c) A sample and payment for a DNA test <strong>of</strong> £45.00 OR notification <strong>of</strong> where a DNA pr<strong>of</strong>ile is<br />
held.<br />
d) Membership Fee £50.<br />

i) £150 + VAT £176.25 is payable with this application<br />
Graded <strong>Stallion</strong>s will be issued with a Covering Book upon receipt <strong>of</strong> a signed declaration<br />
stating that the <strong>Stallion</strong> Owner agrees to abide by the <strong>Horse</strong> Race Betting Levy Board Codes<br />
<strong>of</strong> Practice for the prevention <strong>of</strong> venereal diseases<br />
Completed covering books must be returned to SHB(GB) by no later than 1st October each year.<br />
The Society reserves the right to refuse any entry.<br />
