Je<strong>an</strong> White, RVT, RPhS, president <strong>an</strong>d coowner<br />

<strong>of</strong> Quality Vascular Imag<strong>in</strong>g <strong>in</strong> Venice,<br />

FL, is among the first vascular sonographers<br />

to add the Registered Phlebology Sonographer<br />

(RPhS) credential to her name. In March, she<br />

was among the first to take <strong>an</strong>d successfully<br />

pass the new credential<strong>in</strong>g exam <strong>of</strong>fered by<br />

Cardiovascular Credential<strong>in</strong>g International (CCI.)<br />

White believes the new credential is <strong>in</strong>dicative<br />

<strong>of</strong> the grow<strong>in</strong>g role <strong>of</strong> vascular sonographers<br />

today. “We don’t just do exams to rule out<br />

deep ve<strong>in</strong> thrombosis (DVT) <strong>an</strong>ymore,” she<br />

says. “These days, vascular sonographers are<br />

f<strong>in</strong>d<strong>in</strong>g themselves work<strong>in</strong>g with radiologists,<br />

cardiologists <strong>an</strong>d vascular surgeons on a<br />

number <strong>of</strong> <strong>in</strong>terventional procedures <strong>an</strong>d<br />

screen<strong>in</strong>g exams for diseases <strong>of</strong> the ve<strong>in</strong>s.”<br />

White <strong>an</strong>d her colleagues also expect the<br />

dem<strong>an</strong>d for credentialed vascular sonographers<br />

<strong>an</strong>d their services to cont<strong>in</strong>ue to <strong>in</strong>crease as<br />

the population ages <strong>an</strong>d as, unfortunately,<br />

obesity <strong>an</strong>d diabetes become epidemics <strong>in</strong> this<br />

country.<br />

Laurence Needlem<strong>an</strong>, MD, a radiologist <strong>an</strong>d<br />

associate pr<strong>of</strong>essor at Thomas Jefferson<br />

University Hospital <strong>in</strong> Philadelphia, says the<br />

grow<strong>in</strong>g concerns about radiation safety <strong>an</strong>d<br />

cost conta<strong>in</strong>ment also will cont<strong>in</strong>ue to shift more<br />

vascular diagnostic <strong>an</strong>d treatment procedures<br />

away from CT <strong>an</strong>d MRI sc<strong>an</strong>ners <strong>an</strong>d more<br />

toward sonography. “We’re go<strong>in</strong>g to see some<br />

dim<strong>in</strong>ution <strong>in</strong> the amount <strong>of</strong> CTs <strong>an</strong>d MRIs<br />

performed <strong>an</strong>d some <strong>of</strong> that [work] tr<strong>an</strong>sfer to<br />

sonography,” says, Needlem<strong>an</strong>, who was a presenter<br />

at the <strong>Society</strong> <strong>of</strong> Vascular Ultrasound (SVU) <strong>an</strong>nual<br />

meet<strong>in</strong>g <strong>in</strong> Boston <strong>in</strong> June.<br />

Today, ultrasound equipment has become so<br />

technically adv<strong>an</strong>ced that it’s possible to clearly see<br />

not only the vessels <strong>an</strong>d everyth<strong>in</strong>g around them,<br />

but even to characterize the disease, says Phil<br />

Bendick, PhD, director <strong>of</strong> surgical research at William<br />

Beaumont Hospital <strong>in</strong> Detroit. As a result, he says,<br />

vascular ultrasound is no longer just a prelim<strong>in</strong>ary<br />

tool to see if further imag<strong>in</strong>g studies (X-ray, CT, MR,<br />

PET) are needed but <strong>in</strong> m<strong>an</strong>y cases be<strong>in</strong>g used <strong>in</strong><br />

place <strong>of</strong> them.<br />

For example, “it used to be that vascular sonography<br />

was used to make a prelim<strong>in</strong>ary diagnosis <strong>of</strong> stenosis<br />

(narrow<strong>in</strong>g) <strong>of</strong> the carotid arteries (the ma<strong>in</strong> arteries<br />

<strong>in</strong> the neck that supply blood to the bra<strong>in</strong>,)” Bendick<br />

says. “A sonogram would be the triage tool to decide<br />

whether to have <strong>an</strong> additional imag<strong>in</strong>g study done.”<br />

