OUR MISSION<br />

The mission of <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong> is to provide comprehensive, holistic, community based support service<br />

and care for the terminally ill persons, their caregivers and loved ones without regard to economic status, to enable the<br />

dying person to live peacefully, in comfort, and with dignity to the last moment of life.<br />

OUR LOGO<br />

The circular shape surrounding the design symbolizes the continuity of care provided by <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong><br />

<strong>Hospice</strong> and also symbolizes eternity. The unique combination of people who initiated and continue to make <strong>Joliet</strong><br />

<strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong> a reality is represented by the triangle: Provena Saint Joseph Medical Center, Silver Cross<br />

Hospital, and the community. The three pointed design within the triangle represents the team of volunteers,<br />

professionals, and family members who provide care and can also be considered as a bridge to eternity for the<br />

patient who is symbolized by the solid circle at the center of the logo.<br />

OUR HISTORY<br />

<strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong> (JACH) has been<br />

providing compassionate, professional care to the<br />

terminally ill patients and their families since 1982. We<br />

are a not-for-profit hospice, community based, state<br />

licensed and Medicare/Medicaid Certified. Over 25,000<br />

patients have received our superior care and service since<br />

our incorporation in 1982. <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong><br />

<strong>Hospice</strong> is a long term United Way partner of Will and<br />

Grundy County.<br />

1982- <strong>Hospice</strong> becomes available in the <strong>Joliet</strong> area<br />

through the combined efforts of St. Joseph Medical<br />

Center, Silver Cross Hospital and several community<br />

groups and individuals. <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong><br />

becomes the first hospice established in the community.<br />

1983- Rosemary Bowe is named Executive Director and<br />

opens the initial office located at 333 N. Hammes. The<br />

first group of hospice volunteers received training in<br />

March. <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong> admitted its first<br />

patient onto the Bethany Unit at St. Joseph Medical<br />

Center on April 1st. By December, 127 patients had<br />

been served.<br />

1984- The <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong> Guild<br />

Volunteer fundraising group was formed.<br />

1985- By the end of the year, over 480 patients had<br />

received hospice services. <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong><br />

<strong>Hospice</strong> becomes member of United Way of Will County.<br />

1986- <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong> receives full<br />

licensing from the State of Illinois Department of Health.<br />

Mary Jo Smith becomes the second Executive Director<br />

with 2 additional staff members. In August, the offices<br />

were moved to the Marycrest Shopping Center at 2112<br />

W. Jefferson St.<br />

1986- January - Over 820 patients received services<br />

with an average daily census of 27 patients.<br />

1990- In October, <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong><br />

becomes the first hospice in Will County to qualify as a<br />

provider of Medicare hospice benefits. Over 1,200<br />

patients have received hospice services.<br />

1990- Moved into new offices donated by Jack &<br />

Virgina Rogers in 1989 at 335 W. Jefferson St.<br />

Volunteers spent close to a year renovating the space<br />

with donated services from the City of <strong>Joliet</strong>, Cliff Bender,<br />

architect, and many others to make it functional for the<br />

needs of the organization. Berryman Transfer volunteered<br />

their time to complete the move.<br />

1992- In December, Duane Krieger becomes the third<br />

Executive Director.<br />

1994- Almost 2000 dying patients and their families<br />

have been served by <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong><br />

since opening in 1983.<br />

1999- The Board of Directors begins pursuing a dream<br />

of building the first freestanding <strong>Hospice</strong> Home in Illinois.<br />

2001- A $3 million capital campaign begins to raise the<br />

funds to build the <strong>Hospice</strong> Home.<br />

2002- In September, a groundbreaking ceremony is<br />

held at the site of the <strong>Hospice</strong> Home.<br />

2003- Over 10,000 patients have been served by 110<br />

employees and 200 active volunteers with a daily census<br />

of 140 patients.<br />

2004- In May, the <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong><br />

Home opens, becoming the first freestanding hospice<br />

home in Illinois.<br />

2007- Having served over 12,000 patients with an<br />

average daily census of 165 patients and over 1,200<br />

families using bereavement services, <strong>Joliet</strong> <strong>Area</strong><br />

<strong>Community</strong> <strong>Hospice</strong> continues to grow with over 150<br />

staff members and 250 active volunteers.<br />

2009- Rick Kasper is named and becomes the 4th CEO<br />

of <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong>.<br />

2012- Over 20,000 patients have been cared for by<br />

<strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong> since 1983.<br />

2012- JACH celebrates its 30th year of service to Will,<br />

Grundy, Kendall, Cook, Kankakee, and Dupage<br />

Counties.<br />

MESSAGE TO THE COMMUNITY<br />

Happy Anniversary <strong>Joliet</strong> <strong>Area</strong><br />

<strong>Community</strong> <strong>Hospice</strong><br />

In 1983 <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong> accepted our<br />

very first patient. Now 30 years later we have provided<br />

quality end of life care to over 25,000 patients and have<br />

assisted many more through their grief.<br />

It is a privilege to work with a hospice team of this<br />

caliber. We have so many long term staff. In an industry<br />

where the average tenure is two years, <strong>Joliet</strong> <strong>Area</strong><br />

