<strong>Automated</strong> <strong>Bre<strong>as</strong>t</strong> <strong>Ultr<strong>as</strong>ound</strong> <strong>Not</strong><br />

<strong>Seen</strong> <strong>as</strong> a <strong>Threat</strong> to Sonographers –<br />

Could Mean More Demand for Advanced Sonography Skills<br />

A traditional bre<strong>as</strong>t sonogram involves the<br />

sonographer manually manipulating the transducer<br />

over the bre<strong>as</strong>t. As with all sonograms, it is highly<br />

dependent on the skills <strong>of</strong> the operator.<br />

Over the l<strong>as</strong>t six years or so, three companies have<br />

introduced automated bre<strong>as</strong>t ultr<strong>as</strong>ound. The three<br />

are: SomoV by U-Systems <strong>of</strong> Silicon Valley, California,<br />

<strong>Automated</strong> Whole <strong>Bre<strong>as</strong>t</strong> <strong>Ultr<strong>as</strong>ound</strong> (AWBUS) by<br />

SonoCine, Inc, <strong>of</strong> Reno, Nevada, and the ACUSON<br />

S2000 <strong>Automated</strong> <strong>Bre<strong>as</strong>t</strong> Volume Scanner (ABVS) by<br />

Siemens Healthcare <strong>of</strong> Malvern, Pennsylvania.<br />

“The concept <strong>of</strong> automated bre<strong>as</strong>t ultr<strong>as</strong>ound is<br />

excellent,” says Cindy Rapp, BS, RDMS, RDCS, FSDMS,<br />

FAIUM, Senior Clinical Marketing Manager at Toshiba<br />

America Medical, and a specialist in bre<strong>as</strong>t ultr<strong>as</strong>ound.<br />

“It makes it possible for sonographers at different<br />

skill levels to be able to perform screening bre<strong>as</strong>t<br />

ultr<strong>as</strong>ound.”<br />

Each year in America, more than 200,000 women are<br />

newly diagnosed with bre<strong>as</strong>t cancer. Of women with<br />

bre<strong>as</strong>t cancer, nearly 40,000 will die. When<br />

detected early, bre<strong>as</strong>t cancer survival<br />

improves dramatically.<br />

Screening mammography h<strong>as</strong> long been<br />

the mainstay <strong>of</strong> detecting early, nonpalpable<br />

bre<strong>as</strong>t cancers which are the<br />

most curable. However, mammography<br />

is not <strong>as</strong> accurate in women with dense bre<strong>as</strong>ts <strong>as</strong> it is<br />

women with fatty bre<strong>as</strong>ts. And studies show women<br />

with dense or more glandular bre<strong>as</strong>ts are at higher<br />

risk <strong>of</strong> developing bre<strong>as</strong>t cancer. About 40 percent <strong>of</strong><br />

women have at le<strong>as</strong>t some dense bre<strong>as</strong>t tissue.<br />

More and more facilities are providing bre<strong>as</strong>t<br />

ultr<strong>as</strong>ound for women with dense bre<strong>as</strong>t tissue. A<br />

blinded, prospective, multi-center clinical trial <strong>of</strong><br />

women with dense bre<strong>as</strong>t tissue done by SonoCine<br />

found that more and smaller cancers are found<br />

with mammography and bre<strong>as</strong>t ultr<strong>as</strong>ound than by<br />

mammography alone.<br />

<strong>Not</strong> a replacement for mammograms<br />

<strong>Bre<strong>as</strong>t</strong> ultr<strong>as</strong>ound is seen <strong>as</strong> a complement and not<br />

a replacement for mammography. While demand<br />

for bre<strong>as</strong>t sonography is growing, if it can be done<br />

automatically, what does it all mean for sonographers?<br />

Admittedly some sonographers may feel threatened<br />

by automation. It doesn’t help, Rapp says, “that a<br />

lot <strong>of</strong> the automated bre<strong>as</strong>t ultr<strong>as</strong>ound equipment<br />

manufacturers are telling their customers you can<br />

have anyone run the machine and acquire the<br />

information.”<br />

But Rapp and others agree: the automation is good for<br />

sonographers and is in no way a threat to their jobs<br />

or their skills. Indeed, they say, if anything, automated<br />

bre<strong>as</strong>t ultr<strong>as</strong>ound could mean more work and more<br />

