Update on Advanced Practice in Sonography<br />

Since <strong>the</strong> last update we provided on Advanced<br />

Practice in Sonography, <strong>the</strong> <strong>SDMS</strong> and <strong>the</strong> Advanced<br />

Practice Committee (APC) have been busy working<br />

on Sonography Advanced Practice objectives<br />

and activies. The <strong>SDMS</strong> APC, in collaboration with<br />

several organizational members <strong>of</strong> <strong>the</strong> ultrasound<br />

pr<strong>of</strong>essional community, sponsored a Sonographer<br />

Advanced Practice Survey that was launched in early<br />

February. The survey was targeted at sonographers<br />

who are currently employed as mid-level providers<br />

or Ultrasound Practitioners. We were fortunate <strong>to</strong> be<br />

able <strong>to</strong> partner with o<strong>the</strong>r sonography organizations<br />

in promoting participation in <strong>the</strong> survey. Our<br />

collaborative partners included <strong>the</strong> American Instittue<br />

<strong>of</strong> Ultrasound in Medicine (AIUM), <strong>the</strong> American<br />

Reg<strong>is</strong>try <strong>of</strong> Diagnostic Medical Sonography (ARDMS)<br />

and <strong>the</strong> <strong>Society</strong> for Vascular Ultrasound (SVU).<br />

Although we had no idea how many sonographers<br />

were currently employed as mid-level providers<br />

going in<strong>to</strong> <strong>the</strong> survey, <strong>the</strong> final response significantly<br />

exceeded our expectations.<br />

We have also had a number <strong>of</strong> inquiries from<br />

sonographers throughout <strong>the</strong> country wanting more<br />

information on Advanced Practice. I am pleased <strong>to</strong><br />

see <strong>the</strong> passion and interest within <strong>the</strong> sonographer<br />

community, as well as <strong>the</strong> interest within o<strong>the</strong>r<br />

pr<strong>of</strong>essional sonography organizations on <strong>the</strong> <strong>to</strong>pic <strong>of</strong><br />

advanced practice.<br />

The committee has also been actively involved<br />

in o<strong>the</strong>r areas as well. The dec<strong>is</strong>ion was made <strong>to</strong><br />

evaluate <strong>the</strong> previous advanced practice curriculum<br />

models, and in order <strong>to</strong> facilitate th<strong>is</strong> process a<br />

curriculum sub-committee was created within <strong>the</strong><br />

APC. Th<strong>is</strong> curriculum sub-committee <strong>is</strong> compr<strong>is</strong>ed <strong>of</strong><br />

APC members who have educa<strong>to</strong>rs’ and curriculum<br />

experience as well as o<strong>the</strong>r educational experts from<br />

outside <strong>the</strong> committee. At <strong>the</strong> <strong>SDMS</strong> Spring Board<br />

meeting in April, <strong>the</strong> <strong>SDMS</strong> Board will be considering<br />

<strong>the</strong> <strong>is</strong>sue <strong>of</strong> <strong>the</strong> accreditation process for sonography<br />

educational programs and that will include our<br />

future objective <strong>of</strong> working with <strong>the</strong> Comm<strong>is</strong>sion on<br />

Accreditation <strong>of</strong> Allied Health Education Programs<br />

(CAAHEP) in pursuing programmatic accreditation for<br />

a formal Sonography Advanced Practice educational<br />

program.<br />

In a continuing attempt <strong>to</strong> provide <strong>the</strong> general<br />

membership a clearer picture <strong>of</strong> <strong>the</strong> clinical pr<strong>of</strong>ile<br />

<strong>of</strong> those sonographers who are currently employed<br />

as a mid-level provider, I would like <strong>to</strong> share ano<strong>the</strong>r<br />

personal s<strong>to</strong>ry <strong>of</strong> one <strong>of</strong> <strong>the</strong>se individuals. I would like<br />

<strong>to</strong> highlight one <strong>of</strong> my colleagues, Tim S. Gibbs.<br />

