Acupuncture and Dry Needling Group 2 Western acupuncture supplements a biomechanical corrective model in knee pain Berg D Movehappy Health Care Clinic, ACT A sixty-two year old female presented with a sudden onset of left medial knee pain while walking. The pain was constant and had reduced her walking tolerance from one hour to five minutes. Physical examination excluded structural instability, and a diagnosis of medial patellofemoral and tibiofemoral joint stress due to poor biomechanics was made. Over four sessions various treatments were tried, aimed at reducing the loading of the medial knee. These consisted of both active and passive interventions, including dry needling of the quadriceps group. These treatments were all ineffective, and on some occasions exacerbated the patient’s pain. In an attempt to offer relief from the pain while the patient improved her biomechanics, western acupuncture was trialled. Six sessions of 20 minutes duration over a period of 3 weeks were used. Needles were placed adjacent to the L3 and L4 vertebrae bilaterally, and points SP6, SP9, SP10, GB34, LR3, and, ST36. The needles were stimulated intermittently. These points and the treatment time were chosen to facilitate pain relief via facilitation of local, segmental and extrasegmental effects. Using this treatment regime, the patient achieved excellent pain relief. Upon completion of the 3 weeks, the patient had intermittent pain and was able to walk for 30 minutes. She continued to work on her strengthening and resumed her normal walking routine after 3 additional weeks. This case study demonstrates that the analgesic effect of western acupuncture may facilitate a biomechanical corrective model for musculoskeletal pain. Needle manipulation and fascia: workshop with ultrasound analysis Langevin HM Departments of Neurology, Orthopaedics and Rehabilitation,University of Vermont Ultrasound imaging during acupuncture needle manipulation offers an opportunity to ‘see what is going on beneath the skin’ during acupuncture needling. This workshop will demonstrate how ultrasound can be used to visualise the interaction of acupuncture needles with connective tissues, including subcutaneous and perimuscular fascia, as well as elastography techniques allowing quantification of tissue displacement and strain induced by needle motion. The use of acupunctures in the management of complex regional pain syndrome type I: a pilot study Longbottom JE Acupuncture Association of Chartered Physiotherapists, Peterborough, UK The aim of this small pilot study was to develop a working protocol for the management of pain, anxiety and stress in the condition of complex regional pain syndrome type I. A single system research design comprising of 4 subjects (n = 4) diagnosed with complex regional pain syndrome type I, affecting the upper limb (n = 2) and lower limb (n = 2) was used. In addition to the usual rehabilitation program, subjects underwent acupuncture using auricular, segmental and distal acupuncture points as a means of modifying hyperalgesia within the sympathetic nervous system, anxiety and stress, before a formal exercise program. All 4 subjects received the acupuncture protocol for 6 weeks whilst undergoing a home exercise program, pacing and relaxation training, prior to a formal 6-week intensive rehabilitation program. All participants demonstrated reduction in the Hospital Anxiety and Depression Scores, Visual Outcome Scale for pain, and Quality of Life Score. This study was designed to determine and test the protocol used as a precursor to future research using a single-blind, randomised controlled trial. Western acupuncture and dry needling in spinal regions McCutcheon LM Combined Health Acupuncture and Dry Needling Education, Research Fellow School of Medicine, Griffith University Physiotherapists in ‘western’ countries, including Australia, the UK, New Zealand, Canada, the US and South Africa are increasingly using dry needling and Western Acupuncture. Western Acupuncture has its foundations in neurophysiological clinical reasoning and combines local, segmental and extra-segmental needling points. Even though traditional acupuncture points are used with this style of needling Western Acupuncture is not viewed as Traditional Chinese Medicine (TCM) as no paradigms or traditional assessment methods are adopted from TCM. Dry needling in the spinal region will be discussed considering Travell and Simons, Gunn and Baldry approaches. Clinical reasoning combining both Western Acupuncture and dry needling will be presented along with relevant literature from the current evidence base for needling in spinal regions. Western acupuncture and dry needling for triathletes McCutcheon LM Combined Health Acupuncture and Dry Needling Education, Research Fellow School of Medicine, Griffith University The role of physiotherapy with the triathlete often includes sports screening, injury prevention, performance enhancement and treatment of musculoskeletal injuries. The sport of triathlon has a high incidence of injury ranging from 37% to 91%. Injuries are predominately due to overuse, however they may also be traumatic in origin. The most commonly reported sites of injury are the knee and lower leg regions, although the shoulder, back and neck have also been identified as commonly injured in triathletes. Western acupuncture has developed over the past 30–40 years having its foundations in neurophysiological clinical reasoning and combining local, segmental and extra-segmental needling points. Dry needling using a Travell and Simons style is an established form of needling therapy which addresses the aetiology and treatment of myofascial pain syndromes. Both dry needling and Western Acupuncture can be effective in the treatment of triathlete related injuries although there is only presently limited reference to the use of needling therapies in triathlete related injuries in the research literature. A review of various musculoskeletal conditions that triathletes commonly present with are discussed along with a presentation of dry needling and Western acupuncture techniques that may be employed in the overall management of the triathlete.
