the JDMS Collection Series
SDMS members now have the opportunity to earn an
additional twenty (20) SDMS CME credits with the new
JDMS Collection Series, a compilation of related articles
previously published in the Journal of Diagnostic
Medical Sonography (JDMS), the official publication of
the Society of Diagnostic Medical Sonography (SDMS).
Each collection is comprised of articles on a specific
topic which have been gathered together to provide
sonographers and other health care providers with
extensive information on the topic. In addition, a
continuing medical education (CME) test is provided
for SDMS members to earn CME credit(s) after reading

Update on Advanced Practice in Sonography

Since the last update we provided on Advanced
Practice in Sonography, the SDMS and the Advanced
Practice Committee (APC) have been busy working
on Sonography Advanced Practice objectives
and activies. The SDMS APC, in collaboration with
several organizational members of the ultrasound
professional community, sponsored a Sonographer
Advanced Practice Survey that was launched in early
February. The survey was targeted at sonographers
who are currently employed as mid-level providers
or Ultrasound Practitioners. We were fortunate to be
able to partner with other sonography organizations
in promoting participation in the survey. Our
collaborative partners included the American Instittue
of Ultrasound in Medicine (AIUM), the American
Registry of Diagnostic Medical Sonography (ARDMS)
and the Society for Vascular Ultrasound (SVU).
Although we had no idea how many sonographers
were currently employed as mid-level providers
going into the survey, the final response significantly
exceeded our expectations.
We have also had a number of inquiries from
sonographers throughout the country wanting more
information on Advanced Practice. I am pleased to
see the passion and interest within the sonographer
community, as well as the interest within other
professional sonography organizations on the topic of
advanced practice.
The committee has also been actively involved
in other areas as well. The decision was made to
evaluate the previous advanced practice curriculum
models, and in order to facilitate this process a
curriculum sub-committee was created within the
APC. This curriculum sub-committee is comprised of
APC members who have educators’ and curriculum
experience as well as other educational experts from
outside the committee. At the SDMS Spring Board
meeting in April, the SDMS Board will be considering
the issue of the accreditation process for sonography
educational programs and that will include our
future objective of working with the Commission on
Accreditation of Allied Health Education Programs
(CAAHEP) in pursuing programmatic accreditation for
a formal Sonography Advanced Practice educational
program.

In a continuing attempt to provide the general
membership a clearer picture of the clinical profile
of those sonographers who are currently employed
as a mid-level provider, I would like to share another
personal story of one of these individuals. I would like
to highlight one of my colleagues, Tim S. Gibbs.
One Sonographer’s Path to Advanced
Practice: A Winding Road

I was never officially
hired into a position
with a formal Advanced
Practice Sonographer job
description, but instead
morphed into the role
through the years. Having
a background in radiographic special procedures
was a big help. Working in a smaller community
facility provided me with the opportunity to be
involved in a wide variety of routine and advanced
ultrasound-related imaging procedures.
On any given day, I may be reviewing previously
completed ultrasound examinations to assure
image quality and completeness, or asked to
evaluate the feasibility of using ultrasound
imaging to perform various invasive procedures.
This requires that I review other imaging studies
prior to ultrasound imaging to determine
if ultrasound guidance should and can be
performed. I review pertinent lab results, and
if these studies have not been done then order
the necessary tests as per established protocol.
When the radiologists work load increased, there
was a need created for someone to relieve them
of some of their tasks, allowing them to do more
emergent work. This change reflected the need
for an ultrasound physician extender that was
capable of functioning at an advanced level. This
is when I began inserting peripherally inserted
central catheter (PICC) lines. In the last 6 years, and
working under the supervision of the interpreting
physician, I have inserted more than 1,000 PICC