Today, Bendick says, ultrasound equipment is so<br />

adv<strong>an</strong>ced that people <strong>in</strong> the vascular laboratory trust<br />

it as a primary diagnostic tool. “In this country today,<br />

the majority <strong>of</strong> carotid <strong>in</strong>terventions are done based<br />

on a diagnostic sonogram without further imag<strong>in</strong>g.<br />

If there is a question, then yes, you always have<br />

<strong>an</strong>giography. But we’re able to use sonography – to<br />

comb<strong>in</strong>e the percentage <strong>of</strong> stenosis <strong>an</strong>d characterize<br />

the lesions <strong>in</strong> terms <strong>of</strong> likely stability – to determ<strong>in</strong>e<br />

whether the patient needs treatment or watchful<br />

wait<strong>in</strong>g. You’re go<strong>in</strong>g to do <strong>an</strong>giography to rule out<br />

unfavorable <strong>an</strong>atomy <strong>an</strong>d to help with placement <strong>of</strong><br />

the stent, but the <strong>an</strong>giogram is part <strong>of</strong> the treatment,<br />

it’s not part <strong>of</strong> the diagnosis because vascular<br />

sonography is now much more a primary diagnostic<br />

tool th<strong>an</strong> it ever has been.”<br />

3D, 4D Vascular Sonograms Hold Much Promise<br />

Three-dimensional (3D) <strong>an</strong>d 4D still are relative<br />

newcomers to the vascular world, Bendick says.<br />

But he suspects that as the technology cont<strong>in</strong>ues<br />

to adv<strong>an</strong>ce, it will me<strong>an</strong> <strong>an</strong> even bigger role for<br />

sonography <strong>in</strong> the vascular lab. “We have some<br />

studies <strong>in</strong> my own lab look<strong>in</strong>g at us<strong>in</strong>g 3D <strong>an</strong>d 4D to<br />

characterize plaque surface look<strong>in</strong>g for ulcerations.<br />

The applications have not been proven yet,” he says,<br />

“but it’s certa<strong>in</strong>ly worth <strong>in</strong>vestigat<strong>in</strong>g <strong>an</strong>d opens<br />

vascular sonography to whole new applications if<br />

we c<strong>an</strong> use it to better characterize plaque.” The<br />

research is excit<strong>in</strong>g, he says, <strong>an</strong>d <strong>in</strong> part what makes<br />

vascular sonography fun. “Never discount the fun<br />

factor <strong>in</strong> sonography,” he says.<br />

Bendick also sees potential for 3D <strong>an</strong>d 4D vascular<br />

sonograms to play <strong>an</strong> even greater role <strong>in</strong> detect<strong>in</strong>g<br />

peripheral artery disease (PAD) <strong>an</strong>d for look<strong>in</strong>g for<br />

<strong>an</strong>eurismal disease. “All <strong>of</strong> this is <strong>in</strong> its <strong>in</strong>f<strong>an</strong>cy <strong>in</strong><br />

terms <strong>of</strong> sonography,” he says, “but it’s on its way.<br />

The ultrasound mach<strong>in</strong>e has just gotten so good –<br />

the computer technology is faster, the image quality<br />

is <strong>in</strong>credible, <strong>an</strong>d the probes are better … so you<br />

get to see objects that are deeper <strong>in</strong> the body <strong>an</strong>d<br />

with more clarity,” he says. Doppler, which is the<br />

technology for imag<strong>in</strong>g blood flow, also cont<strong>in</strong>ues to<br />

improve. “Color Doppler imag<strong>in</strong>g is now miles <strong>an</strong>d<br />

miles ahead <strong>of</strong> where it was only four, five years ago.<br />

The images it provides are much brighter <strong>an</strong>d more<br />

brilli<strong>an</strong>t, giv<strong>in</strong>g us a much better def<strong>in</strong>ition <strong>of</strong> the<br />

lumen (the central open<strong>in</strong>g <strong>of</strong> the ve<strong>in</strong> through which<br />

the blood flows.)”<br />

Bendick says that when contrast is approved for<br />

vascular applications as it is now for cardiac,<br />

vascular sonograms will play <strong>an</strong> even bigger role <strong>in</strong><br />

the diagnosis <strong>an</strong>d treatment <strong>of</strong> peripheral vascular<br />

disease. “A little bit <strong>of</strong> contrast enables you to<br />

visualize the orig<strong>in</strong> <strong>of</strong> renal arteries <strong>in</strong> virtually<br />

everyone, <strong>an</strong>d allows you to very clearly del<strong>in</strong>eate the<br />

lumen from lesion <strong>an</strong>d wall. There’s a lot that contrast<br />

will enable us to do <strong>in</strong> a still strictly non-<strong>in</strong>vasive<br />

way <strong>an</strong>d <strong>in</strong>crease our accuracy for m<strong>an</strong>y diagnostic<br />

