Care Management of Mental and Physical Co-morbidities: A Triple Aim Bulls-Eye, DHHS award 1C1CMS331048 

(Unützer, PI of Subcontract), (11/1/12 - 10/31/15); $18,000,000 

Part of the $1 billion CMS Health Care Innovation Challenge, the Institute for Clinical Systems Improvement 

(ICSI) received funding to implement an evidence-based, multi-condition collaborative care management model 

for patients with depression plus diabetes and/or cardiovascular disease in primary care in seven states. The 

AIMS Center at UW will help design the program based on adaptations of the TEAMcare model and will train the 

implementing organizations in the model, provide technical assistance for implementation across all participating 

partners and participate in evaluation of the program.

Pathway to War-Related PTSD Care and Recovery: Access, Quality and Interventions, Defense Advanced 

Research Projects Agency (Co-PI: Katon; Project, PI: Engel), *(09/01/09-08/31/14) 

A multi-institutional consortium of research experts in medicine, epidemiology, survey design, health services 

research, clinical trials, social sciences, qualitative research, clinical decision support, health economics, health 

informatics, health care quality assessment, and biostatistics will complete the first major examination of the 

significant pathways to mental health services for military personnel returning from war and their families. This 

work will conduct a large multicenter randomized trial comparing optimized usual care to telephone case 

management using nurses collaborating with community providers and centrally supervised by mental health 

providers to improve continuity and quality of PTSD care.

Reducing Disparities & Improving Care for Depression in OB-GYN Clinic, NIMH R01 MH085668 (PI: Katon), 

(08/01/09-07/30/14); $2,316,585 

The overall goal of our research is to improve the detection of depression, quality of depression treatment, 

depressive symptoms, and functional outcomes of women with depression presenting to OB-GYN clinics. We 

propose a three-part intervention, integrated with OB-GYN care, consisting of: 1) enhanced education; 2) 

engagement; and 3) depression care management (with antidepressant monitoring and/or provision of brief 

psychotherapy. We hypothesize that a three-part depression intervention, integrated into the OB-GYN clinical 

setting, will improve treatment outcomes for depression, functional outcomes, and satisfaction with depression 

care, and will be a sustainable model of care.

Adolescent Collaborative Care Treatment for Depression, Seattle Children's Hospital Research Institute 

(PI: Richardson), (08/24/09-08/23/13); $70,126 

This project will test the Adolescent collaborative Care Treatment intervention, a model designed to improve 

major depression management among adolescents. The intervention will be tested in a population-based sample 

of adolescents in four primary care clinics. 


Swedish-Providence Family Medicine Residency Program (Providence campus), Swedish Family Medicine 

Contract (#FA18940), (7/01/12-6/30/13), $46,207 

The aim of this project is to provide ducational, instructional and professional training to family medicine residents 

at Providence Family Practice Medicine in addition to patient care. Resident training includes, but is not limited 

to, clinical instruction, didactics and Balint sessions. This contract with Providence Family Medicine has been in 

effect since July of 1980.

FOR MOMS: Culturally Relevant Treatment Services for Perinatal Depression, NIMH R01 MH084897-01A2

This proposed randomized, controlled trial will evaluate the effects of a culturally relevant, multicomponent 

intervention for antenatal depression (FOR MOMS, “Maintain Our Mothers’ Strength”) that has the potential to 

overcome patient, provider, and system-level barriers to care and engage depressed, low-income women in 

evidence-based treatments to reduce antenatal depressive symptoms, improve maternal psychosocial 

functioning, and ameliorate postpartum depression. 


Psychiatry-Primary Care Intervention Program, NIH Grant # 5 K24 MH 069471 08 (PI: Katon); 

(04/03/09-03/31/13, final year is a no-cost extension); $451,801 

Midcareer Investigator Award in Patient-Oriented Research (K-24) 

This K-24 will enhance Dr. Katon's expertise in epidemiology, biostatistics, health economics, organizational 

theory and disease management, which will improve his ability to mentor junior colleagues as well as to carry out 

the patient-oriented research proposed in his current NIMH R01 and other ongoing and new research proposals. 

The main research emphasis will be to expand Dr. Katon's work on depression and anxiety and medical 

comorbidity and his dissemination of collaborative care models. 


Telemental Health to Improve Mental Health Care and Outcomes for Children with ADHD in Rural Washington, 

NIMH R01 MH081997-01A1 (Myers), (09/01/08-08/31/13); $__________________ 

This study will test the efficacy of protocol-driven medication management and behavioral interventions via 

telemedicine for children between 7-11 years in rural Washington State who have been diagnosed with ADHD 

and treated by a pediatric primary care physician. 


