Care Management of Mental and Physical Co-morbidities: A Triple Aim Bulls-Eye, DHHS award 1C1CMS331048<br />

Part of the $1 billion CMS Health Care Innovation Challenge, the Institute for Clinical Systems Improvement<br />

(ICSI) received funding to implement an evidence-based, multi-condition collaborative care management model<br />

for patients with depression plus diabetes and/or cardiovascular disease in primary care in seven states. The<br />

AIMS Center at UW will help design the program based on adaptations of the TEAMcare model and will train the<br />

implementing organizations in the model, provide technical assistance for implementation across all participating<br />

partners and participate in evaluation of the program.<br />

Pathway to War-Related PTSD Care and Recovery: Access, Quality and Interventions, Defense Advanced<br />

A multi-institutional consortium of research experts in medicine, epidemiology, survey design, health services<br />

research, clinical trials, social sciences, qualitative research, clinical decision support, health economics, health<br />

informatics, health care quality assessment, and biostatistics will complete the first major examination of the<br />

significant pathways to mental health services for military personnel returning from war and their families. This<br />

work will conduct a large multicenter randomized trial comparing optimized usual care to telephone case<br />

management using nurses collaborating with community providers and centrally supervised by mental health<br />

providers to improve continuity and quality of PTSD care.<br />

The overall goal of our research is to improve the detection of depression, quality of depression treatment,<br />

depressive symptoms, and functional outcomes of women with depression presenting to OB-GYN clinics. We<br />

propose a three-part intervention, integrated with OB-GYN care, consisting of: 1) enhanced education; 2)<br />

engagement; and 3) depression care management (with antidepressant monitoring and/or provision of brief<br />

psychotherapy. We hypothesize that a three-part depression intervention, integrated into the OB-GYN clinical<br />

setting, will improve treatment outcomes for depression, functional outcomes, and satisfaction with depression<br />

care, and will be a sustainable model of care.<br />

This project will test the Adolescent collaborative Care Treatment intervention, a model designed to improve<br />

major depression management among adolescents. The intervention will be tested in a population-based sample<br />

This proposed randomized, controlled trial will evaluate the effects of a culturally relevant, multicomponent<br />

intervention for antenatal depression (FOR MOMS, “Maintain Our Mothers’ Strength”) that has the potential to<br />

overcome patient, provider, and system-level barriers to care and engage depressed, low-income women in<br />

evidence-based treatments to reduce antenatal depressive symptoms, improve maternal psychosocial<br />

functioning, and ameliorate postpartum depression.<br />

The program is designed to produce primary care mental health clinical investigators by combining research<br />

projects and experience under the direction of a mentor with rigorous, research-based graduate level training in<br />

the School of Public Health’s Masters in Public Health Program. Two fellows will be admitted to the program each<br />

year, joining two second-year fellows for a total of four fellows per year. Two fellows will pursue adult mental<br />

health services research training and the other two fellows will train as child mental health services researchers.<br />

The program includes 5 components: 1) coursework toward MPH degree; 2) research project with supervision by<br />

a senior investigator at the UW or Group Health; 3) clinical rounds with the psychiatric consultation liaison service<br />

at University Hospital or Children's Hospital; 4) regular participation at teaching and research conferences; and 5)<br />

bimonthly meetings with either Dr. <strong>Katon</strong> or Dr. Richardson to evaluate ongoing course work, research and career<br />

This study aims to develop a culturally appropriate, sustainable, and cost-effective depression treatment program<br />

that can be incorporated into routine primary care practices serving low-income rural Latinos and that can be<br />

tested in a full scale randomized controlled trial.<br />

This grant will test a disease-focused depression treatment program through a randomized controlled trial in<br />

which 300 persons with poorly controlled diabetes or CHD (i.e. HbA 1c >8.5%, blood pressure >140/90, LDL >130)<br />

and major depression/dysthymia are assigned to either: 1) a chronic disease-focused depression treatment<br />

program that addresses management of both depression and comorbid disease, or 2) care as usual.<br />

Center activities will include the integration of phamacotherapy and behavioral interventions across the range of<br />

mood disorders, developing programs and systems to facilitate effective self-management, and use of information<br />

technology to promote self-management and effective patient-provider communication.<br />

This is a 5-year follow-up study of a cohort of 4800 patients with diabetes enrolled in Group Health that<br />