procedures,” he says.<br />

Vascular sonography benefits from fusion technology<br />

In fall 2008, one <strong>of</strong> the ultrasound equipment<br />

m<strong>an</strong>ufacturers <strong>in</strong>troduced <strong>an</strong> ultrasound system for<br />

radiology <strong>an</strong>d vascular applications that allows the<br />

cl<strong>in</strong>ici<strong>an</strong> to fuse sonographic images with those from<br />

other imag<strong>in</strong>g modalities such as CT <strong>an</strong>d MR. “To<br />

be able to fuse a sonogram with a CT sc<strong>an</strong> is a big<br />

deal,” says George H. Meier, MD, RVT, FACS, pr<strong>of</strong>essor<br />

<strong>an</strong>d chief <strong>of</strong> vascular surgery at the University <strong>of</strong><br />

C<strong>in</strong>c<strong>in</strong>nati College <strong>of</strong> Medic<strong>in</strong>e. The technology has<br />

m<strong>an</strong>y applications but it’s prov<strong>in</strong>g to be extremely<br />

valuable <strong>in</strong> look<strong>in</strong>g at abdom<strong>in</strong>al aortic <strong>an</strong>eurysms


<strong>an</strong>d endovascular <strong>an</strong>eurysms (abdom<strong>in</strong>al aortic <strong>an</strong>d<br />

thoracic aortic) that have been repaired. “This allows<br />

us to look at th<strong>in</strong>gs with sonography that before were<br />

relegated to CT sc<strong>an</strong>ners,” he says. Be<strong>in</strong>g able to use<br />

sonography rather th<strong>an</strong> CT to follow endovascular<br />

repairs, for example, is a huge adv<strong>an</strong>tage to the<br />

patient because the patient is not exposed to all that<br />

radiation from the CT sc<strong>an</strong>ner.<br />

Vascular sonography’s role <strong>in</strong> treat<strong>in</strong>g varicose ve<strong>in</strong>s,<br />

patients with kidney failure <strong>in</strong>creas<strong>in</strong>g<br />

Two other areas where improvements <strong>in</strong> technology<br />

have <strong>in</strong>creased the role <strong>of</strong> the vascular sonographer<br />

are <strong>in</strong> the treatment <strong>of</strong> varicose ve<strong>in</strong>s <strong>an</strong>d arterial<br />

venous fistulas for patients undergo<strong>in</strong>g dialysis.<br />

For m<strong>an</strong>y years, the st<strong>an</strong>dard procedure for treat<strong>in</strong>g<br />

large ve<strong>in</strong>s was ve<strong>in</strong> stripp<strong>in</strong>g – a surgical procedure<br />

that <strong>in</strong>volves remov<strong>in</strong>g a long ve<strong>in</strong> (the great or<br />

the small saphenous ve<strong>in</strong>) through small <strong>in</strong>cisions<br />

at the gro<strong>in</strong>, knee <strong>an</strong>d <strong>an</strong>kle. Patients had to be<br />

hospitalized, required <strong>an</strong>esthesia, <strong>an</strong>d could take<br />

two weeks or more to heal. In most cases today,<br />

ve<strong>in</strong> stripp<strong>in</strong>g has been replaced by endovenous<br />

thermal ablation – a m<strong>in</strong>imally <strong>in</strong>vasive procedure<br />

that is <strong>of</strong>ten performed <strong>in</strong> the suite <strong>of</strong> a phlebologist,<br />

vascular surgeon, cardiologist or radiologist. The<br />

ablation c<strong>an</strong> be done with radi<strong>of</strong>requency, lasers<br />

or chemicals. The dem<strong>an</strong>d for the procedure is<br />

<strong>in</strong>creas<strong>in</strong>g <strong>an</strong>d vascular sonographers are <strong>in</strong> the<br />

forefront as they assist the physici<strong>an</strong>s before, dur<strong>in</strong>g<br />

<strong>an</strong>d after the procedure, Je<strong>an</strong> White says. “Two days<br />

later, the patient comes back <strong>an</strong>d you do a follow-up<br />

exam, which is more like the exam to rule out DVT.<br />

You’re look<strong>in</strong>g to make sure the ve<strong>in</strong> that you treated<br />

is occluded as well as whether or not they develop a<br />

heat <strong>in</strong>duced thrombosis (clot.) It’s a pretty new th<strong>in</strong>g<br />

<strong>an</strong>d pretty awesome <strong>in</strong> the vascular world,” White<br />

says.<br />

Natalie Marks, MD, RVT, RDMS, technical director<br />

<strong>of</strong> the vascular lab at Maimonides Medical Center <strong>in</strong><br />