Institutional National Research Service Award (NRSA) T-32, NIH Grant #5 T32 MH 020021, (07/01/08-06/30/13); 

$1,188,787 

The program is designed to produce primary care mental health clinical investigators by combining research 

projects and experience under the direction of a mentor with rigorous, research-based graduate level training in 

the School of Public Health’s Masters in Public Health Program. Two fellows will be admitted to the program each 

year, joining two second-year fellows for a total of four fellows per year. Two fellows will pursue adult mental 

health services research training and the other two fellows will train as child mental health services researchers. 

The program includes 5 components: 1) coursework toward MPH degree; 2) research project with supervision by 

a senior investigator at the UW or Group Health; 3) clinical rounds with the psychiatric consultation liaison service 

at University Hospital or Children's Hospital; 4) regular participation at teaching and research conferences; and 5) 

bimonthly meetings with either Dr. Katon or Dr. Richardson to evaluate ongoing course work, research and career 

opportunities.

Telephone Based Depression Care among Rural Latino Patients in Primary Care, NIMH R34 MH079191-01A1

This study aims to develop a culturally appropriate, sustainable, and cost-effective depression treatment program 

that can be incorporated into routine primary care practices serving low-income rural Latinos and that can be 

tested in a full scale randomized controlled trial.

A Randomized Trial of Liaison Psychiatry in Primary Care, NIMH #R01MH 41739-19 (PI: Katon),

This grant will test a disease-focused depression treatment program through a randomized controlled trial in 

which 300 persons with poorly controlled diabetes or CHD (i.e. HbA 1c >8.5%, blood pressure >140/90, LDL >130) 

and major depression/dysthymia are assigned to either: 1) a chronic disease-focused depression treatment 

program that addresses management of both depression and comorbid disease, or 2) care as usual.

CALM: Improving Primary Care Anxiety Outcomes, NIMH 2 U01 MH057858-06 (PI: Roy-Byrne),

The goal of this project is to test the effectiveness of an intervention strategy for four common anxiety disorders 

(panic, social phobia, PTSD, GAD) among a heterogeneous sample of primary care patients in five sites in the 

United States. 


Patient-Centered Interventions for Mood Disorders, NIMH P20 MH068572-03 (Co-PI: Katon); Group Health 

Cooperative (PI: Simon), (07/15/05-12/31/09); $66,017 

Group Health of Puget Sound Centers for Health Studies 

This project's aim is to develop a Center for Intervention and Services Research to rigorously evaluate and 

disseminate systematic patient-centered interventions for depression and bipolar disorder. Core goals guiding all 

Center activities will include the integration of phamacotherapy and behavioral interventions across the range of 

mood disorders, developing programs and systems to facilitate effective self-management, and use of information 

technology to promote self-management and effective patient-provider communication.

Depression-Diabetes: Care Disparities, Adverse Outcomes, NIMH R01 MH073686-03 (Co-PI: Katon);

This is a 5-year follow-up study of a cohort of 4800 patients with diabetes enrolled in Group Health that 

Dr. Katon's research team has already enrolled. The aim of this grant is to study whether depression is 

associated with increased mortality and complications of diabetes and to better understand the effect of 

depression on quality of disease control (lipid, blood pressure and glycemic control) and on self-management of 

diabetes (diet, exercise, smoking and adherence to medication).

Promoting Collaborative Care for Late Life Depression: Project IMPACT Coordinating Center, John A. Hartford

The principal goal of this grant is to disseminate the IMPACT care model nationally. 


Impact of Comorbid Anxiety Disorders in Adolescents with Asthma, NIMH R01 MH0678587-03, (PI: Katon); 

(04/01/04-03/31/08), $1,026,493. 

This grant will compare 1) the prevalence of meeting criteria for >1 anxiety or depressive disorders in a 

population-based sample of youth with asthma compared to a random sample of control youth, and 2) to describe 

the impact of anxiety/depression comorbidity on the personal, familial and societal impacts of asthma such as 

medical symptom burden, functional status, quality of life, medical costs and on the quality of asthma 

self-management.

The NRSA program trains physicians for academic research careers aimed at improving recognition and 

management of mental health problems encountered in primary medical care settings. The program recruits 

physicians in psychiatry and primary care for a minimum of a two-year fellowship. Fellows are mentored by 

senior health services researchers in the University of Washington Department of Psychiatry and Family Medicine 

and from the Center for Health Studies at Group Health Cooperative. The two-year training program has four 

components: 1) a Master of Public Health Degree program; 2) research experience in mental health programs in 

primary care settings; 3) experience with mentors who serve as role models, teachers, researchers and clinicians 

in the interface of medicine and psychiatry; and 4) clinical experience providing behavioral science consultations

To develop and test a mental illness diagnostic tool for use among farm workers with varying literacy skills in 

Washington. Oregon and Idaho. The study will determine whether the workers diagnosed with mental illness 

with this tool seek recommended follow-up at specific clinical settings in their geographic area.

