New Mexico Licensure Bill Sets New Standard<br />

In early April the <strong>SDMS</strong> announced that New Mexico<br />

had become the first state in the country to establish<br />

a licensure law for sonographers. New Mexico<br />

Governor, Billy Richardson, signed the bill into law on<br />

April 6. This is a historic benchmark for our pr<strong>of</strong>ession.<br />

Prior to the passage <strong>of</strong> the New Mexico licensure<br />

law anyone could perform sonograms in the state,<br />

regardless <strong>of</strong> qualifications, education, or certification.<br />

The unregulated environment for sonography in New<br />

Mexico has been changed irrevocably, and for the<br />

better. The New Mexico licensure law sets educational<br />

and certification standards for medical imaging<br />

pr<strong>of</strong>essionals, including sonographers. It is a good law<br />

for patients, providers, and the pr<strong>of</strong>ession. We now<br />

have a model to point other states to as they consider<br />

changing the licensure environment for sonographers<br />

within their own state. The commitment to establish<br />

appropriate standards for those who choose to practice<br />

in our pr<strong>of</strong>ession has been and continues to be, the<br />

cornerstone <strong>of</strong> our strategic plan. The continued<br />

evolution <strong>of</strong> sonography as a pr<strong>of</strong>ession is dependent,<br />

in large part, on our being successful with the<br />

establishment <strong>of</strong> certification and education standards.<br />

As for education standards, a significant step was taken<br />

in that direction with the completion <strong>of</strong> the National<br />

Education Curriculum (NEC) last year. This was a<br />

<strong>SDMS</strong> facilitated process that resulted in the creation<br />

<strong>of</strong> a national curriculum for sonography education<br />

programs throughout the country. Having continuity in<br />

the sonography educational curriculum is one side <strong>of</strong><br />

the sonography quality-standards coin; the other side <strong>of</strong><br />

that coin is certification. In the case <strong>of</strong> New Mexico, the<br />

certification standard took the form <strong>of</strong> state licensure. To<br />

We have been asked if the passage <strong>of</strong> the New<br />

Mexico licensure law represents a change in course<br />

or strategy by the <strong>SDMS</strong> (federal-level certification).<br />

The short answer to that question is, ‘no’. <strong>SDMS</strong><br />

remains committed to the establishment <strong>of</strong> federal-level<br />

certification standards, and will continue to pursue this<br />

goal with the introduction <strong>of</strong> the CARE Bill in the 111 th<br />

Congress. However, what can be said regarding statebased<br />

efforts targeted at licensure is that <strong>SDMS</strong>, along<br />

with the Sonography Licensure Coalition (SLC), will be<br />

closely monitoring state-based proposals as they arise.<br />

Historically, <strong>SDMS</strong> has opposed specific efforts to create<br />

state-by-state licensure for sonographers, and instead<br />

focused its attention<br />

and resources on<br />

developing federal<br />

certification standards.<br />

However, at the state<br />

level it is clear that<br />

we are seeing an<br />

increased level <strong>of</strong><br />

frustration with the unregulated nature <strong>of</strong> sonography<br />

and its potential consequences for bad patient<br />

outcomes and healthcare cost inefficiency. This statelevel<br />

frustration has begun to express itself in the form<br />

<strong>of</strong> proposed ‘legislative fixes’; and while <strong>of</strong>ten wellintentioned,<br />

the initial statutory language has not always<br />

been sufficient to adequately address the problem.<br />

Where these initial legislative fixes have fallen short <strong>of</strong><br />

the mark <strong>SDMS</strong> has intervened, and will continue to do<br />

so, to ensure that the best interests <strong>of</strong> our members and<br />

the patients that they serve is addressed.<br />

<strong>SDMS</strong> is vigilant in monitoring legislative and regulatory<br />

activity at the state level. It is an integral part <strong>of</strong> what<br />

we do to protect your pr<strong>of</strong>ession, your career interests,<br />

and provide for public safety. While it may be true that<br />

the majority <strong>of</strong> sonographers have little understanding<br />

<strong>of</strong>, appreciation for, or interest in advocacy work, the<br />

fact is that <strong>SDMS</strong> maintains a highly competent team<br />

<strong>of</strong> pr<strong>of</strong>essionals to monitor and respond to legislative<br />

threats and opportunities. It is also our responsibility<br />

to keep the membership informed about legislative<br />

and regulatory changes that can potentially affect our<br />

members. It is the members’ responsibility to remain<br />

knowledgeable by reading the periodic updates that are<br />

provided in <strong>SDMS</strong> publications.<br />

We are learning our lessons about the nuances <strong>of</strong><br />

state-based licensure efforts, and one key lesson that<br />

we have learned is this…it is critical that the local<br />

membership notify and involve the <strong>SDMS</strong> <strong>of</strong>fice as early<br />

in the process as possible. As always, we welcome and<br />

Sonographers At Risk<br />

According to the National Institute <strong>of</strong> Occupational Safety<br />

and Health (NIOSH), sonographers are at risk for developing<br />

work-related musculoskeletal disorders such as inflammation<br />

<strong>of</strong> the tendons (tendonitis) or tendon sheaths (tenosynovitis),<br />

bursitis, muscle strains, and pathology <strong>of</strong> the nerves in the upper<br />

