<strong>William</strong> <strong>Jewell</strong> <strong>College</strong><br />

B e n e f i t P l a n S u m m a r y<br />

<strong>William</strong> <strong>Jewell</strong> <strong>College</strong> offers employees a comprehensive benefit package. To be<br />

eligible for part-time benefits you must be a regular employee (not seasonal or<br />

temporary) working more than 1040 hours per year. To be eligible for full-time benefits<br />

you must be a regular employee working more than 1560 hours per year.<br />

Insurance coverage for full-time benefits becomes effective 30 days after the date of<br />

hire provided you are actively at work and enroll within 30 days of your date of hire.<br />

<strong>Benefit</strong>s for part-time employees begin after a 6 month waiting period. After the initial<br />

enrollment, changes to your enrollment will only be allowed during the annual open<br />

enrollment period becoming effective on January 1 unless you have a qualifying event.<br />

All changes must be made within 30 days of a qualifying event, becoming effective on<br />

the date the qualifying event occurred.<br />

Open Enrollment for <strong>2013</strong> <strong>Benefit</strong>s<br />

Your annual enrollment period for the <strong>2013</strong> plan year opens November 2 and runs<br />

through November 20, 2012. All changes to your benefits must be made during this time.<br />

All benefit eligible employees must return a <strong>2013</strong> FLEX election form. All other benefits<br />

will continue as previously elected unless you submit a change by filling out the proper<br />

forms and turning them into the Office of Human Resources by November 20, 2012.<br />

This Summary of <strong>Benefit</strong>s is designed to give you an overview of the benefits offered to<br />

benefit eligible employees of <strong>William</strong> <strong>Jewell</strong> <strong>College</strong> for <strong>2013</strong>. The benefit plans are governed<br />

by legal plan documents and <strong>College</strong> policies. In the event of a conflict between this<br />

summary and a plan document, the terms of the plan document will be the final authority.<br />

In the event of a discrepancy between this brochure and the plan document, the plan document will prevail. Possession<br />

of this document is not a guarantee of coverage.<br />