Dr. <strong>Katon</strong>'s research team has already enrolled. The aim of this grant is to study whether depression is<br />

associated with increased mortality and complications of diabetes and to better understand the effect of<br />

depression on quality of disease control (lipid, blood pressure and glycemic control) and on self-management of<br />

diabetes (diet, exercise, smoking and adherence to medication).<br />

population-based sample of youth with asthma compared to a random sample of control youth, and 2) to describe<br />

the impact of anxiety/depression comorbidity on the personal, familial and societal impacts of asthma such as<br />

medical symptom burden, functional status, quality of life, medical costs and on the quality of asthma<br />

self-management.<br />

The NRSA program trains physicians for academic research careers aimed at improving recognition and<br />

management of mental health problems encountered in primary medical care settings. The program recruits<br />

physicians in psychiatry and primary care for a minimum of a two-year fellowship. Fellows are mentored by<br />

senior health services researchers in the University of Washington Department of Psychiatry and Family Medicine<br />

and from the Center for Health Studies at Group Health Cooperative. The two-year training program has four<br />

components: 1) a Master of Public Health Degree program; 2) research experience in mental health programs in<br />

primary care settings; 3) experience with mentors who serve as role models, teachers, researchers and clinicians<br />

in the interface of medicine and psychiatry; and 4) clinical experience providing behavioral science consultations<br />

and teaching to residents in a primary care clinic.<br />

This grant includes 2 major studies: 1) an epidemiologic study that screened approximately 7,000 adult patients<br />

with diabetes for depression in order to better understand the impact of depression on symptom burden, selfmanagement<br />

(diet, exercise, adherence to medication and glucose checks), functioning, medical costs and<br />

HbA 1C ; 2) an intervention trial that randomized approximately 330 patients to a collaborative intervention that<br />

provided enhanced education about depression via a videotape and book and 4 to 8 visits with a nurse who<br />

worked with the primary care physician to either provide antidepressant medication or problem-solving therapy.<br />

Improving Mood-Promoting Access to <strong>Collaborative</strong> Treatment Program, John A. Hartford Foundation, #98301-G,<br />

This project is a randomized controlled trial of a nurse intervention vs. usual primary care in the treatment of<br />

patients with major depression and dysthymia in patients age 60 and over. A major goal of the project is to<br />

compare the efficacy of an enhanced depression disease management intervention vs. usual care.<br />

Population-Based Psychiatric Interventions in Primary Care, NIMH #K02 MH01643-01 (12/98-11/03); $640,007<br />

This proposal has four aims: 1) to test the generalizability of the population-based disease management<br />

interventions that my research group has developed and tested in urban HMO patients in other geographic,<br />

socioeconomic, and clinical populations; 2) to continue to extend my research teams’ knowledge and expertise to<br />

provide cost-effective intervention for major depression to the 23 primary care clinics of Group health<br />

Cooperative; 3) to develop collaborative ties with a multi-site research network or large managed care system in<br />

order to demonstrate that improving the treatment of major depression in patients with chronic medical illness will<br />

improve: habituation to aversive medical symptoms, functioning, quality of life, and adherence to medical<br />

regimens; and 4) to continue my extensive local and national mentorship of junior investigators. Dr. <strong>Katon</strong> is<br />

responsible for the development, management, and analysis of above-mentioned research goals and for<br />

mentoring junior faculty and fellows.<br />

This project is a substudy from the IMPACT study and will analyze depressive outcomes and diabetes outcomes<br />

A Predictive Cardiologic-Psychiatric Study of Chest Pain Patients Who go to Cardiac Angiography<br />

To define psychiatric and cardiologic variables associated with chest pain and normal cardiac angiograms.<br />

Almost 80% of patients with chest pain and negative coronary angiograms had panic disorder or major<br />

depression versus less than 10% of controls.<br />

Chronic Pelvic Pain: Relationship to Psychiatric Diagnoses and Childhood Sexual Abuse<br />

A comparison of 50 women with chronic pelvic pain to 50 controls with infertility or women who were scheduled to<br />

have a bilateral tubal ligation. All women were interviewed using the Diagnostic Interview Schedule and a sexual<br />

abuse interview and underwent laparoscopy. A significantly higher prevalence of depression, somatization, adult<br />

sexual dysfunction and childhood sexual abuse was found in the women with chronic pelvic pain when compared<br />

with controls.<br />

Irritable Bowel Syndrome (IBS): Relationship to Psychiatric Diagnoses, $1,700 NIMH funding for medical student<br />

A comparison of 30 patients with IBS to 30 controls with Ulcerative Colitis or Crohn's Disease using the<br />

Diagnostic Interview Schedule.<br />

Chronic Fatigue Syndrome: A <strong>Collaborative</strong> Psychiatric, Virologic and Immunologic Study<br />

A study of 26 patients with chronic fatigue syndrome and comparison to 20 controls using a structured psychiatric<br />

interview (DIS) and intensive virological and immunological parameters. Patients with chronic fatigue had a<br />

significantly higher lifetime prevalence of major depression than controls, but few differences on virological or<br />

immunologic tests.<br />

Boeing Sudy<br />

A study of 37 Boeing workers who were involved in disability claims secondary to supposed exposure to organic<br />

solvents. Workers were collaboratively studied with occupational medicine exams, immunologic testing and<br />

allergy testing by other University of Washington faculty. On the Diagnostic Interview Schedule (DIS), 30 of the<br />

37 workers suffered from panic disorder or major depression but had few abnormalities on the other medical<br />