demand for the sonographers’ skills.<br />

“<strong>Automated</strong> acquisition takes bre<strong>as</strong>t ultr<strong>as</strong>ound<br />

to a more advanced level, incre<strong>as</strong>ing the value <strong>of</strong><br />

sonographers and their skills,” says Jacqueline Bailey,<br />

RDMS, RVT, RT, Global Marketing Manager at Siemens<br />

Medical Solutions, <strong>Ultr<strong>as</strong>ound</strong> Business Unit. “This is<br />

advancing sonographers, enhancing their skills and<br />

giving them more valuable responsibilities.”<br />

Chiqeeta Jameson, Director <strong>of</strong> Marketing for<br />

SonoCine, also sees the growing use <strong>of</strong> automated<br />

bre<strong>as</strong>t ultr<strong>as</strong>ound resulting in more demand for<br />

sonographers. “As more women are beginning to say,<br />

‘Mammography is not enough. I want an additional<br />

test to find out if there is something there,’ physicians<br />

are turning to automated bre<strong>as</strong>t ultr<strong>as</strong>ound for a more<br />

complete and comprehensive examination and that is<br />

job security for sonographers,” she says.<br />

It’s job security, Jameson says, because “this is an<br />

additional exam that will require more [sonographers]<br />

to perform, and thus the diagnostic exam and biopsy<br />

procedure that will ultimately follow.”<br />

Karen Emory, RT(R)(M), a senior clinical specialist with<br />

U-Systems, concedes that its system occ<strong>as</strong>ionally<br />

meets with some resistance from sonographers<br />

initially. But once they understand what it is all about,<br />

they’re happy their facilities acquired it, she says.<br />

“Truly, I can’t think <strong>of</strong> any facility we’ve been in where<br />

the sonographers aren’t ple<strong>as</strong>ed after they see what<br />

we’re doing and how the images are acquired. The<br />

sonographers realize it is <strong>of</strong> great benefit to them <strong>as</strong><br />

far <strong>as</strong> helping with their workday. It allows them to<br />

focus on diagnostic exams and biopsies.”<br />

“If anything, automated<br />

bre<strong>as</strong>t ultr<strong>as</strong>ound could mean<br />

more work and more demand<br />

for the sonographers’ skills.”<br />

Screening could mean more ultr<strong>as</strong>ound-guided<br />

bre<strong>as</strong>t biopsies<br />

If more bre<strong>as</strong>t screenings are performed with<br />

ultr<strong>as</strong>ound, it could lead to more ultr<strong>as</strong>ound guided<br />

biopsies, which require the skills <strong>of</strong> a trained<br />

sonographer, Bailey says. Automatic bre<strong>as</strong>t ultr<strong>as</strong>ound<br />

can provide a good overview <strong>of</strong> the bre<strong>as</strong>t tissue,<br />

but if specific are<strong>as</strong> need further review, handhelds<br />

could target them better. “<strong>Ultr<strong>as</strong>ound</strong> guided biopsy<br />

is where you need the skills <strong>of</strong> a trained sonographer,”<br />

says Jameson, a bre<strong>as</strong>t cancer survivor who wishes the<br />

automated bre<strong>as</strong>t ultr<strong>as</strong>ound technology had been<br />

available for her.<br />

Rapp likes that the automated bre<strong>as</strong>t ultr<strong>as</strong>ound takes<br />

the pressure <strong>of</strong>f the sonographer and puts it back on<br />

the radiologist. “A lot <strong>of</strong> times the sonographer will<br />

scan the bre<strong>as</strong>t and not see anything, but you could<br />

be less than a centimeter away from a lesion you<br />

didn’t catch,” she says. Because the automated bre<strong>as</strong>t<br />

ultr<strong>as</strong>ound scans the entire bre<strong>as</strong>t, it puts the onus<br />