Brooklyn, N.Y., says vascular sonographers also are<br />

play<strong>in</strong>g a greater role <strong>in</strong> treat<strong>in</strong>g patients with kidney<br />

failure who must undergo dialysis. Dialysis requires<br />

access to the bloodstream. The majority <strong>of</strong> patients<br />

undergo creation <strong>of</strong> <strong>an</strong> arterio-venous fistula on<br />

the forearm or arm which requires a m<strong>in</strong>or surgical<br />

procedure <strong>in</strong> which <strong>an</strong> artery <strong>an</strong>d ve<strong>in</strong> are jo<strong>in</strong>ed<br />

under the sk<strong>in</strong>. “Arterio-venous fistulas <strong>of</strong>ten take<br />

several months to mature <strong>an</strong>d to be made ready for<br />

dialysis,” Marks says.<br />

At Maimonides, Marks says, their surgeons <strong>an</strong>d<br />

sonographers work together to help accelerate<br />

maturation <strong>of</strong> newly created fistulas under<br />

sonographic guid<strong>an</strong>ce rather th<strong>an</strong> X-ray or<br />

<strong>an</strong>giograms. “Unfortunately, after dialysis start,<br />

these fistulas <strong>of</strong>ten develop scar tissue creat<strong>in</strong>g<br />

narrow<strong>in</strong>gs, which need to be treated to cont<strong>in</strong>ue<br />

successful use. These fail<strong>in</strong>g fistulas are treated with<br />

balloon <strong>an</strong>gioplasties guided by sonography as well.<br />

Because they are done with sonography rather th<strong>an</strong><br />

X-ray, it becomes a quick <strong>of</strong>fice procedure rather<br />

th<strong>an</strong> a hospitalization. Marks says. “As <strong>an</strong> outpatient<br />

procedure, it’s not only less costly but it’s also safer<br />

because m<strong>an</strong>y <strong>of</strong> the patients who have fail<strong>in</strong>g<br />

kidneys are fight<strong>in</strong>g for every dialysis-free day <strong>an</strong>d<br />

couldn’t tolerate the contrast agent that is needed to<br />

perform the procedure with X-ray.”<br />

Maimonides vascular department pioneers<br />

procedure for peripheral artery disease<br />

Vascular surgeons <strong>an</strong>d technologists at Maimonides<br />

have pioneered <strong>an</strong>other exceptional sonography<br />

application to guide endovascular procedures for<br />

blocked arteries <strong>in</strong> the legs. Traditionally, these<br />

procedures are performed via a puncture <strong>in</strong> the<br />

gro<strong>in</strong> under local <strong>an</strong>esthesia <strong>an</strong>d require the use<br />

<strong>of</strong> fluoroscopy (radiation exposure) <strong>an</strong>d contrast<br />

material. In m<strong>an</strong>y patients the contrast material<br />

necessary to visualize the arteries on X-ray may<br />

cause severe allergic reactions or even kidney failure.<br />

The risk <strong>of</strong> kidney damage is <strong>in</strong>creased <strong>in</strong> diabetic<br />

patients, Marks says. Additionally, as vascular<br />

surgeons prefer these m<strong>in</strong>imally <strong>in</strong>vasive procedures<br />

<strong>an</strong>d perform them more <strong>of</strong>ten, the patients become<br />

<strong>in</strong>creas<strong>in</strong>gly susceptible to the complications <strong>of</strong><br />

radiation exposure among which are impaired vision,<br />

sk<strong>in</strong> damage <strong>an</strong>d <strong>in</strong>fertility, Marks says.<br />

In multiple publications, Enrico Ascher, MD,<br />

Marks <strong>an</strong>d colleagues have shown the safety <strong>an</strong>d<br />

durability <strong>of</strong> perform<strong>in</strong>g balloon <strong>an</strong>gioplasties<br />

under sonographic guid<strong>an</strong>ce alone without the<br />

use <strong>of</strong> contrast material or radiation exposure.<br />

The Maimonides group has successfully utilized<br />

sonography to guide balloon <strong>an</strong>gioplasties with or<br />

without stent<strong>in</strong>g <strong>in</strong> hundreds <strong>of</strong> patients, she says.<br />

“This unique experience is very excit<strong>in</strong>g <strong>an</strong>d promis<strong>in</strong>g<br />

because it <strong>of</strong>fers <strong>an</strong>other alternative to patients who<br />

are at <strong>in</strong>creased risk <strong>of</strong> develop<strong>in</strong>g kidney failure<br />

or severe allergic reactions from contrast material.<br />

It also <strong>of</strong>fers vascular surgeons with a me<strong>an</strong>s to<br />

m<strong>in</strong>imize radiation exposure with obvious benefits to<br />

their health.”