lines and now assist in teaching formal classes for
other healthcare professionals wishing to learn
this type of procedure.
One of my first experiences with accessing vessels
with ultrasound was when I was called to the
Heart Cath lab to assist with arterial access in a
patient having an acute myocardial infarction
(heart attack). When I arrived, the cardiologist
was attempting a brachial artery cut-down in
an attempt to access the coronary arteries. I
was directed to access the femoral artery while
the cardiologist continued with the arterial cutdown.
I successfully accessed the artery allowing
the cardiologist to open the coronary arteries
successfully. I have since used this same technique
to educate other physicians in the use of
ultrasound guided vascular access for angiography
and internal jugular vein dialysis catheters.
I often interact with the medical staff regarding
ultrasound imaging. Sometimes it is as simple as
showing them the anatomical anomaly written
in the report, other times it requires developing
new imaging techniques to better care for their
patients. On one such occasion, I was asked
to develop a technique to assist emergency
physicians with the removal of soft tissue foreign
bodies not seen radiographically. More recently,
there was a need to develop a safe approach to
drain a tonsilar abscess. Often these patients
are extremely hypoxic. I worked closely with the
treating physician and developed a safe and
effective approach that benefitted the treating
medical staff and certainly the patient.
Working at this level can be very rewarding.
I have been able to have a positive impact in
patient care. However, it can also be daunting.
This is why I believe education is a critically
important component of any advanced practice
model that we embrace in the future. I chose to
return to school and obtain my baccalaureate
degree in sonography through distance learning.
Completing this degree program has enhanced
my skills and allowed me to provide better care for
my patients.
Although I have been encouraged by all the recent
developments on the topic of sonography advanced
practice, there is still a good deal of work to be
done. After we have had opportunity to analyze the
responses to the recent Advanced Practice survey,
a review of the survey results will be provided to the
membership , this review is tentatively scheduled for
publication in the June issue of SDMS News Wave. I

ARDMS Announces Specialty Exam Fee Increase
Board of Directors Approves the First Fee Increase Since 1998
Rockville, Md. (March 26, 2012)---The Board of Directors
of the American Registry for Diagnostic Medical
Sonography® (ARDMS®), the globally recognized
standard of excellence in sonography, announced
today that effective April 4, 2012, specialty examination
fees, including those exclusively for physicians, will be
increasing for the first time since 1998.
The specialty examination fees will increase from $200
to $250 while the Physicians’ Vascular Interpretation
(PVI) examination fee will now be $600, up from
$500. The fee for the Sonography Principles &
Instrumentation (SPI) examination will not change and
will remain at $200.
According to Dale R. Cyr, ARDMS CEO and Executive
Director, “the ARDMS general operating expenses have
more than doubled since we last raised examination
fees more than fourteen years ago. As we know,
sonography continues to change, expand and evolve
requiring us to remain vigilant that our specialty
examinations are current with the best practices of the
profession.”

Scholarship Program Recipient in 2012
Nicole Norwick of

York is the first SDMS
Foundation Sonography
Student Scholarship
Program recipient of
2012. Nicole is currently
attending Washburn
University and is in her
second semester of
the General/Vascular
Ultrasound Program. She
will be graduating in the
summer of 2013.
Nicole was overwhelmed by the idea of being the
student scholarship winner. “Receiving this scholarship
means so much to me. At first, I was honestly in disbelief
that I was the one receiving this scholarship, but once
the realization of it had settled in I felt so grateful. This is
unmistakably one of the best feelings in the world and I
feel honored to have been given this award. It is the first
time I have ever won an award like this and I wanted to
thank you so much for choosing me as the recipient of
this award.”
Nicole has been a member of SDMS since August
2011. We asked Nicole why she decided to join
SDMS. She said, “I joined SDMS because when I was
starting my schooling to become a sonographer, I
found that SDMS is one of the most recognizable
societies out there for sonography. The organization
is very informative on standards, technology, patient
care, etc. Knowing all this, I wanted to become a
member because I knew it would not only benefit
me while I’m in school, but as I continue my career in
ultrasound.”
The SDMS Foundation provides a variety of
scholarship and grant opportunities to SDMS
members. The Sonography Student Scholarship
Program provides a $2,500 scholarship to a
sonography student who has been accepted or
is currently enrolled in a educational program in
diagnostic medical sonography or cardiovascular
technology that has been accredited by the
Commission on Accreditation of Allied Health
Educational Programs (CAAHEP). Two sonography
student scholarships are awarded a year. Recipients
can use the scholarship for tuition, books, or
educational fees.