<strong>of</strong> finding suspicious lesions back on the radiologist<br />

reading the results.<br />

Bailey says some radiologists using the ACUSON<br />

S2000 <strong>Automated</strong> <strong>Bre<strong>as</strong>t</strong> Volume Scanner are having<br />

mammographers do the scan but are <strong>as</strong>king their<br />

sonographers to look at the multitude <strong>of</strong> slices on the


workstation before they do. The sonographers who<br />

have the training are being <strong>as</strong>ked to provide the first<br />

look and bookmark are<strong>as</strong> <strong>of</strong> interest. “This is taking<br />

advanced sonographers and giving them more skills<br />

and responsibilities,” Bailey says.<br />

Should the radiologists see something on the<br />

ultr<strong>as</strong>ound slices that they want to look at more<br />

closely, they will have the sonographer do the followup<br />

scan using handheld ultr<strong>as</strong>ound, Bailey says. “They<br />

will <strong>as</strong>k the more trained sonographer to go and do a<br />

second look.”<br />

Automation helps cut down repetitive stress injuries<br />

Another advantage for the sonographer is that the<br />

automation cuts down on repetitive stress injuries that<br />

are causing many to leave the pr<strong>of</strong>ession. Screening<br />

bre<strong>as</strong>t ultr<strong>as</strong>ound requires very repetitive motions.<br />

Because it’s automated, it eliminates the repetitive<br />

motion injury issue that so many sonographers face<br />

early in their careers, Bailey says.<br />

Some facilities with SonoCine’s automated whole<br />

bre<strong>as</strong>t ultr<strong>as</strong>ound equipment are having their<br />

mammographers do the initial screenings, Jameson<br />

says. But while they train one or two mammographers,<br />

most sites also have a sonographer serve <strong>as</strong> the lead,<br />

she says. “There is a learning curve and this is why it’s<br />

advised a sonographer be one <strong>of</strong> the technologists in<br />

the SonoCine examination.”<br />

Sonographers at many sites are so overworked already<br />

they’re going to be happy if the responsibility falls to<br />

mammographers and not them, Rapp says.<br />

One <strong>of</strong> the drawbacks that could keep automated<br />

bre<strong>as</strong>t ultr<strong>as</strong>ound from taking <strong>of</strong>f <strong>as</strong> a diagnostic tool<br />

is the cost, Rapp notes. “One <strong>of</strong> the problems is there is<br />

no CPT code for screening bre<strong>as</strong>t ultr<strong>as</strong>ound. Women<br />

who are having it are paying out <strong>of</strong> their own pockets,<br />

unless they live in Connecticut, which just p<strong>as</strong>sed a<br />

law requiring it be covered.” California narrowly missed<br />

p<strong>as</strong>sing a similar law.<br />

While sonograms are far less costly than MRIs, which<br />

also are used for cancer screening in high-risk women,<br />

women may not want to pay out <strong>of</strong> pocket.<br />

More automation to come?<br />

Could bre<strong>as</strong>t ultr<strong>as</strong>ound be just the first <strong>of</strong> many<br />

sonograms to be automated?<br />

Bailey and Rapp don’t think so. “Screening bre<strong>as</strong>t<br />

ultr<strong>as</strong>ound with a handheld transducer is very tedious<br />

and very un-reproducible,” Bailey says. “Automating it<br />

makes the scan very efficient and reliable.”<br />

The main re<strong>as</strong>on automated bre<strong>as</strong>t ultr<strong>as</strong>ound<br />

equipment w<strong>as</strong> devised is that handheld screening<br />

bre<strong>as</strong>t ultr<strong>as</strong>ound is not e<strong>as</strong>y for everyone to do,<br />

Rapp says. “You’ve got to make sure you’ve looked at<br />

everything and have documentation.” No one wants<br />

to miss finding suspicious lesions. “There are probably<br />

more lawsuits filed for missing bre<strong>as</strong>t cancer than any<br />

other cancer,” Rapp says.<br />

The U-Systems and Siemens automated bre<strong>as</strong>t<br />

ultr<strong>as</strong>ound equipment use a larger transducer to<br />

automatically scan the entire bre<strong>as</strong>t in about 10<br />

minutes. “We don’t set dynamic range or what<br />

megahertz transducer we’re going to use,” Emory says.<br />

“The machine does that automatically.”<br />

“It’s about one minute for one volume scan <strong>of</strong> which<br />

typically three per bre<strong>as</strong>t are done to construct the<br />

image,” Bailey says. The three-dimensional scans are<br />

sent to the workstation where the radiologist reads<br />

them.<br />

Since 2005, U-Systems’ automated bre<strong>as</strong>t ultr<strong>as</strong>ound<br />

