C. Staff Member Whose Whereabouts are Unknown: (09/06)<br />

Medical staff members are responsible for notifying <strong>the</strong> hospital <strong>of</strong> any changes in <strong>the</strong>ir <strong>of</strong>fice and home<br />

addresses and telephone numbers and o<strong>the</strong>r essential contact information. A staff member whose<br />

whereabouts are unknown will be considered <strong>to</strong> have voluntarily resigned from <strong>the</strong> medical staff and <strong>the</strong><br />

member’s membership and clinical privileges, if any, will terminate au<strong>to</strong>matically due <strong>to</strong> unexplained<br />

D. Au<strong>to</strong>matic Relinquishment for OIG Sanctions: (01/07)<br />

Action by any appropriate agency that excludes, suspends, or debars an individual from participation in<br />

federal healthcare programs, <strong>the</strong> conviction <strong>of</strong> a criminal <strong>of</strong>fense related <strong>to</strong> <strong>the</strong> provision <strong>of</strong> pr<strong>of</strong>essional<br />

healthcare services, or any o<strong>the</strong>r event that o<strong>the</strong>rwise makes <strong>the</strong> individual ineligible for participation in<br />

federal healthcare programs, will result in au<strong>to</strong>matic resignation <strong>of</strong> medical staff membership and clinical<br />

privileges as <strong>of</strong> that date. Only upon reinstatement by <strong>the</strong> agency may <strong>the</strong> individual reapply as a new<br />

applicant. (01/09)<br />

E. Failure <strong>to</strong> Provide Emergency Department Call Coverage: (01/09)<br />

Failure <strong>to</strong> fulfill emergency call responsibilities as scheduled by <strong>the</strong> clinical department chair shall result in<br />

au<strong>to</strong>matic relinquishment <strong>of</strong> <strong>the</strong> individual’s membership and clinical privileges.<br />

F. Effect <strong>of</strong> Above Actions:<br />

The above actions concerning staff members shall not be considered pr<strong>of</strong>essional review actions and shall not<br />

entitle <strong>the</strong> individual concerned <strong>to</strong> any <strong>of</strong> <strong>the</strong> procedural rights provided in <strong>the</strong> Fair Hearing and Appeal Policy<br />

and Procedure with respect <strong>to</strong> hearings or appeals.<br />

VIII.<br />

LEAVE OF ABSENCE<br />

A. Procedure for Leave <strong>of</strong> Absence:<br />

Members <strong>of</strong> <strong>the</strong> medical staff may be granted leaves <strong>of</strong> absence by <strong>the</strong> Board for a definitely stated period <strong>of</strong><br />

time not <strong>to</strong> exceed one (1) year or <strong>the</strong> remainder <strong>of</strong> <strong>the</strong> member’s current term <strong>of</strong> <strong>appointment</strong>, whichever is<br />

shortest. Written requests for leaves <strong>of</strong> absence shall be made <strong>to</strong> <strong>the</strong> respective department chairman and shall<br />

state <strong>the</strong> beginning and ending dates <strong>of</strong> <strong>the</strong> requested leave and <strong>the</strong> reason leave is requested. The department<br />

chairman shall transmit <strong>the</strong> request <strong>to</strong>ge<strong>the</strong>r with a recommendation <strong>to</strong> <strong>the</strong> Medical Executive Committee<br />

which shall make a report and a recommendation, and transmit it <strong>to</strong> <strong>the</strong> Board. No member may take more<br />

than one consecutive one-year leave <strong>of</strong> absence. Leaves <strong>of</strong> absence are a matter <strong>of</strong> courtesy, not <strong>of</strong> right. In<br />

<strong>the</strong> event that a requested leave <strong>of</strong> absence is denied, or a leave <strong>of</strong> absence is granted for a shorter period <strong>of</strong><br />

time than requested, <strong>the</strong> determination is final, with no recourse <strong>to</strong> any procedural rights. Leaves <strong>of</strong> absence<br />

are typically granted for reasons such as military duty, additional training, family matters, or personal health<br />

matters. Members who are relocating and who do not anticipate returning <strong>to</strong> this area are discouraged from<br />

requesting a leave <strong>of</strong> absence and should consider resignation. (01/09)<br />

B. Reinstatement Following Leave <strong>of</strong> Absence:<br />

A staff member who desires reinstatement following a leave <strong>of</strong> absence shall submit a written request for<br />

reinstatement at least ninety (90) days prior <strong>to</strong> <strong>the</strong> termination <strong>of</strong> <strong>the</strong> leave <strong>of</strong> absence. The staff member shall<br />

submit a written summary <strong>of</strong> relevant activities during <strong>the</strong> leave. The Credentials Committee shall make a<br />

recommendation <strong>to</strong> <strong>the</strong> Medical Executive Committee and <strong>the</strong> Medical Executive Committee shall make a<br />

recommendation <strong>to</strong> <strong>the</strong> Board concerning <strong>the</strong> reinstatement <strong>of</strong> <strong>the</strong> staff member's privileges. (01/09)<br />