One Sonographer’s Path <strong>to</strong> Advanced<br />

Practice: A Winding Road<br />

I was never <strong>of</strong>ficially<br />

hired in<strong>to</strong> a position<br />

with a formal Advanced<br />

Practice Sonographer job<br />

description, but instead<br />

morphed in<strong>to</strong> <strong>the</strong> role<br />

through <strong>the</strong> years. Having<br />

a background in radiographic special procedures<br />

was a big help. Working in a smaller community<br />

facility provided me with <strong>the</strong> opportunity <strong>to</strong> be<br />

involved in a wide variety <strong>of</strong> routine and advanced<br />

ultrasound-related imaging procedures.<br />

On any given day, I may be reviewing previously<br />

completed ultrasound examinations <strong>to</strong> assure<br />

image quality and completeness, or asked <strong>to</strong><br />

evaluate <strong>the</strong> feasibility <strong>of</strong> using ultrasound<br />

imaging <strong>to</strong> perform various invasive procedures.<br />

Th<strong>is</strong> requires that I review o<strong>the</strong>r imaging studies<br />

prior <strong>to</strong> ultrasound imaging <strong>to</strong> determine<br />

if ultrasound guidance should and can be<br />

performed. I review pertinent lab results, and<br />

if <strong>the</strong>se studies have not been done <strong>the</strong>n order<br />

<strong>the</strong> necessary tests as per establ<strong>is</strong>hed pro<strong>to</strong>col.<br />

When <strong>the</strong> radiolog<strong>is</strong>ts work load increased, <strong>the</strong>re<br />

was a need created for someone <strong>to</strong> relieve <strong>the</strong>m<br />

<strong>of</strong> some <strong>of</strong> <strong>the</strong>ir tasks, allowing <strong>the</strong>m <strong>to</strong> do more<br />

emergent work. Th<strong>is</strong> change reflected <strong>the</strong> need<br />

for an ultrasound physician extender that was<br />

capable <strong>of</strong> functioning at an advanced level. Th<strong>is</strong><br />

<strong>is</strong> when I began inserting peripherally inserted<br />

central ca<strong>the</strong>ter (PICC) lines. In <strong>the</strong> last 6 years, and<br />

working under <strong>the</strong> superv<strong>is</strong>ion <strong>of</strong> <strong>the</strong> interpreting<br />

physician, I have inserted more than 1,000 PICC

lines and now ass<strong>is</strong>t in teaching formal classes for<br />

o<strong>the</strong>r healthcare pr<strong>of</strong>essionals w<strong>is</strong>hing <strong>to</strong> learn<br />

th<strong>is</strong> type <strong>of</strong> procedure.<br />

One <strong>of</strong> my first experiences with accessing vessels<br />

with ultrasound was when I was called <strong>to</strong> <strong>the</strong><br />

Heart Cath lab <strong>to</strong> ass<strong>is</strong>t with arterial access in a<br />

patient having an acute myocardial infarction<br />

(heart attack). When I arrived, <strong>the</strong> cardiolog<strong>is</strong>t<br />

was attempting a brachial artery cut-down in<br />

an attempt <strong>to</strong> access <strong>the</strong> coronary arteries. I<br />

was directed <strong>to</strong> access <strong>the</strong> femoral artery while<br />

<strong>the</strong> cardiolog<strong>is</strong>t continued with <strong>the</strong> arterial cutdown.<br />

I successfully accessed <strong>the</strong> artery allowing<br />

<strong>the</strong> cardiolog<strong>is</strong>t <strong>to</strong> open <strong>the</strong> coronary arteries<br />

successfully. I have since used th<strong>is</strong> same technique<br />

<strong>to</strong> educate o<strong>the</strong>r physicians in <strong>the</strong> use <strong>of</strong><br />

ultrasound guided vascular access for angiography<br />

and internal jugular vein dialys<strong>is</strong> ca<strong>the</strong>ters.<br />

I <strong>of</strong>ten interact with <strong>the</strong> medical staff regarding<br />

ultrasound imaging. Sometimes it <strong>is</strong> as simple as<br />

showing <strong>the</strong>m <strong>the</strong> ana<strong>to</strong>mical anomaly written<br />

in <strong>the</strong> report, o<strong>the</strong>r times it requires developing<br />

new imaging techniques <strong>to</strong> better care for <strong>the</strong>ir<br />

patients. On one such occasion, I was asked<br />

<strong>to</strong> develop a technique <strong>to</strong> ass<strong>is</strong>t emergency<br />

physicians with <strong>the</strong> removal <strong>of</strong> s<strong>of</strong>t t<strong>is</strong>sue foreign<br />

bodies not seen radiographically. More recently,<br />

<strong>the</strong>re was a need <strong>to</strong> develop a safe approach <strong>to</strong><br />

drain a <strong>to</strong>nsilar abscess. Often <strong>the</strong>se patients<br />

