JOB DESCRIPTION<br />

Reports To: Department Director<br />

Position Summary: Facilitates communication process between <strong>health</strong> professionals in relation to patient care<br />

<strong>and</strong> provides general clerical support to the unit. When so assigned, performs nursing care tasks under the<br />

supervision of the Regi<strong>st</strong>ered Nurse of Licensed Practical Nurse. Adheres to the performance <strong>st</strong><strong>and</strong>ards<br />

e<strong>st</strong>ablished for this position. This position supports the mission & philosophy of CHI <strong>and</strong> St. Joseph’s Hospital <strong>and</strong><br />

Health Center.<br />

Key Relationships: Directors, Charge Nurses, Admini<strong>st</strong>ration, ancillary Department Directors/Supervisors,<br />

medical <strong>and</strong> nursing <strong>st</strong>aff, patients <strong>and</strong> visitors.<br />

BASIC FUNCTIONS:<br />

1. Delivers personal care to patients in a caring <strong>and</strong> respectful manner as directed by the RN/LPN <strong>and</strong><br />

e<strong>st</strong>ablished plan of care:<br />

a. Delivers direct patient care as delegated, checks with RN/LPN for changes in care, explains to the<br />

patient what s/he is doing, seeks assi<strong>st</strong>ance when unable to answer patient’s que<strong>st</strong>ions, frequently<br />

check with patient to determine that s/he is comfortable, maintains privacy while doing procedures.<br />

b. Communicates <strong>and</strong> works cooperatively with RN/Chg/LPN by offering sugge<strong>st</strong>ions, reporting<br />

observations <strong>and</strong> patients’ response to procedures, <strong>and</strong> providing pertinent information for shift report.<br />

c. Documents accurately <strong>and</strong> timely (at time of occurrence) in the medical record, vital signs, I & O,<br />

weights, procedures (enemas, etc.), personal care (baths, skin care, etc.), ADLs, specimens as directed,<br />

observations, diet intake, <strong>and</strong> ambulation.<br />

d. Performs tasks for all age groups following specific in<strong>st</strong>ructions or RN/LPN.<br />

e. Seeks assi<strong>st</strong>ance for new/unfamiliar skill/task that is delegated.<br />

2. Maintains competency in clinical skills for service to which assigned:<br />

a. Demon<strong>st</strong>rates competence in the performance of selected skills <strong>and</strong> knowledge on an annual basis.<br />

b. Routinely performs selected tasks/skills <strong>and</strong> aspects of patient care (skills, ADL, treatments <strong>and</strong> use of<br />

equipment) in a timely <strong>and</strong> safe manner according to e<strong>st</strong>ablished <strong>st</strong><strong>and</strong>ards, policies <strong>and</strong> procedures.<br />

c. Seeks assi<strong>st</strong>ance for new or unfamiliar skills.<br />

3. Assumes routine coordination of clerical activities under the direction of the RN/Charge Nurse (omit this<br />

section if this role does not apply):<br />

a. Acts as a communication liaison between co-workers <strong>and</strong> other departments, reports te<strong>st</strong> results when<br />

obtained, notifies <strong>st</strong>aff on new/<strong>st</strong>at orders, reports information received from other departments/<strong>st</strong>aff,<br />

reports unit problems to charge <strong>nurse</strong>.<br />

b. Checks <strong>st</strong>ock supplies on a routine basis to assure consi<strong>st</strong>ent adequate supplies.<br />

c. Promptly locates charts <strong>and</strong> other materials when reque<strong>st</strong>ed.<br />

d. Completes clerical tasks accurately <strong>and</strong> timely, admission, discharge, transfer of patient.<br />

e. Transcribes orders accurately <strong>and</strong> according to policies.


QUALIFICATIONS:<br />

1. Ability to read <strong>and</strong> write English<br />

2. Nurse Aide Training<br />

• Swing Bed Unit – Certification upon employment<br />

• Home Health – Certification upon employment<br />

• Med/Surg/Peds – Certification upon employment<br />

• All other acute care areas – certification within 60 days of employment<br />

3. Regi<strong>st</strong>ration through ND Board of Nursing or State Board of Health<br />

4. Capable of meeting the physical <strong>and</strong> emotional dem<strong>and</strong>s of the position<br />
