• In daily practice, it is common to find acute
cases where the most remarkable symptoms
belong “to the disease”
• Nevertheless, even in such cases it is possible
to individualize, when we take into account
minimal variations regarding the typical
presentation of the disease

• 55 year-old patient, suppurated middle ear
effusion after exposure to a draft of air
• Took ATB for 4 days, without improvement
• Cannot hear on the left side, then pain began and
extended to the right side. Noises in left ear “as if
by a compressor” and disagreeable feeling of
distension in ears
• Pain began suddenly, all over the left side of the
head, “terrible”, as in migraine; it irradiates to the
face and the jaw; agg. by minimal touch

• Feels wetness in the ear, whereas
there is not copious secretion
• But feels the secretion in the
fingers, when she scratches
•• Itch in meatus
•.
•Yellow, adhesive secretion
•Congestion of tympanic
membrane

1. Onset resembling migraine, of neuralgic
intensity
2. Yellow, adhesive secretion, associated to itch;
keeps boring the ear with the finger
3. Tinnitus + pain

• 8 year-old girl, ear infection 3
weeks earlier treated with ATB.
Several episodes the previous
year, always on the left side.
• Awaken in the night by strong
pain, stitching and tearing, amel.
temporarily by the heat of the
pillow
• Before onset, had vomiting and
after, diarrhea, with pain and
noises in the abdomen
• Infections always begin during the
night
• During crises, becomes pale, nose
and ears cold; fever from
moderate to low; pain agg. on
swallowing

• Turns around in bed, screaming out of pain
• Congestive tympanic membrane, with
peritympanic vesicles, on a congestive
background

Individualizing traits
1. Left side
2. Paleness, even during fever
3. Cold nose
4. Concomitants: abdominal
pain, vomiting, diarrhea
5. Local: inflammation +
vesicles
Remedies
• Mercurius
• Aconitum
• Belladona
• Chamomilla
• Rhus-t
• Arsenicum

• Within the context of nightly onset,
• Concomitance of abdominal symptoms become
more significant.
• On the other hand, concomitance of other painful
manifestations with earache is characteristic of
Ars (Lach, Spig)
• Vesicular eruptions in ears: alum., ars., meph.,
nat-m., Olnd., phos., Psor., ptel., rhus-t., rhus-v.,
sep., Tell.
– EAR - ERUPTIONS - vesicles - surrounded by inflamed
base: ars.

• “Bifid tongue”

• Inflammation of
conjunctiva, but
more of the lids
• In the morning, eyes
glued by secretion –
whitish -, esp. in the
angles, esp. the