are extremely hypoxic. I worked closely with <strong>the</strong><br />

treating physician and developed a safe and<br />

effective approach that benefitted <strong>the</strong> treating<br />

medical staff and certainly <strong>the</strong> patient.<br />

Working at th<strong>is</strong> level can be very rewarding.<br />

I have been able <strong>to</strong> have a positive impact in<br />

patient care. However, it can also be daunting.<br />

Th<strong>is</strong> <strong>is</strong> why I believe education <strong>is</strong> a critically<br />

important component <strong>of</strong> any advanced practice<br />

model that we embrace in <strong>the</strong> future. I chose <strong>to</strong><br />

return <strong>to</strong> school and obtain my baccalaureate<br />

degree in sonography through d<strong>is</strong>tance learning.<br />

Completing th<strong>is</strong> degree program has enhanced<br />

my skills and allowed me <strong>to</strong> provide better care for<br />

my patients.<br />

Although I have been encouraged by all <strong>the</strong> recent<br />

developments on <strong>the</strong> <strong>to</strong>pic <strong>of</strong> sonography advanced<br />

practice, <strong>the</strong>re <strong>is</strong> still a good deal <strong>of</strong> work <strong>to</strong> be<br />

done. After we have had opportunity <strong>to</strong> analyze <strong>the</strong><br />

responses <strong>to</strong> <strong>the</strong> recent Advanced Practice survey,<br />

a review <strong>of</strong> <strong>the</strong> survey results will be provided <strong>to</strong> <strong>the</strong><br />

membership , th<strong>is</strong> review <strong>is</strong> tentatively scheduled for<br />

publication in <strong>the</strong> June <strong>is</strong>sue <strong>of</strong> <strong>SDMS</strong> News Wave. I<br />

ARDMS Announces Specialty Exam Fee Increase<br />

Board <strong>of</strong> Direc<strong>to</strong>rs Approves <strong>the</strong> First Fee Increase Since 1998<br />

Rockville, Md. (March 26, 2012)---The Board <strong>of</strong> Direc<strong>to</strong>rs<br />

<strong>of</strong> <strong>the</strong> American Reg<strong>is</strong>try for Diagnostic Medical<br />

Sonography® (ARDMS®), <strong>the</strong> globally recognized<br />

standard <strong>of</strong> excellence in sonography, announced<br />

<strong>to</strong>day that effective April 4, 2012, specialty examination<br />

fees, including those exclusively for physicians, will be<br />

increasing for <strong>the</strong> first time since 1998.<br />

The specialty examination fees will increase from $200<br />

<strong>to</strong> $250 while <strong>the</strong> Physicians’ Vascular Interpretation<br />

(PVI) examination fee will now be $600, up from<br />

$500. The fee for <strong>the</strong> Sonography Principles &<br />

Instrumentation (SPI) examination will not change and<br />

will remain at $200.<br />

According <strong>to</strong> Dale R. Cyr, ARDMS CEO and Executive<br />

Direc<strong>to</strong>r, “<strong>the</strong> ARDMS general operating expenses have<br />

more than doubled since we last ra<strong>is</strong>ed examination<br />

fees more than fourteen years ago. As we know,<br />

sonography continues <strong>to</strong> change, expand and evolve<br />

requiring us <strong>to</strong> remain vigilant that our specialty<br />

examinations are current with <strong>the</strong> best practices <strong>of</strong> <strong>the</strong><br />

pr<strong>of</strong>ession.”<br />

2012. Nicole <strong>is</strong> currently<br />

attending Washburn<br />

University and <strong>is</strong> in her<br />

second semester <strong>of</strong><br />

<strong>the</strong> General/Vascular<br />

Ultrasound Program. She<br />

will be graduating in <strong>the</strong><br />

summer <strong>of</strong> 2013.<br />

Nicole was overwhelmed by <strong>the</strong> idea <strong>of</strong> being <strong>the</strong><br />

student scholarship winner. “Receiving th<strong>is</strong> scholarship<br />

means so much <strong>to</strong> me. At first, I was honestly in d<strong>is</strong>belief<br />

that I was <strong>the</strong> one receiving th<strong>is</strong> scholarship, but once<br />