Jordyn Leyerzapf is a second
year student in the Medical
Diagnostic Imaging Program
at Fort Hays State University.
She has already obtained a
Bachelor’s degree in Animal
Science and decided to
continue her education
by pursuing an additional
degree in Medical Diagnostic
Imaging. Jordan is from a
farming community in Northwest Kansas. She was
married a year and half ago and recently moved back
to the family farm. Through her additional education,
Jordyn and her husband hope to expand their
breeding heifer operation by utilizing sonography to
detect early pregnancy in their artificially inseminated
heifers.
Jordyn has always had an interest in sonography. She
said, “The technology is continually advancing and
changing to better help patients. It is a non-invasive
technique that is used for a variety of practices and
the sonographer plays an instrumental role in patient
care.” She said there seems to be a pre-conceived
notion that sonography’s main purpose is only
imaging women during pregnancies when in fact
there are many other ways sonography is used, such
as in physician-guided procedures. “The wide variety
of exams keeps the day interesting” Jordyn says.
Jordyn said her favorite SDMS benefit is the online
Journal of Diagnostic Medical Sonography (JDMS).
She enjoys reading the variety of articles offered and
gaining knowledge about new techniques which
contribute to her continuing education and scanning
abilities. In addition, the JDMS makes a great
research resource.

What inspired you to make a career of sonography?
My mother had an ultrasound of her aorta when I was
in high school. I became interested in sonography
as a career and asked the sonographer what colleges
offered ultrasound programs. She told me that at
this time, the only program in the state of Iowa was
at the University of Iowa and that I would need to go
to Radiology Technology school first. I applied to the
School of Radiolgic Technology at the University of
Iowa and was accepted. After radiology technology
school, I applied to the University of Iowa’s Diagnostic
Medical Sonography program and was accepted.
Did you have a memorable role model/teacher?
Who was it?
My diagnostic medical sonography program director
was my mentor and role model. After I graduated,
my mentor was Glenda Mueller, the supervisor in
diagnostic medical sonography at my first job as a
sonographer. Glenda could image every anatomical
system with ultrasound and made everything look
so easy. She also was very gifted at sharing her
knowledge and teaching us any of the new imaging
modalities. She always made sure that everyone
had the education and training to implement new
technology as it became available in our lab.
What was the most significant advance in the
technology that you directly experienced?
I would say 3D technology. While I still think that we,
as a profession, are developing new uses for 3D, the
impact it has had on cardiac valve anatomy with TEE,
obstetrical sonography and gynecological sonography
have changed our imaging protocols for certain exam
indications. I believe the full use of this technology
is yet to come, and I do think it will impact how we
practice and educate for the future.
Were there any memorable developments, cases
or events that changed the way you practiced
sonography?
The growth of image resolution and how we
image the fetal heart has evolved from when I first
started performing obstetrical and pediatric cardiac
sonography. We have seen professional organizations
advocate adding screening heart views to obstetric
protocols and we are detecting defects earlier.
What changes do you predict in the immediate
future?
I think we will continue to see an evolution of how 3D
technology can add to sonographic exams across all
disciplines.
What advice would you give to students/ future
sonographers?
Run to the new technology and never be comfortable
continuing to practice the way you were taught. Jump
to learn and implement the new technology as it
becomes available, this will always keep you current
and add to your value in your imaging lab.
What’s your favorite part of your job?
My favorite part is working with new students
preparing to enter the field. I enjoy working with
students as they begin to explore diagnostic medical
sonography as a career and then watching them be
accepted into a program, and then graduating and
entering the profession. Many go on to publish,
become educators, supervisors and want to contribute
to the future shaping of our profession.
