Updates <str<strong>on</strong>g>for</str<strong>on</strong>g> Ophthalmic Specialist Trainees<br />

The following paragraphs c<strong>on</strong>tain important in<str<strong>on</strong>g>for</str<strong>on</strong>g>mati<strong>on</strong> that has ei<strong>the</strong>r been discussed<br />

and agreed by <strong>the</strong> Training Committee or is now a GMC requirement. Please read this<br />

letter carefully as failure to comply with <strong>the</strong> requirements may affect a trainee’s <str<strong>on</strong>g>ARCP</str<strong>on</strong>g><br />

outcome and/or <strong>the</strong> recommendati<strong>on</strong>/award of CCT.<br />

MSF assessor guidance<br />

The subject of nominati<strong>on</strong>s <str<strong>on</strong>g>for</str<strong>on</strong>g> MSF assessors was discussed at <strong>the</strong> Training Committee<br />

and it was agreed that <strong>the</strong> guidance <strong>on</strong> MSF assessors needs to be more specific. With<br />

immediate effect (<str<strong>on</strong>g>for</str<strong>on</strong>g> those at <strong>the</strong> stage of seeking approval by <strong>the</strong> educati<strong>on</strong>al supervisor)<br />

<strong>the</strong> 15 assessors selected must include:<br />

2 c<strong>on</strong>sultant clinical supervisors<br />

2 o<strong>the</strong>r <str<strong>on</strong>g>trainees</str<strong>on</strong>g><br />

1 senior nurse in <strong>the</strong> operating <strong>the</strong>atre (if <strong>the</strong> trainee has been per<str<strong>on</strong>g>for</str<strong>on</strong>g>ming surgery)<br />

1 senior nurse in <strong>the</strong> out-patient department<br />

1 o<strong>the</strong>r member of <strong>the</strong> out-patient staff (nurse/optometrist/orthoptist)<br />

1 medical secretary who has been dealing with <strong>the</strong> trainee’s work.<br />

The entire list still requires Educati<strong>on</strong>al Supervisor approval.<br />

Trainees should start <strong>the</strong>ir MSF at least 2 m<strong>on</strong>ths in advance of <strong>the</strong> date it is required <str<strong>on</strong>g>for</str<strong>on</strong>g><br />

<strong>the</strong>ir <str<strong>on</strong>g>ARCP</str<strong>on</strong>g>. MSFs will <strong>on</strong>ly close and generate a summary report when ALL assessors<br />

have resp<strong>on</strong>ded or a m<strong>on</strong>th has elapsed. It is not possible to close <strong>the</strong> MSF at an earlier<br />

date; please do not ring <strong>the</strong> Training Department to request this.<br />

Global assessment with OSATS1 <str<strong>on</strong>g>for</str<strong>on</strong>g>ms<br />

The intended purpose of <strong>the</strong> OSATS assessment is to take a snapshot of <strong>the</strong> trainee’s<br />

surgical per<str<strong>on</strong>g>for</str<strong>on</strong>g>mance in regard to a particular case and to assess it <str<strong>on</strong>g>for</str<strong>on</strong>g>m<str<strong>on</strong>g>all</str<strong>on</strong>g>y in a <str<strong>on</strong>g>for</str<strong>on</strong>g>mative<br />

way ensuring that appropriate feedback is given. This feedback should include a note of<br />

what was d<strong>on</strong>e well during <strong>the</strong> case and areas <str<strong>on</strong>g>for</str<strong>on</strong>g> development.<br />

Trainees and trainers have asked why it cannot relate to a series of operati<strong>on</strong>s. It is<br />

intended that this sort of global/l<strong>on</strong>gitudinal judgment is <str<strong>on</strong>g>for</str<strong>on</strong>g> <strong>the</strong> Clinical Supervisor to make<br />

in his/her end-of-attachment report. The Clinical Supervisor will use multiple OSATS<br />

assessments as an aid to that judgment. (The Educati<strong>on</strong>al Supervisor and ultimately <strong>the</strong><br />

<str<strong>on</strong>g>ARCP</str<strong>on</strong>g> panel will also use <strong>the</strong> outcomes of <strong>the</strong> series of assessments, al<strong>on</strong>g with <strong>the</strong><br />

Clinical Supervisors' reports, as part of <strong>the</strong> evidence <str<strong>on</strong>g>for</str<strong>on</strong>g> <strong>the</strong>ir c<strong>on</strong>clusi<strong>on</strong>s.)<br />

Whilst <strong>the</strong> OSATS assessment is intended to be applied to a single case, <strong>the</strong> College has<br />

agreed that it is sometimes reas<strong>on</strong>able <str<strong>on</strong>g>for</str<strong>on</strong>g> it to be applied to a series of cases undertaken<br />