<strong>the</strong> realization <strong>of</strong> it had settled in I felt so grateful. Th<strong>is</strong> <strong>is</strong><br />

unm<strong>is</strong>takably one <strong>of</strong> <strong>the</strong> best feelings in <strong>the</strong> world and I<br />

feel honored <strong>to</strong> have been given th<strong>is</strong> award. It <strong>is</strong> <strong>the</strong> first<br />

time I have ever won an award like th<strong>is</strong> and I wanted <strong>to</strong><br />

thank you so much for choosing me as <strong>the</strong> recipient <strong>of</strong><br />

th<strong>is</strong> award.”<br />

Nicole has been a member <strong>of</strong> <strong>SDMS</strong> since August<br />

2011. We asked Nicole why she decided <strong>to</strong> join<br />

<strong>SDMS</strong>. She said, “I joined <strong>SDMS</strong> because when I was<br />

starting my schooling <strong>to</strong> become a sonographer, I<br />

found that <strong>SDMS</strong> <strong>is</strong> one <strong>of</strong> <strong>the</strong> most recognizable<br />

societies out <strong>the</strong>re for sonography. The organization<br />

<strong>is</strong> very informative on standards, technology, patient<br />

care, etc. Knowing all th<strong>is</strong>, I wanted <strong>to</strong> become a<br />

member because I knew it would not only benefit<br />

me while I’m in school, but as I continue my career in<br />

ultrasound.”<br />

The <strong>SDMS</strong> Foundation provides a variety <strong>of</strong><br />

scholarship and grant opportunities <strong>to</strong> <strong>SDMS</strong><br />

members. The Sonography Student Scholarship<br />

Program provides a $2,500 scholarship <strong>to</strong> a<br />

sonography student who has been accepted or<br />

<strong>is</strong> currently enrolled in a educational program in<br />

diagnostic medical sonography or cardiovascular<br />

technology that has been accredited by <strong>the</strong><br />

Comm<strong>is</strong>sion on Accreditation <strong>of</strong> Allied Health<br />

Educational Programs (CAAHEP). Two sonography<br />

student scholarships are awarded a year. Recipients<br />

can use <strong>the</strong> scholarship for tuition, books, or<br />

educational fees.<br />

Jordyn Leyerzapf <strong>is</strong> a second<br />

year student in <strong>the</strong> Medical<br />

Diagnostic Imaging Program<br />

at Fort Hays State University.<br />

She has already obtained a<br />

Bachelor’s degree in Animal<br />

Science and decided <strong>to</strong><br />

continue her education<br />

by pursuing an additional<br />

degree in Medical Diagnostic<br />

Imaging. Jordan <strong>is</strong> from a<br />

farming community in Northwest Kansas. She was<br />

married a year and half ago and recently moved back<br />

<strong>to</strong> <strong>the</strong> family farm. Through her additional education,<br />

Jordyn and her husband hope <strong>to</strong> expand <strong>the</strong>ir<br />

breeding heifer operation by utilizing sonography <strong>to</strong><br />

detect early pregnancy in <strong>the</strong>ir artificially inseminated<br />

heifers.<br />

Jordyn has always had an interest in sonography. She<br />

said, “The technology <strong>is</strong> continually advancing and<br />

changing <strong>to</strong> better help patients. It <strong>is</strong> a non-invasive<br />

technique that <strong>is</strong> used for a variety <strong>of</strong> practices and<br />

<strong>the</strong> sonographer plays an instrumental role in patient<br />

care.” She said <strong>the</strong>re seems <strong>to</strong> be a pre-conceived<br />

notion that sonography’s main purpose <strong>is</strong> only<br />

imaging women during pregnancies when in fact<br />

<strong>the</strong>re are many o<strong>the</strong>r ways sonography <strong>is</strong> used, such<br />

as in physician-guided procedures. “The wide variety<br />

<strong>of</strong> exams keeps <strong>the</strong> day interesting” Jordyn says.<br />

Jordyn said her favorite <strong>SDMS</strong> benefit <strong>is</strong> <strong>the</strong> online<br />

Journal <strong>of</strong> Diagnostic Medical Sonography (<strong>JDMS</strong>).<br />

She enjoys reading <strong>the</strong> variety <strong>of</strong> articles <strong>of</strong>fered and<br />

gaining knowledge about new techniques which<br />

contribute <strong>to</strong> her continuing education and scanning<br />

abilities. In addition, <strong>the</strong> <strong>JDMS</strong> makes a great<br />

research resource.<br />

What inspired you <strong>to</strong> make a career <strong>of</strong> sonography?<br />

