DATE, TIME AND PLACE OF ACCIDENT OR OCCURRENCE:<br />

June 19, 2005<br />

CAUSE OF INJURY:<br />

Plaintiff claimed that she was injured as a result of a rear-end<br />

motor vehicle accident that occurred on June 19, 2005. The<br />

Defendant was stopped behind the Plaintiff at a red light. The<br />

Plaintiff began moving forward. The Defendant also began moving<br />

forward when the Plaintiff stopped his vehicle. The Defendant’s<br />

vehicle struck the rear of the Plaintiff’s vehicle at a low<br />

speed.<br />

NATURE OF INJURY:<br />

Plaintiff claimed that she sustained a disc herniation at C6-7<br />

and a disc bulge at L4-5.<br />