<strong>Community</strong> <strong>Hospice</strong> has tenure of more than three times<br />

that. We have long standing relationships with hospitals,<br />

assisted living facilities, nursing homes, and families.<br />

Despite the many changes in the healthcare environment<br />

that have and will continue to take place over the coming<br />

years, we are holding true to our values. We will continue<br />

to do what’s best for our patients and families. We will<br />

continue to provide comprehensive, holistic, community<br />

based support and care for terminally ill persons, their<br />

caregivers, and loved ones without regard to economic<br />

status. That is what has driven us for the past thirty years<br />

and that will help guide us through the unknown waters of<br />

healthcare reform that lay ahead of us.<br />

It's what our staff does every day for our patients. It is<br />

how we transform despair into hope. We call this power<br />

"the Mayo Effect." Working together as one organization<br />

with one purpose, one direction, one plan, we strive to<br />

turn the impossible into the possible.<br />

So what does the future look like for <strong>Joliet</strong> <strong>Area</strong><br />

<strong>Community</strong> <strong>Hospice</strong>? I feel it is strong. More people are<br />

better educated about their healthcare options. This<br />

means that more people are choosing hospice care over<br />

other forms of healthcare because of our holistic,<br />

community based support services. This means that no<br />

matter what we face in the future, we know that making<br />

sure we take care of our patients and families and<br />

continue to do the right thing; we are making a difference<br />

right here where it matters to us.<br />

VOLUNTEERS<br />

Our volunteers always rise to the occasion with<br />

enthusiasm, a smile, and a caring heart. Whether they<br />

are sitting vigil, providing direct care, helping in the<br />

hospice home, gardening, tending to a veteran, or being<br />

community ambassadors, they can always be counted on<br />

to go above and beyond the call of duty for <strong>Joliet</strong> <strong>Area</strong><br />

<strong>Community</strong> <strong>Hospice</strong>. Take Genevieve Vranicar for<br />

example (pictured to the right). She is 95 years young<br />

and shows up for work faithfully each week to tackle<br />

each task that is presented before her. She’s never late<br />

and is always on time. The remarkable thing is that these<br />

efforts are not unique to just Genevieve, the same can be<br />

said for all of our 260 volunteers. Below is an example of<br />

SOCIAL SERVICES<br />

Special Services, performed by special people, that<br />

routinely go beyond the requirements for hospice. Our<br />

Social Work team provides an amazing array of support<br />

and joy to the patients, families and communities in<br />

which <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong> has been proudly<br />

serving for 30 years.<br />

Services provided include:<br />

Counseling, Bereavement, Support groups,<br />

Music Therapy, Massage Therapy,<br />

Pet Therapy, and Art Therapy.<br />

CLINICAL<br />

Providing hospice care is a calling and not just a<br />

profession. The requirements for each patient are often<br />

different. Our clinicians must be sensitive to needs of the<br />

patients and their families, experts in hospice care, all the<br />

while possessing the ability to adjust to situations at a<br />

moment’s notice. Thankfully these are skill sets that the<br />

clinicians at <strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong> possess.<br />

Whether patients are being cared for in a skilled nursing<br />

facility, assisted living facility, private residence, the<br />

hospice home or in our pediatric program they will<br />

always receive superior compassionate hospice care that<br />

is second to none.<br />

“To have a child with a life limiting illness is overwhelming but<br />

<strong>Joliet</strong> <strong>Area</strong> <strong>Community</strong> <strong>Hospice</strong> provided the entire family with<br />

love, care and support to manage through the final short<br />

months of Peyton’s short life. We will never forget the support<br />

they were able to provide for us to enjoy the time we had.”<br />

THE HOSPICE HOME<br />

The hospice home is just that “A Home”. A home that is<br />

designed to provide 24 hours/7 days a week of short<br />

term care to terminally ill patients who are experiencing<br />

The word hospice is often mistakenly thought of as being synonymous<br />

with death but in actuality it’s just the opposite. Its about living.<br />

JACH has been the final stop in life’s journey for many people.<br />

But this summer it was the beginning. We had our 1st wedding.<br />

THE LIVES WE’VE<br />

TOUCHED IN 2012<br />

My father was at Sunny Hill Nursing Home. He made a turn for the worse, so we called <strong>Joliet</strong> <strong>Area</strong><br />

<strong>Community</strong> <strong>Hospice</strong>. The care my dad received was over the top. I had a great peace. I knew he<br />

was being very well taken care of. They played music for him, got special things for his legs. If he<br />

had pain they called the Doctor right away. They treated him with dignity and he received the best<br />

care. His last days and hours were very pleasant.<br />

I am forever grateful and honored to say thank you.<br />