My mo<strong>the</strong>r had an ultrasound <strong>of</strong> her aorta when I was<br />

in high school. I became interested in sonography<br />

as a career and asked <strong>the</strong> sonographer what colleges<br />

<strong>of</strong>fered ultrasound programs. She <strong>to</strong>ld me that at<br />

th<strong>is</strong> time, <strong>the</strong> only program in <strong>the</strong> state <strong>of</strong> Iowa was<br />

at <strong>the</strong> University <strong>of</strong> Iowa and that I would need <strong>to</strong> go<br />

<strong>to</strong> Radiology Technology school first. I applied <strong>to</strong> <strong>the</strong><br />

School <strong>of</strong> Radiolgic Technology at <strong>the</strong> University <strong>of</strong><br />

Iowa and was accepted. After radiology technology<br />

school, I applied <strong>to</strong> <strong>the</strong> University <strong>of</strong> Iowa’s Diagnostic<br />

Medical Sonography program and was accepted.<br />

Did you have a memorable role model/teacher?<br />

Who was it?<br />

My diagnostic medical sonography program direc<strong>to</strong>r<br />

was my men<strong>to</strong>r and role model. After I graduated,<br />

my men<strong>to</strong>r was Glenda Mueller, <strong>the</strong> superv<strong>is</strong>or in<br />

diagnostic medical sonography at my first job as a<br />

sonographer. Glenda could image every ana<strong>to</strong>mical<br />

system with ultrasound and made everything look<br />

so easy. She also was very gifted at sharing her<br />

knowledge and teaching us any <strong>of</strong> <strong>the</strong> new imaging<br />

modalities. She always made sure that everyone<br />

had <strong>the</strong> education and training <strong>to</strong> implement new<br />

technology as it became available in our lab.<br />

What was <strong>the</strong> most significant advance in <strong>the</strong><br />

technology that you directly experienced?<br />

I would say 3D technology. While I still think that we,<br />

as a pr<strong>of</strong>ession, are developing new uses for 3D, <strong>the</strong><br />

impact it has had on cardiac valve ana<strong>to</strong>my with TEE,<br />

obstetrical sonography and gynecological sonography<br />

have changed our imaging pro<strong>to</strong>cols for certain exam<br />

indications. I believe <strong>the</strong> full use <strong>of</strong> th<strong>is</strong> technology<br />

<strong>is</strong> yet <strong>to</strong> come, and I do think it will impact how we<br />

practice and educate for <strong>the</strong> future.<br />

Were <strong>the</strong>re any memorable developments, cases<br />

or events that changed <strong>the</strong> way you practiced<br />

sonography?<br />

The growth <strong>of</strong> image resolution and how we<br />

image <strong>the</strong> fetal heart has evolved from when I first<br />

started performing obstetrical and pediatric cardiac<br />

sonography. We have seen pr<strong>of</strong>essional organizations<br />

advocate adding screening heart views <strong>to</strong> obstetric<br />

pro<strong>to</strong>cols and we are detecting defects earlier.<br />

What changes do you predict in <strong>the</strong> immediate<br />

future?<br />

I think we will continue <strong>to</strong> see an evolution <strong>of</strong> how 3D<br />

technology can add <strong>to</strong> sonographic exams across all<br />

d<strong>is</strong>ciplines.<br />

What advice would you give <strong>to</strong> students/ future<br />

sonographers?<br />

Run <strong>to</strong> <strong>the</strong> new technology and never be comfortable<br />

continuing <strong>to</strong> practice <strong>the</strong> way you were taught. Jump<br />

<strong>to</strong> learn and implement <strong>the</strong> new technology as it<br />

becomes available, th<strong>is</strong> will always keep you current<br />

and add <strong>to</strong> your value in your imaging lab.<br />

What’s your favorite part <strong>of</strong> your job?<br />

My favorite part <strong>is</strong> working with new students<br />

preparing <strong>to</strong> enter <strong>the</strong> field. I enjoy working with<br />

students as <strong>the</strong>y begin <strong>to</strong> explore diagnostic medical<br />

sonography as a career and <strong>the</strong>n watching <strong>the</strong>m be<br />

accepted in<strong>to</strong> a program, and <strong>the</strong>n graduating and<br />

entering <strong>the</strong> pr<strong>of</strong>ession. Many go on <strong>to</strong> publ<strong>is</strong>h,<br />

become educa<strong>to</strong>rs, superv<strong>is</strong>ors and want <strong>to</strong> contribute<br />

<strong>to</strong> <strong>the</strong> future shaping <strong>of</strong> our pr<strong>of</strong>ession.