extremities, neck, and back. NIOSH recommends appropriate<br />

engineering controls, work practices, hazard communication,<br />

and training to prevent these work-related musculoskeletal<br />

disorders.<br />

What inspired you to<br />

start your career in<br />

sonography?<br />

Sonography chose me. My first job in the field was<br />

in 1965 as a research assistant in the lab <strong>of</strong> Joseph<br />

H. Holmes, MD, at the University <strong>of</strong> Colorado<br />

<strong>Medical</strong> Center in Denver. I had two research<br />

assistant job <strong>of</strong>fers there. One was working<br />

Monday to Friday and Saturday morning and killing<br />

rabbits to examine their brains and the other was<br />

Monday to Friday, no Saturdays for the same<br />

salary. The choice was easy especially since Dr.<br />

Holmes seemed like a nice man who had pictures<br />

in his billfold <strong>of</strong> scans <strong>of</strong> dogs taken in a waterbath<br />

machine. My next job in 1967 was for group<br />

<strong>of</strong> neurosurgeons and neurologists in Lynwood,<br />

CA, primarily performing echoencephalograms<br />

and abdominal sonograms looking for metastatic<br />

disease. I left the field from 1969-1974. In 1975, I<br />

started working at a new hospital in New Orleans<br />

that had state-<strong>of</strong>-the-art ultrasound equipment<br />

(articulated arm gray scale.) I have worked in<br />

sonography ever since. I moved to Dallas in<br />

1983 and was associated with Marveen Craig,<br />

RDMS, F<strong>SDMS</strong>, at the International Ultrasound<br />

Institute and in the 1990s I taught at El Centro<br />

Community College. In 1987, I started my own<br />

business, Sonographer Services, to provide sono<br />

and echo vacation relief services and to educate<br />

sonographers. I work part-time and enjoy keeping<br />

my hand in.<br />

What was the most significant advance in<br />

ultrasound technology that you directly<br />

experienced?<br />

I have seen many changes in the last 44 years. The<br />

first machine took two people to operate and was,<br />

<strong>of</strong> course, bi-stable. We would write the patent’s<br />

name on Scotch tape and put it on the screen<br />

so we could identify the images. Next came the<br />

articulated arm, still bi-stable. We have progressed<br />

to gray scale, real-time, spectral Doppler, color<br />

and power Doppler, 3D, 4D and laptop units. Now<br />

ultrasound is in space. In the echo arena, we<br />

started with A-mode, then M-mode, to 2D gray<br />

scale and all the different Doppler technologies.<br />

Hard copy documentation has changed from timelapse<br />

Polaroid pictures that had to be coated using<br />

a little sticky stick to all digital. The technology<br />

I enjoy the most (not counting better images) is<br />

digital imaging. No more cassettes and dark rooms,<br />

what a time-saver!<br />

What changes do you predict for the field in the<br />

immediate future?<br />

Only our imagination limits what the future holds.<br />

Many <strong>of</strong> the applications we use now, the pioneers<br />

in the field had envisioned but they were limited by<br />

the technology.<br />

What advice would you give to students/future<br />

sonographers?<br />

Get used to change. The best first job would be in a<br />

busy hospital department during a shift that allows<br />

you to work with an experienced sonographer. You<br />

can never know everything but the more curious<br />

you are, the more you learn.<br />

What’s your favorite part <strong>of</strong> your job?<br />

My favorite part is being able to customize each<br />

study for the patient to ferret out the needed<br />

information to help the doctor make a diagnosis.<br />

I know I have helped save lives by aiding in an<br />

early diagnosis. I don’t have any plans on total<br />

retirement. It’s just too interesting and I want to be<br />

there for the next big change.


New Member Scan<br />

This month’s New<br />

Member Scan pr<strong>of</strong>ile<br />

takes us out <strong>of</strong> the<br />

United States and to<br />

China. Juqian Zhang<br />

is a sonographer/<br />

physician in Chengdu,<br />

the capital <strong>of</strong> Sichuan<br />

Province in China.<br />

Chengdu has an<br />

important industrial<br />

base, with an area<br />

<strong>of</strong> 12,390 square<br />

kilometers and a<br />

population <strong>of</strong> 9.4 million, but it may be most familiar<br />

because <strong>of</strong> the 7.8 magnitude earthquake suffered<br />

in that region in May 2008. Dr. Zhang is recently<br />

married, has finished her college and graduate<br />

study at the West China College <strong>of</strong> Clinical Medicine<br />

<strong>of</strong> Sichuan University, and received her medical<br />

license last year. Dr. Zhang made the following<br />

observation regarding the differences in the clinical<br />

aspects <strong>of</strong> sonography in the United States and<br />

China, “Since the working system <strong>of</strong> diagnostic<br />

ultrasound is quite different in China from that in the<br />

US, I work as [the] sonographer and interpreting<br />

physician as well.”<br />

Dr. Zhang joined <strong>SDMS</strong> to get more information<br />

and opportunities in sonography. She says, “Since<br />

we are trained in China, the knowledge and skills<br />

taught are somewhat different from those taught in<br />

the States, and I find it very rewarding to be able to<br />

join <strong>SDMS</strong>. I have learned a lot from the webinars<br />

and conference materials posted on the website.”<br />

Regarding the future <strong>of</strong> sonography, Dr. Zhang<br />

hopes that “the <strong>SDMS</strong> will develop standardized<br />

practice guidelines and instructions, especially for<br />

foreign sonographers.” She explains that although<br />

these guidelines may seem to be too basic for<br />

sonographers in the US, they are the core <strong>of</strong><br />

everyday practice in China.<br />

Many thanks to Dr. Zhang for sharing her history<br />