Vascular procedure for MS also shows promise<br />

Ann Marie Kup<strong>in</strong>ski, PhD, RVT, RDMS president <strong>an</strong>d<br />

co-owner <strong>of</strong> North Country Vascular Diagnostics <strong>in</strong><br />

Glen Falls, N.Y., <strong>an</strong>d cl<strong>in</strong>ical associate pr<strong>of</strong>essor <strong>of</strong><br />

radiology at Alb<strong>an</strong>y Medical College, says <strong>an</strong>other new<br />

area for vascular sonography that’s promis<strong>in</strong>g – yet<br />

somewhat controversial – is imag<strong>in</strong>g <strong>of</strong> the ve<strong>in</strong>s that<br />

dra<strong>in</strong> blood from the bra<strong>in</strong>. “Some people believe a<br />

condition known as chronic cerebral sp<strong>in</strong>al venous<br />

<strong>in</strong>sufficiency seems to be present <strong>in</strong> patients with<br />

multiple sclerosis,” she expla<strong>in</strong>s.<br />

Researchers <strong>in</strong> Italy have found that some <strong>of</strong> the<br />

ve<strong>in</strong>s <strong>in</strong> the bra<strong>in</strong> don’t seem to be as open as they<br />

should <strong>an</strong>d so the blood doesn’t flow from the bra<strong>in</strong><br />

as quickly as it should. “We c<strong>an</strong> do a sonogram<br />

<strong>an</strong>d evaluate these ve<strong>in</strong>s. Then the surgeon or<br />

<strong>in</strong>terventional radiologist will go <strong>in</strong>to the ve<strong>in</strong> <strong>an</strong>d<br />

dilate it with <strong>an</strong>gioplasty if necessary. This procedure<br />

seems to lessen the patient’s symptoms,” Kup<strong>in</strong>ski<br />

says. The procedure is <strong>of</strong>f-label <strong>an</strong>d <strong>in</strong>sur<strong>an</strong>ce won’t<br />

cover it. “But patients who are f<strong>in</strong>d<strong>in</strong>g out about it are<br />

call<strong>in</strong>g by the hundreds to get ve<strong>in</strong>s evaluated <strong>an</strong>d<br />

to see if they c<strong>an</strong> be helped by the procedure,” she<br />

says. “One <strong>of</strong> the radiologists I work with is booked<br />

through the summer with four to five patients a day<br />

dem<strong>an</strong>d<strong>in</strong>g these procedures.” Hopefully, Kup<strong>in</strong>ski<br />

says, further research will validate the procedure <strong>an</strong>d<br />

“the government will get on board <strong>an</strong>d pay for it.”<br />

Downsiz<strong>in</strong>g equipment, credential<strong>in</strong>g hot topics for<br />

vascular sonographers<br />

Another welcome trend <strong>in</strong> vascular sonography,<br />

Kup<strong>in</strong>ski says, is the downsiz<strong>in</strong>g <strong>of</strong> ultrasound<br />

equipment. “Some comp<strong>an</strong>ies have been really<br />

attentive to portability <strong>an</strong>d ergonomics <strong>in</strong> a big,<br />

big way. Pretty much everyone is m<strong>an</strong>ufactur<strong>in</strong>g<br />

equipment now that is much more adjustable<br />

<strong>an</strong>d lighter – all <strong>in</strong> <strong>an</strong> attempt to reduce workrelated<br />

<strong>in</strong>juries – <strong>an</strong>d that’s a good th<strong>in</strong>g for all<br />

sonographers, not just those who work <strong>in</strong> vascular,”<br />

she says. Because the equipment is portable, it’s also<br />

allow<strong>in</strong>g vascular sonographers to provide services<br />

<strong>in</strong> rural areas that they couldn’t previously.” Patients<br />

no longer have to travel to the major cities for the<br />

diagnosis <strong>an</strong>d treatment <strong>of</strong> their vascular issues,<br />

Kup<strong>in</strong>ski says.<br />

Credential<strong>in</strong>g <strong>an</strong>d accreditation <strong>of</strong> labs are yet<br />

