Note
i. A member entitled to attend and vote at the Meeting is entitled to appoint a Proxy or Proxies to attend and vote instead of him/her. A Proxy
need not be a member of the company.
ii. A Form of Proxy accompanies this notice.
iii.The completed Form of Proxy should be deposited at the registered office of the Company at No.505, Union Place, Colombo 02 (not less than
48 hours before the time appointed for the holding of meeting.)
iv. In terms of the Article 82 of the Articles of Association of the Company the instrument appointing a proxy shall be printed or written and shall
signed by the appointor, or if such appointer be a corporation, it shall be under the Common seal of such corporation.
Shareholders/Proxies attending the meeting are requested to bring their National Identity Card or Passport.
Instructions as to Completion
1. Please perfect the Form of Proxy by filling in legibly your full name and address, by signing in the space provided and filling in the date of
signature.
2. The completed Form of Proxy should be deposited at the Registered Office of the Company, at No. 505 Union Place, Colombo 2, not less than
48 hours before the time appointed for the holding of the Meeting
3. If the Form of Proxy is signed by an Attorney, the relative Power of Attorney should Accompany the Form of Proxy for registration, if such
Power of Attorney has already Been registered with the Company.
4. If the appointer is a Company or Corporation, this Form must be executed under the Common Seal which should be attested in the manner
prescribed by its Articles of Association.
5. If this Form is returned without any indication as to how the person appointed as proxy shall vote, the Proxy shall exercise his/her discretion
as to how he/she votes, or whether he/she abstains form voting.