h<strong>as</strong> had clearance for diagnostic studies. In April,<br />

U-Systems received FDA panel approval for use <strong>of</strong> its<br />

automated bre<strong>as</strong>t screening device <strong>as</strong> an adjunct to<br />

mammography. “It’s really up to the facilities to decide<br />

how they’re going to use it,” Emory says.<br />

SonoCine uses the transducer <strong>of</strong> the facility’s existing<br />

ultr<strong>as</strong>ound machine to produce images in a movie<br />

format, which produces a fully documented, quality<br />

controlled and reproducible examination. The format<br />

allows cancers to be identified by dynamic disruption<br />

<strong>of</strong> normal tissue architecture between frames. “It’s <strong>as</strong><br />

if the doctors are standing there and looking at the<br />

entire image,” Jameson says.<br />

She ad libs her role. “They didn’t have a script,” Rapp<br />

One <strong>of</strong> the show’s four doctors, Lisa M<strong>as</strong>terson, an<br />

OB/GYN, w<strong>as</strong> supposed to demonstrate the equipment<br />

but they found it w<strong>as</strong> e<strong>as</strong>ier just to have Rapp do it.<br />

The episode w<strong>as</strong> supposed to air in May, but h<strong>as</strong> been<br />

delayed. (That’s show business.)<br />

Rapp says the show’s producers try to keep up with<br />

new developments in medicine. “When they Googled<br />

‘new in sonography,’ Toshiba’s 3D volume rendering<br />

‘Fly Thru’ caught their eye.”<br />

Why did your program make the commitment<br />

to acquire and retain CAAHEP programmatic<br />

accreditation?<br />

Caro: Our program goals are the same <strong>as</strong> CAAHEP<br />

standards: to provide an educational experience that<br />

reflects the accepted standards <strong>of</strong> the pr<strong>of</strong>ession and<br />

to graduate students who are clinically, technically<br />

and pr<strong>of</strong>essionally prepared to be sonographers.<br />

How have your students and faculty benefitted from<br />

being a CAAHEP accredited educational program?<br />

Caro: The students have set their goals to become<br />

registered sonographers, not only by presenting and<br />

obtaining a p<strong>as</strong>sing score in their SPI examination,<br />

but also by pursuing their specialty examination. The<br />

faculty benefits by helping to prepare students to<br />

work in an honorable pr<strong>of</strong>ession and by seeing their<br />

efforts rewarded.<br />

How have employers in your region benefited<br />

from having graduates from a CAAHEP accredited<br />

program available <strong>as</strong> prospective employees?<br />

Caro: Employees know they have quality and<br />

competent prospective employees who can stimulate<br />

the continuous learning experience <strong>of</strong> their current<br />

employed sonographers.<br />

Describe some <strong>of</strong> the positive <strong>as</strong>pects <strong>of</strong> CAAHEP<br />

accreditation and your relationship/partnership<br />

with your clinical education sites?<br />

Caro: Becoming an accredited program h<strong>as</strong> opened<br />

new opportunities to our continuous effort to bring<br />

new quality clinical affiliates. This will help us <strong>of</strong>fer our<br />

students and graduates an improved program and<br />

better employment opportunities.<br />

What are some things that sonographers and<br />

physicians in your region say about your CAAHEP<br />

accredited program?<br />

Caro: The physicians and sonographers acknowledge<br />

that we are a new program with ambitions <strong>of</strong> growth<br />

and improvement <strong>of</strong> our students competitiveness.<br />

They congratulated us for accomplishing accreditation<br />

standards and have committed to supporting our<br />

effort. Some <strong>of</strong> these affiliates have also hired some <strong>of</strong><br />

our graduates.<br />

What tips/suggestions do you have for education<br />

programs that are considering pursuing CAAHEP<br />

programmatic accreditation?<br />

Caro: <strong>Not</strong>hing worth obtaining is e<strong>as</strong>y. Our faculty<br />

knows this very well and is working every day<br />

to comply with the standards <strong>of</strong> accreditation. I<br />

invite those who are interested in programmatic<br />

accreditation to visit the CAAHEP webpage and read<br />

the standards and to work <strong>as</strong> a team pursuing this<br />

goal.
