Figure 15 looks complicated when first looked at; but in fact the only things
defined here are the section names and whether they should be ranked or
unranked. The contents are the entries defined earlier; they simply need to be
picked from the dropdown.
The toolkit makes the process itself very simple; the hardest part is working
out which entry belongs to which section. We can see that the first section contains
two entries, the clinic date and the reason for visit.
When this was done, the exact process was repeated to assign the various
sections to the appropriate forms (Figure 16). Again, this was a very simple
process because all the sections had already been defined; they just needed to be
linked to the forms they belonged to. If a section (or an entry) appeared more than
once through the forms, they could just be picked again from the list.