A Randomized Trial of Liaison Psychiatry in Primary Care, NIMH #R01MH 41739-13, (PI: Katon); 

(09/1/00-8/31/06); $2,902,548 

This grant includes 2 major studies: 1) an epidemiologic study that screened approximately 7,000 adult patients 

with diabetes for depression in order to better understand the impact of depression on symptom burden, selfmanagement 

(diet, exercise, adherence to medication and glucose checks), functioning, medical costs and 

HbA 1C ; 2) an intervention trial that randomized approximately 330 patients to a collaborative intervention that 

provided enhanced education about depression via a videotape and book and 4 to 8 visits with a nurse who 

worked with the primary care physician to either provide antidepressant medication or problem-solving therapy.

Improving Mood-Promoting Access to Collaborative Treatment Program, John A. Hartford Foundation, #98301-G, 

(01/01/99-12/31/04); $1,193,027 

This project is a randomized controlled trial of a nurse intervention vs. usual primary care in the treatment of 

patients with major depression and dysthymia in patients age 60 and over. A major goal of the project is to 

compare the efficacy of an enhanced depression disease management intervention vs. usual care. 


Population-Based Psychiatric Interventions in Primary Care, NIMH #K02 MH01643-01 (12/98-11/03); $640,007 

This proposal has four aims: 1) to test the generalizability of the population-based disease management 

interventions that my research group has developed and tested in urban HMO patients in other geographic, 

socioeconomic, and clinical populations; 2) to continue to extend my research teams’ knowledge and expertise to 

provide cost-effective intervention for major depression to the 23 primary care clinics of Group health 

Cooperative; 3) to develop collaborative ties with a multi-site research network or large managed care system in 

order to demonstrate that improving the treatment of major depression in patients with chronic medical illness will 

improve: habituation to aversive medical symptoms, functioning, quality of life, and adherence to medical 

regimens; and 4) to continue my extensive local and national mentorship of junior investigators. Dr. Katon is 

responsible for the development, management, and analysis of above-mentioned research goals and for 

mentoring junior faculty and fellows.

Glycemic Control in Patients with Depression and Diabetes: Impact of Treatment for Depression

This project is a substudy from the IMPACT study and will analyze depressive outcomes and diabetes outcomes 

(self-care and HbA 1 C for the approximately 300 elderly diabetics that were randomized to a nurse intervention vs.

Depression and Chronic Tinnitus, (04/89-04/92); $427,716 total cost 

A double-blind, placebo-controlled study of the effect of nortriptyline. This grant was the culmination of our pilot 

study data demonstrating that 60% of patients with chronic tinnitus suffered from major depression. A second 

pilot study demonstrated relief of depression and tinnitus disability with an open trial of nortriptyline.

The Prevalence of Panic Disorder in Primary Care 

This was the first study of the prevalence of this severe anxiety disorder in a primary care clinic. 


A Predictive Cardiologic-Psychiatric Study of Chest Pain Patients Who go to Cardiac Angiography 

To define psychiatric and cardiologic variables associated with chest pain and normal cardiac angiograms. 

Almost 80% of patients with chest pain and negative coronary angiograms had panic disorder or major 

depression versus less than 10% of controls. 


Chronic Pelvic Pain: Relationship to Psychiatric Diagnoses and Childhood Sexual Abuse 

A comparison of 50 women with chronic pelvic pain to 50 controls with infertility or women who were scheduled to 

have a bilateral tubal ligation. All women were interviewed using the Diagnostic Interview Schedule and a sexual 

abuse interview and underwent laparoscopy. A significantly higher prevalence of depression, somatization, adult 

sexual dysfunction and childhood sexual abuse was found in the women with chronic pelvic pain when compared 

with controls. 

Irritable Bowel Syndrome (IBS): Relationship to Psychiatric Diagnoses, $1,700 NIMH funding for medical student 

interviewer 

A comparison of 30 patients with IBS to 30 controls with Ulcerative Colitis or Crohn's Disease using the 

Diagnostic Interview Schedule.

Chronic Fatigue Syndrome: A Collaborative Psychiatric, Virologic and Immunologic Study 

A study of 26 patients with chronic fatigue syndrome and comparison to 20 controls using a structured psychiatric 

interview (DIS) and intensive virological and immunological parameters. Patients with chronic fatigue had a 

significantly higher lifetime prevalence of major depression than controls, but few differences on virological or 

immunologic tests. 

Boeing Sudy 

A study of 37 Boeing workers who were involved in disability claims secondary to supposed exposure to organic 

solvents. Workers were collaboratively studied with occupational medicine exams, immunologic testing and 

allergy testing by other University of Washington faculty. On the Diagnostic Interview Schedule (DIS), 30 of the 

37 workers suffered from panic disorder or major depression but had few abnormalities on the other medical 

tests.