<strong>an</strong>other big concern these days <strong>of</strong> vascular<br />

sonography world. Needlem<strong>an</strong> says credential<strong>in</strong>g,<br />

accreditation <strong>an</strong>d education were hot topics at the<br />

SUV meet<strong>in</strong>g he attended <strong>in</strong> June. “Basically,” he<br />

says, “it is hoped that by 2012, all those enter<strong>in</strong>g<br />

the field will be graduates <strong>of</strong> accredited schools.”<br />

Certification requirements could become a fact, he<br />

says, if a bill pend<strong>in</strong>g <strong>in</strong> Congress, the CARE bill, were<br />

to become law.<br />

However, Needlem<strong>an</strong> says, it’s a concern because<br />

the need for vascular sonographers is outstripp<strong>in</strong>g<br />

the schools’ ability to produce enough graduates.<br />

The dem<strong>an</strong>d for all sonographers <strong>in</strong>clud<strong>in</strong>g vascular<br />

is on the rise because <strong>of</strong> the demographics <strong>of</strong> the<br />

population – not only are patients gett<strong>in</strong>g older<br />

<strong>an</strong>d need<strong>in</strong>g more medical services, he says, but<br />

sonographers themselves are ag<strong>in</strong>g <strong>an</strong>d retir<strong>in</strong>g. No<br />

one at the SVU meet<strong>in</strong>g could accurately estimate the<br />

number <strong>of</strong> sonographers who are retir<strong>in</strong>g, Needlem<strong>an</strong><br />

says, but everyone agreed that the need for vascular<br />

sonographers is strong <strong>an</strong>d will cont<strong>in</strong>ue to be for<br />

years to come especially as the number <strong>of</strong> procedures<br />

they’re <strong>in</strong>volved <strong>in</strong> exp<strong>an</strong>ds.<br />

Licens<strong>in</strong>g Requirements Effective July 1, 2010<br />

New Oregon Licensure July 1, 2010<br />

The follow<strong>in</strong>g new medical imag<strong>in</strong>g modalities must<br />

be licensed as <strong>of</strong> July 1, 2010:<br />

Nuclear Medic<strong>in</strong>e:<br />

Any person who operates nuclear medic<strong>in</strong>e<br />

equipment or uses radiopharmaceuticals on a live<br />

hum<strong>an</strong> body must have a license.<br />

Magnetic Reson<strong>an</strong>ce Imag<strong>in</strong>g (MRI):<br />

Any person who operates MRI equipment or<br />

places a live hum<strong>an</strong> body with<strong>in</strong> the magnetic field<br />

generated by the MRI equipment must be licensed.<br />

Sonography:<br />

Any person who operates sonographic equipment<br />

or passes energy through a live hum<strong>an</strong> body must<br />

be licensed.<br />

All medical Imag<strong>in</strong>g licensees must be credentialed<br />

by a credential<strong>in</strong>g org<strong>an</strong>ization <strong>in</strong> a medical imag<strong>in</strong>g<br />

modality recognized by the Board.<br />

Notice To Hospitals And Imag<strong>in</strong>g Facilities<br />

The Board needs your help <strong>an</strong>d underst<strong>an</strong>d<strong>in</strong>g. It is not<br />

the <strong>in</strong>tent or expectation <strong>of</strong> the Board to have all new<br />

modalities; MRI, Nuclear Medic<strong>in</strong>e <strong>an</strong>d Sonography<br />

licensed on July 1, 2010, when the new law takes effect.<br />

The Board considers this a tr<strong>an</strong>sitional period <strong>an</strong>d will<br />

be work<strong>in</strong>g with new modality applic<strong>an</strong>ts <strong>an</strong>d current<br />

licensees add<strong>in</strong>g MRI, Nuclear Medic<strong>in</strong>e or Sonography<br />

to their licensure.<br />

The Board needs your cooperation to make sure patient<br />

care is not compromised by jo<strong>in</strong><strong>in</strong>g them <strong>in</strong> this good<br />

faith effort to license these new modalities. Please ask<br />

your employees to send <strong>in</strong> their applications as close<br />

to the July 1 implementation date as possible. We will<br />

beg<strong>in</strong> process<strong>in</strong>g them <strong>in</strong> date order <strong>in</strong> accord<strong>an</strong>ce with<br />

our statutes <strong>an</strong>d rules.<br />

Dur<strong>in</strong>g this tr<strong>an</strong>sitional period, the Board will not<br />

take discipl<strong>in</strong>ary action aga<strong>in</strong>st the new applic<strong>an</strong>t or<br />

licensee add<strong>in</strong>g MRI, Nuclear Medic<strong>in</strong>e or Sonography<br />

for practic<strong>in</strong>g without a license. Our soon to be new<br />

licensees <strong>an</strong>d exist<strong>in</strong>g licensees should be allowed<br />

to practice their pr<strong>of</strong>ession(s) dur<strong>in</strong>g this tr<strong>an</strong>sitional<br />

period.<br />

This notice will be removed from the website <strong>an</strong>d the<br />

tr<strong>an</strong>sitional period will end at the discretion <strong>of</strong> the<br />

