9. Q: I forgot my password, how do I get it reset?<br />

10. Q: I have questions regarding the Ages and Stages Screening process; whom do I contact?<br />

A: You can contact Rachel Winters at the University of Pittsburgh School of Social Workrrw14@pitt.edu<br />

11. Q: Do I have to enter screenings that occurred prior to July 1, 2009?<br />

A. No, results from screens conducted prior to July 1 are not required to be entered. Children<br />

screened prior to July 1 can be entered into the application when a subsequent screen is conducted<br />

after July 1, 2009. If your county does require that screens prior to July 1 are entered, the<br />

application will accept the data.<br />

12. Q: Can I see children entered into the application by other agencies or people?<br />

A: You will only be able to see children who have been entered by your county. Child information<br />

is not linked to the person who entered the information but to the county that staff member is<br />

assigned. For example, Ms. Data Entry from Cumberland County will be able to access records<br />

for all children who have been entered by staff from Cumberland County. However, she would not<br />

be able to access data from nearby Dauphin County. But Ms. Data Entry would be able to access<br />

the file of a Cumberland County child entered into the system by Ms. Data Entry’s co-worker Mr.<br />

Data Recorder. This is so that other people can update files when someone is sick, etc.<br />

However, if you are a provider or other agency entering information for a county children and<br />

youth agency you will be able to see <strong>ASQ</strong> files of children you are not tracking. For example, Mr.<br />

Data Entry from Pike County MH/MR who is conducting screens on some of Pike County Children<br />

& Youth consumers could access files of Pike County children whom his agency is not tracking but<br />

Pike County Children and Youth is monitoring regarding <strong>ASQ</strong> screenings. Counties and agencies<br />

who are in this situation may want to strengthen their confidentiality agreements.<br />

13. Q: How can I access the Training Program’s online data entry application?<br />

A: The application can be accessed through the program’s home page<br />

http://www.pacwcbt.pitt.edu/ by selecting the Ages & Stages link on the bottom left of the page.<br />

Or, you can bypass the homepage and go directly to the site through<br />

http://www.encompass.pitt.edu/agesstages/<strong>ASQ</strong>HomePage.aspx<br />
