<strong>RELEASE</strong> <strong>FORM</strong><br />
The Cultural Preservation Department of the <strong>Absentee</strong> <strong>Shawnee</strong> <strong>Tribe</strong> of <strong>Oklahoma</strong> will be<br />
sponsoring activities to promote the learning of our language and culture. The event will<br />
occur from Friday, September 28, 2012 to Sunday, September 30, 2012, at the Resource<br />
Center on Peebley Rd., in Little Axe. Parents are strongly encouraged to attend and<br />
participate. All children 12 and under must be accompanied by a parent or appointed<br />
guardian.<br />
Each participant must have a release form on file, in order to attend. No exceptions.<br />
Please submit forms to the Cultural Preservation Department by Thursday, September 26,<br />

Each participant agrees to follow any and all rules and regulations set forth while attending<br />
the Immersion Camp. As consideration of my selection, I agree to not hold liable the<br />
<strong>Absentee</strong> <strong>Shawnee</strong> <strong>Tribe</strong>, Cultural Preservation Dept. staff and/or volunteers for any<br />
injuries which may result from accidents during the attendance of this event.<br />

The weekend activities will focus on different aspects of <strong>Shawnee</strong> culture – language,<br />
genealogy, history, with plant and animal identification. Please dress appropriately for the<br />
conditions, as we will be outdoors for parts of the instruction.<br />