Board, however only after adequate time is given to our<br />

new licens<strong>in</strong>g modalities to br<strong>in</strong>g them <strong>in</strong> compli<strong>an</strong>ce<br />

I w<strong>an</strong>ted to write <strong>an</strong>d personally th<strong>an</strong>k each <strong>of</strong> you for select<strong>in</strong>g me as the 2009 scholarship recipient.<br />

The scholarship allowed me to complete my Masters <strong>in</strong> Adm<strong>in</strong>istrative Studies with <strong>an</strong> emphasis <strong>in</strong><br />

Communications at Missouri State University. With your support, I graduated on May 12, 2010. The<br />

scholarship allowed me to complete my Masters ahead <strong>of</strong> schedule which <strong>in</strong> turn has allowed our DMS<br />

program to apply to the Higher Learn<strong>in</strong>g Commission to turn our hospital-based sonography program <strong>in</strong>to a<br />

bachelor’s degree program. This is a very excit<strong>in</strong>g time for our faculty <strong>an</strong>d students.<br />

In addition, I would like to th<strong>an</strong>k each <strong>of</strong> you for your cont<strong>in</strong>ued persistence, vision <strong>an</strong>d passion toward our<br />

pr<strong>of</strong>ession which was demonstrated <strong>in</strong> the past year.<br />

What <strong>in</strong>spired you<br />

to make a career <strong>in</strong><br />

sonography?<br />

Actually, I k<strong>in</strong>d <strong>of</strong> fell <strong>in</strong>to sonography. I was <strong>in</strong> my<br />

first-year as <strong>an</strong> X-ray student <strong>an</strong>d realized that<br />

there was no way I w<strong>an</strong>ted to make a career as <strong>an</strong><br />

X-ray tech. However, I am not one that gives up on<br />

someth<strong>in</strong>g I start. There was a second-year X-ray<br />

student who was at the same cl<strong>in</strong>ical site as me <strong>an</strong>d<br />

she was go<strong>in</strong>g <strong>in</strong>to sonography. So, I figured I would<br />

give it a try, too. I really did not know much about<br />

sonography. I had not even had a rotation through<br />

it dur<strong>in</strong>g my X-ray tra<strong>in</strong><strong>in</strong>g. To be honest, once I<br />

started the sonography program, I thought I had<br />

made the biggest mistake <strong>of</strong> my life. How on earth<br />

could someone make <strong>an</strong>yth<strong>in</strong>g out <strong>of</strong> those dots on<br />

the screen? I started sonography do<strong>in</strong>g some static<br />

sc<strong>an</strong>n<strong>in</strong>g....(OK, I ‘m not that old...we also had realtime.)<br />

However, the image quality back then was<br />

noth<strong>in</strong>g like it is today.<br />

Did you have a memorable role model/teacher?<br />

Who was it?<br />

In the early ’80s, there were not a lot <strong>of</strong> sonography<br />

programs from which to choose. I was fortunate to<br />

have my tra<strong>in</strong><strong>in</strong>g at one <strong>of</strong> the best. My <strong>in</strong>structor<br />

was Marie DeL<strong>an</strong>ge from Loma L<strong>in</strong>da University.<br />

She is a great teacher <strong>an</strong>d friend. There is a<br />

sonographer from Loma L<strong>in</strong>da whom I observed a<br />

lot dur<strong>in</strong>g my tra<strong>in</strong><strong>in</strong>g: Curtis Serikaku, who was (<strong>an</strong>d<br />

I am sure still is) a perfectionist. Every femur was<br />

straight across the image, BPDs were perfect, CBDs<br />

were l<strong>in</strong>ed up to <strong>in</strong>clude the hepatic artery (HA) <strong>an</strong>d<br />

portal ve<strong>in</strong> (PV). He was not one that just went <strong>in</strong><br />

<strong>an</strong>d whipped out his study, he took the time to create<br />

a masterpiece with every case he did. I learned a lot<br />

from Curtis. The other person whom I need to give<br />

credit to is Dr. Tom Stavros. I was fortunate to work<br />

with a brilli<strong>an</strong>t sonologist (although I would never<br />

tell him that) for 15 years. We did a lot <strong>of</strong> research<br />

<strong>an</strong>d work <strong>in</strong> breast sonography, sonography <strong>of</strong> the<br />

appendix <strong>an</strong>d hernias. I worked <strong>in</strong> <strong>an</strong> adv<strong>an</strong>ced<br />

role as a sonographer practitioner. Part <strong>of</strong> my<br />

job description was to get <strong>in</strong>volved with different<br />

sonography org<strong>an</strong>izations <strong>an</strong>d to start lectur<strong>in</strong>g.<br />

