Interdisciplinary Geriatric Health Care<br />

Research, Dr. Naylor has been leading<br />

a diverse team <strong>of</strong> scholars from the<br />

<strong>School</strong>s <strong>of</strong> <strong>Nursing</strong>, Medicine and<br />

Social Policy and Practice to create the<br />

model environment for interdisciplinary<br />

research in order to advance<br />

knowledge in the areas <strong>of</strong> transitional<br />

care, mental health, and end <strong>of</strong> life care.<br />

“Chronically ill older adults are<br />

the largest growing segment <strong>of</strong> the<br />

population as baby boomers age.The<br />

needs <strong>of</strong> these elders is placing a<br />

tremendous strain on our healthcare<br />

system and, most especially, on the lives<br />

<strong>of</strong> many families. Now is the time to<br />

invest in interdisciplinary teams committed<br />

to the development <strong>of</strong> knowledge<br />

and the translation <strong>of</strong> new evidence<br />

into needed clinical practice, and<br />

policy changes to enhance the care and<br />

outcomes <strong>of</strong> this population and their<br />

families,” says Dr. Naylor.<br />

New Health Transitions Interest Group Formed<br />

The Health Transitions Interest Group, which intends to become a new<br />

center at <strong>Penn</strong> <strong>Nursing</strong>, seeks to determine and evaluate best practices<br />

in care for individuals, families, health providers, systems <strong>of</strong> care, and<br />

society for those chronically ill adults and caregivers undergoing health<br />

transitions. Such health transitions are crucial, <strong>of</strong>ten complex and multidimensional<br />

processes that affect every aspect <strong>of</strong> life. Understanding<br />

health transitions is essential for preventive guidance, effective<br />

management, and positive outcomes. Specifically with chronically ill<br />

adults, these transitions require research to unlock the essential<br />

processes and properties that contribute to postitive health outcomes.<br />

The Health Transitions Interest Group aims to provide an environment<br />

to foster multi-disciplinary mentorship and collaboration in order to<br />

develop common constructs to drive scholarly investigations leading<br />

to information dissemination.<br />