<strong>on</strong> a single operating list. In this case <strong>the</strong> operati<strong>on</strong>s included in <strong>on</strong>e assessment MUST <str<strong>on</strong>g>all</str<strong>on</strong>g><br />

be d<strong>on</strong>e by <strong>the</strong> trainee <strong>on</strong> ONE list which must (obviously) be directly supervised. If<br />

ano<strong>the</strong>r list is also to be assessed, this must be d<strong>on</strong>e as a separate assessment. It should<br />

be remembered that <strong>the</strong> curricular requirements <str<strong>on</strong>g>for</str<strong>on</strong>g> <strong>the</strong> number of satisfactorily per<str<strong>on</strong>g>for</str<strong>on</strong>g>med<br />

assessments are MINIMA - <strong>the</strong>re is nothing to stop a trainee having more than <strong>the</strong><br />

specified number.<br />

WpBAs and <strong>the</strong> E-Portfolio<br />

The CCT sub-committee still see many examples of WpBAs being bunched (e.g. <str<strong>on</strong>g>all</str<strong>on</strong>g> CBDs<br />

being completed <strong>on</strong> <strong>on</strong>e or two days, usu<str<strong>on</strong>g>all</str<strong>on</strong>g>y immediately preceding an <str<strong>on</strong>g>ARCP</str<strong>on</strong>g>). This will<br />

result in an <str<strong>on</strong>g>ARCP</str<strong>on</strong>g> Outcome 5 and a CCT applicati<strong>on</strong> being declined.<br />

Disappointingly <strong>the</strong> CCT sub-committee still find examples of E-Portfolios not being a state<br />

of readiness <str<strong>on</strong>g>for</str<strong>on</strong>g> review following an applicati<strong>on</strong> <str<strong>on</strong>g>for</str<strong>on</strong>g> CCT. This too will result in <strong>the</strong><br />

applicati<strong>on</strong> being declined. Heads of School have agreed that an unsatisfactory<br />

presentati<strong>on</strong> of an E-Portfolio at <str<strong>on</strong>g>ARCP</str<strong>on</strong>g> will result in an outcome 5.<br />

Cataract audit<br />

Please remember to upload <strong>the</strong> audit of 50 c<strong>on</strong>secutive cataract cases is to <strong>the</strong> e-portfolio<br />

as a resource and assign it to <strong>the</strong> appropriate <str<strong>on</strong>g>ARCP</str<strong>on</strong>g>. See DRMCJ3<br />

Accelerated Training<br />

Please note <strong>the</strong> guidance <strong>on</strong> this subject in previous letters. These can be found <strong>on</strong> <strong>the</strong><br />

Trainees who are thinking of applying <str<strong>on</strong>g>for</str<strong>on</strong>g> accelerated training should discuss this with <strong>the</strong>ir<br />

Educati<strong>on</strong>al Supervisors and Training Programme Directors well ahead of time so that<br />

appropriate preparati<strong>on</strong> (and applicati<strong>on</strong>) can be made. An applicati<strong>on</strong> <str<strong>on</strong>g>for</str<strong>on</strong>g> accelerated<br />

training cannot be made retrospectively.<br />

Time limiting of CCT applicati<strong>on</strong>s<br />

As announced by <strong>the</strong> GMC in December 2012, as of 31 March 2013 <str<strong>on</strong>g>all</str<strong>on</strong>g> doctors will have a<br />

maximum of 12 m<strong>on</strong>ths to apply <str<strong>on</strong>g>for</str<strong>on</strong>g> <strong>the</strong>ir Certificate of Completi<strong>on</strong> of Training (CCT). E.g.<br />

if your CCT date is 1 August 2013 you must apply to <strong>the</strong> GMC <str<strong>on</strong>g>for</str<strong>on</strong>g> your CCT be<str<strong>on</strong>g>for</str<strong>on</strong>g>e 30 July<br />

2014. Any doctor who fails to comply with this will have to submit an applicati<strong>on</strong> <str<strong>on</strong>g>for</str<strong>on</strong>g> a<br />

Certificate of Eligibility <str<strong>on</strong>g>for</str<strong>on</strong>g> Specialist Registrati<strong>on</strong> (CESR) in order to gain <strong>the</strong>ir specialist<br />

registrati<strong>on</strong>.<br />

QA of Educati<strong>on</strong> Supervisor and Clinical Supervisor reports.<br />

QA of <strong>the</strong> assessment process is a subject of GMC interest. This year External Advisors<br />

to <strong>the</strong> <str<strong>on</strong>g>ARCP</str<strong>on</strong>g> Panels will be piloting a QA process of <strong>the</strong>se reports. The pilot document is<br />

attached <str<strong>on</strong>g>for</str<strong>on</strong>g> your in<str<strong>on</strong>g>for</str<strong>on</strong>g>mati<strong>on</strong>.<br />

Educati<strong>on</strong>al and Clinical Supervisor report <str<strong>on</strong>g>for</str<strong>on</strong>g>ms<br />