What was the most signific<strong>an</strong>t adv<strong>an</strong>ce <strong>in</strong><br />

technology that you have directly experienced?<br />

I have been through m<strong>an</strong>y adv<strong>an</strong>ces <strong>in</strong> sonography;<br />

c<strong>in</strong>eloop – yes <strong>in</strong> the old days we use to have to<br />

keep freez<strong>in</strong>g <strong>an</strong>d unfreez<strong>in</strong>g to get the correct<br />

image. Color <strong>an</strong>d power Doppler, Elastography <strong>an</strong>d<br />

all k<strong>in</strong>ds <strong>of</strong> image improvements. I would say that the<br />

most signific<strong>an</strong>t adv<strong>an</strong>ce has been the improvement<br />

<strong>of</strong> the 2D image quality. In the old days, the dot size<br />

was about the same size as <strong>an</strong> Oreo cookie. We<br />

now have such good resolution that we see th<strong>in</strong>gs<br />

we have never seen before.<br />

Were there <strong>an</strong>y memorable developments,<br />

cases or events that ch<strong>an</strong>ged the way you<br />

practiced sonography?<br />

With the improvements to image quality, I have had<br />

to look up a lot <strong>of</strong> normal <strong>an</strong>atomy structures. One<br />

<strong>of</strong> the best reference books to have <strong>in</strong> a sonography<br />

department is Netter’s Atlas <strong>of</strong> Hum<strong>an</strong> Anatomy.<br />

I would say that breast sonography has had one<br />

<strong>of</strong> the biggest impacts with the improvements to<br />

image quality. We now have super high-frequency<br />

tr<strong>an</strong>sducers, broad-b<strong>an</strong>d technology, harmonics,<br />

compound imag<strong>in</strong>g, color <strong>an</strong>d power Doppler <strong>an</strong>d<br />

elastography. About 10 years ago, all breast cysts<br />

seemed to be simple, now we see debris <strong>an</strong>d<br />

echoes <strong>in</strong> a lot <strong>of</strong> breast cysts. This is not artifact; it<br />

is true <strong>in</strong>formation <strong>an</strong>d we see<strong>in</strong>g it because <strong>of</strong> all<br />

the improvements to image quality.<br />

What ch<strong>an</strong>ges do you predict <strong>in</strong> the immediate<br />

future?<br />

That is a good question. I th<strong>in</strong>k that the biggest<br />

ch<strong>an</strong>ge <strong>in</strong> the immediate future will be how healthcare<br />

reform will affect all <strong>of</strong> us? I believe that we<br />

will see <strong>an</strong> <strong>in</strong>crease <strong>in</strong> our volume. Other modalities<br />

such as MRI <strong>an</strong>d CT will have a greater impact. As


far as sonography equipment, we are already see<strong>in</strong>g<br />

systems gett<strong>in</strong>g smaller <strong>an</strong>d smaller. There is now a<br />

system about the same size <strong>of</strong> <strong>an</strong> iPod. Sonography<br />

will truly be the “stethoscope <strong>of</strong> the future.”<br />

What advice would you give to students/ future<br />

sonographers?<br />

My advice to students is make sonography a career<br />

not just a job. Take the time to learn all you c<strong>an</strong>,<br />

attend conferences (like SDMS <strong>in</strong> Denver this year),<br />

read journals <strong>an</strong>d get <strong>in</strong>volved.<br />

What’s your favorite part <strong>of</strong> your job?<br />

Currently, I work for <strong>an</strong> automated breast<br />

sonography comp<strong>an</strong>y. The favorite part <strong>of</strong> my job<br />

is giv<strong>in</strong>g lectures <strong>an</strong>d help<strong>in</strong>g others learn how to<br />

improve their breast sonography images.<br />

When asked what <strong>in</strong>spired Wieslawa to choose<br />

became <strong>in</strong>terested <strong>in</strong> sonography dur<strong>in</strong>g her two<br />

pregn<strong>an</strong>cies, <strong>an</strong>d when her mother was diagnosed<br />

with ovari<strong>an</strong> c<strong>an</strong>cer. She was amazed <strong>an</strong>d<br />

fasc<strong>in</strong>ated by the details, <strong>an</strong>d images which were<br />

obta<strong>in</strong>ed through the ultrasound.<br />

Wieslawa’s favorite benefit so far has been the<br />

Journal <strong>of</strong> Diagnostic Medical Sonography (JDMS).<br />
