinoids: the use of retinoids such as
Accutane ® is highly irritating for lipidic skin
and should only be prescribed for genuine
cystic acne problems. Some potential complications
include increased dehydration
of the skin and mucous membranes, hair
loss, nausea, headaches, vision problems,
muscle pain and exacerbated photosensitivity.
Moreover, in the U.S., the FDA recently
recommended the addition of a warning
concerning the possibility that this medication
may lead to serious depression.
Cosmetics: aggressive products and detergents
shift the skin’s pH level in the alkaline
range. Lipidic skin is generally already
slightly alkaline, which makes it even more
vulnerable to microbial invasions. It is therefore
preferable to choose slightly acidic
cosmetics to help maintain the integrity of
the horny layer.

First of all, it is very important to explain
to these clients how critical daily hygiene
is to improve their situation. However,
this part of their daily routine requires
extra gentleness and respect, since their
skin is reactive and vulnerable to inflammation.
A quick morning and evening
cleansing routine with cleansing milk or
non-foaming gel is generally more than
enough. Avoid regular soap, foaming
gel and surfactants, since they are very
irritating and trigger excessive sebum
production. To rinse, use disposable compresses
rather than a facecloth that tends
to collect bacteria, and avoid rubbing the
face needlessly.
Thorough cleansing should always be
followed by a lotion. For lipidic skins,
choose a soothing, antiseptic or
balancing lotion to reach full imbibition
of the horny layer. It will restore the pH
level and enhance the penetration of
subsequent corrective cosmetics.
Botanical enzyme or AHA-based exfoliation
is strongly suggested twice a
week. Enzymes or AHAs are preferable
to mechanical exfoliation (with granules),
which tends to assault skin tissues.
Followed by a mask, this short home
care routine is sufficient to maintain the
skin’s water-lipid balance. In your salon, a
light microdermabrasion treatment with
the VitaPeel may be performed once
a month to refine the skin texture, often
visible in lipidic skins, but only if the skin
does not show an acne condition.
Water-based specific care products
should also be used to address specific
problems associated with lipidic skins.
Choose products that can effectively
reduce inflammation, control sebaceous
gland activity, and provide moisture. If
you use essential oils, apply them only
in the evening to avoid photosensitive
reactions.
Many clients feel it is not necessary to
moisturize lipidic skin. You will need to
make them realize that although this skin
type has an overabundance of sebum,
it is almost always strongly dehydrated
(lack of water), and when the skin is
dehydrated, its glands secrete even more
sebum! Moreover, since it is very delicate,
it should be protected against external
aggressions. Your client must therefore
learn to protect her skin every morning
with a matifying, photo-protecting,
moisturizing and antioxidant cosmetic
to prevent the formation of blackheads.
Also, the use of powder-based make-up
adds extra protection and helps alleviate
the unpleasant oily aspect that often
appears during the day.
A monthly appointment in your salon will
be useful to avoid pimple and blackhead
invasions. The presence of comedones
can ultimately lead to microcysts and
acne papules and pustules if they are not
dealt with. If this is the case, then your
duty is to recommend a one-month cure
of at least four treatments.

Salon treatments using continuous current
are particularly recommended for
this skin type. Anaphoresis, for example,
is used for electrical deep-cleansing,
which helps eliminate excess sebum,
loosen pores (facilitate comedo extraction)
and stimulate circulation and tissue
oxygenation. Cataphoresis on the other
hand, soothes redness, tightens pores
and restores the pH level. Moreover, ionization
of a polarized cosmetic suited for
lipidic skins can help restructure the skin
barriers.
Manual or electrical lymphatic drainage
with the VitaLift is an essential massage
to combine to your lipidic skin facial,
since it contributes in eliminating toxins
from the connective tissue and improves
cellular renewal. As well, Vital•O 2
treatments with pure oxygen are strongly
recommended for this skin type given
the powerful antibacterial potential of
oxygen (see “Dectro Specials”).
For those of you who have access to
a light therapy device such as the
Vita•Lux, blue light can be used to control
microbial invasions, whereas green
light helps relieve inflammation. As for
masks, gel-based masks or collagen
sheets are the most appropriate for this
particular skin type, to keep manipulations
to a minimum for these delicate
skins.
Fortunately, lipidic skin is not only an
inconvenience! It offers undeniable
benefits compared to other skin types. Its
sub-cutaneous tissue is generally thicker
and stronger, which delays the appearance
of fine lines and wrinkles. So let’s
avoid assaulting these skins needlessly
and instead help preserve their integrity,
so they can age in a healthy and beautiful
fashion. ♦

he growing trend for smooth, hair-free
skin is here to stay, and teenagers are
first in line when it comes to removing
unwanted hair. As fine and light-coloured
as it may be, hair is simply not tolerated.
Young girls compare with each other; one
friend shaves, the other doesn’t. One sees
an aesthetician for waxing while the other
has the privilege of undergoing electrolysis.
One question remains in the minds
of many parents: at what age should we
allow teenagers to begin hair removal,
and which method is best?
here is no ideal age to begin hair removal;
the best course of action is to discuss
the subject openly with teenagers before
they pick up a razor in the bathroom and
shave all unwanted hair with unfortunate
consequences. Here are a few methods of
temporary hair removal, pilosity reduction,
and permanent hair removal available
to today’s teenagers. We qualify certain
techniques of “temporary”, because in
these cases, hair is bound to grow back
in the short or medium term, and it may
even become darker and stronger than it
was before.

Last September, Mr. Clément Beaumont, President of Dectro International, and Mrs. Cinthia Caron,
instructor at the Institut Dectro, travelled to Jidda, in Saudi Arabia. They attended the opening of Dectro
Saudi and offered training to the current team, but also gave a number of demonstrations to doctors in
Jidda and Riyadh. Even prior to the arrival of Mr. Beaumont and Mrs. Caron, the staff at Dectro Saudi had
good working knowledge of Apilus ® , VitaPeel, Vitaderm ® and Vital•O2 devices. The team showed a
high level of professionalism and warmly greeted their Canadian visitors. We wish them good luck in their

AEA Convention
From October 19 to 22, 2006, the American Electrology Association (AEA) held its annual meeting in
Newark, New Jersey, just outside of New York City. This year as in previous years, Dectro International and
Dectro California were there to meet their esteemed clientele. Mr. Clément Beaumont, President of Dectro
International and an expert in electrolysis, also gave an informative seminar on new hair removal techniques
by electroepilation and laser. The talk was highly appreciated by all AEA members! We would like to take
