Test Principle<br />
Our <strong>LUCIO®</strong>-<strong>Medical</strong> and Sero <strong>ELISA</strong> are all based<br />
on the principle of antibody-antigen binding,<br />
which results in an extremely high sensitivity.<br />
Thereby, specially developed monoclo<strong>nal</strong> antibodies<br />
(MAb), which specifically recognize an<br />
epitope of an antigen, are used causing a very<br />
high specificity.<br />
To be able to quantitatively evaluate the extent<br />
of the antibody-antigen binding, an enzyme<br />
(mostly horseradish-peroxidase) changing a<br />
colorless substrate into a colored product, is<br />
used. Depending on the issue in question or on<br />
the molecule of interest, this principle is performed<br />
in different ways.<br />

1. Direct Detection of the Antigen<br />
Antibodies against the antigen of interest are<br />
bound to the microtiter plate. The binding of<br />
the antigen is visualized either by means of<br />
an antibody-enzyme conjugate (Sandwich-<br />
<strong>ELISA</strong>) or by a competing antigen-enzyme<br />
conjugate (competitive <strong>ELISA</strong>).<br />

2. Direct Detection of the Antibody<br />
Antigens are bound to the microtiter plate.<br />
Antibodies of interest from the sample (IgG,<br />
IgM or IgA) bind to this antigen. The binding<br />
is visualized by using an antibody-enzymeconjugate<br />
that recognizes all bound human<br />
antibodies. Since IgG-antibodies are present<br />
in abundance, they would superimpose the<br />
detection of IgM and IgA antibodies - for this<br />
reason, an <strong>ELISA</strong> sorbent, which removes all<br />
IgG antibodies from the sample, has to be<br />
used to detect IgM and IgA antibodies.<br />

3. Indirect Detection of IgM-Antibodies<br />
(µ-capture)<br />
For the indirect detection of IgM-antibodies<br />
by means of a µ-capture, firstly all IgMantibodies<br />
in the samples are bound to<br />
anti-IgM antibodies on the microtiter plate.<br />
The specific detection of the IgM-antibodies<br />
of interest is made by the binding of an<br />
enzyme-linked antigen. This method does<br />
not require the use of a sorbent.<br />

Autoimmune diseases are diseases<br />
of the immune system where endogenous<br />
body tissue is erroneously<br />
attacked by the immune system. The<br />
detection of these antibodies can indicate the<br />
existence of possible diseases. The classification<br />
of antibodies also enables the determination of<br />
the affected organ. In such cases, the rapid and<br />
reliable results of the Enzyme-Immunoassays<br />
are necessary to initiate the correct treatment.<br />
Naturally, a systematic therapy is preferred to a<br />

hormones. The synthesis of glucocorticoids (cortisol,<br />

Endogenous messengers stimulating<br />
cell division and thus growth are<br />
designated growth factors. The<br />
measurement of those substances<br />
(mostly hormones or peptides) in serum is important<br />
for various reasons. It can confirm the<br />
diagnosis and therefore initiate the treatment<br />
of growth disorders with children and youths.<br />
Another possible application is the monitoring<br />
of the endocrinologic disease Acromegaly by the<br />
determination of the polypeptide IGF-I. Diseases<br />
as AIDS or malignant tumors as e.g. glioblastoma<br />
lead to an increase of TGF-βI. Therefore, the<br />

provided by <strong>nal</strong> <strong>von</strong> <strong>minden</strong>, for the serological diagnosis of infectious agents in human serum<br />
Introduction<br />
Enzyme-linked Immunosorbent Assay (<strong>ELISA</strong>) is a standard<br />
method for the serological detection of pathogen specific<br />
antibodies. The diagnostic sensitivity (proportion of actual<br />
positive samples which are correctly identified) and diagnostic<br />
specificity (proportion of negative samples which are<br />
correctly identified) are important parameters during the<br />
validation process of assays intended for routine laboratory<br />
diagnostic use.<br />
In this study, the efficiency of three different <strong>ELISA</strong> kits was<br />
evaluated by comparing the diagnostic sensitivities and<br />
specificities with those of benchmark assays. All tests were<br />
performed at the Department of Virology, Institute of <strong>Medical</strong><br />
Microbiology and Hygiene, University of Regensburg,<br />
Germany (RIMMH).<br />
Study design and results<br />
The study comprised testing of the following <strong>ELISA</strong> kits and<br />
their corresponding parameters; the focus was on the detection<br />
of specific antibodies against pathogens which can be<br />
relevant during pregnancy:<br />
Chlamydia pneumoniae (IgG, IgA), Chlamydia trachomatis<br />
(IgG, IgA), CMV (IgG, IgM), EBV EA (IgG), EBV EBNA-1 (IgG),<br />
EBV VCA (IgG, IgM), HSV-1/2 (IgG, IgM), Measles (IgG, IgM),<br />
Mumps (IgG, IgM), Rubella (IgG, IgM), Toxoplasma gondi<br />
(IgG, IgM), Treponema pallidum (IgG, IgM), VZV (IgG, IgM).<br />
The study was performed in two steps. First, patients‘<br />
frozen sera were tested by three different immunoassays<br />
provided by <strong>nal</strong> <strong>von</strong> <strong>minden</strong> in parallel with the corresponding<br />
comparator assays. In a second step, discrepant<br />
results (5–15 samples per parameter) obtained by the best<br />
performing <strong>ELISA</strong>s were repeated and tested against the<br />
comparator.<br />

Conclusion<br />
In consideration of several aspects (e.g. test performance<br />
and handling) <strong>ELISA</strong> kits provided by <strong>nal</strong> <strong>von</strong><br />
<strong>minden</strong>, were successfully evaluated in this study, with<br />
focus on detection of specific antibodies against pathogens<br />
relevant during pregnancy. The tests are easy to<br />
use and handle, in accordance with the description provided<br />
within the package inserts. Moreover, the packaging<br />
of the <strong>ELISA</strong> kits is suitable for storage and is made from<br />
recycled and reusable material.<br />
In conclusion, the mostly high agreement of diagnostic sensitivity<br />
and specificity for all parameters tested indicates<br />
that the diagnostic performance of the provided <strong>ELISA</strong> kits<br />
is comparable to the comparator assays used in this study.<br />
