ELIGIBILITY: Rural transportation services within <strong>St</strong>. <strong>Tammany</strong> <strong>Parish</strong>.<br />

FEES:<br />

At this time is free to promote ridership. Fares will be charged at a later date to be determined by <strong>St</strong>. <strong>Tammany</strong> <strong>Parish</strong> <strong>Government</strong>.<br />

SERVICES: <strong>St</strong>. <strong>Tammany</strong> <strong>Parish</strong> has contracted with the Council On Aging <strong>St</strong>. <strong>Tammany</strong> (COAST) to operate<br />

rural transportation services within <strong>St</strong>. <strong>Tammany</strong> <strong>Parish</strong>. Demand-Response Rural Transportation <strong>Service</strong>s are provided for any person in <strong>St</strong>.<br />

<strong>Tammany</strong> <strong>Parish</strong> as long as trips originate in designated rural areas of the <strong>Parish</strong> as determined by the U.S. Census bureau. Transportation<br />

services are provided to any location within the <strong>Parish</strong>, however no trips are provided to get children, grades 1-12 to and from school during<br />

school hours. Trips can be provided for after school activities.<br />

SLIDELL MEMORIAL HOSPITAL (SMH):<br />

AREA SERVED: Within the City of Slidell<br />

ELIGIBILITY: Rural transportation services within <strong>St</strong>. <strong>Tammany</strong> <strong>Parish</strong>.<br />

FEES:<br />

Free<br />

SERVICES: Do not take people to other hospitals besides SMH, but will take to doctor's offices. Provide 7,200 trips annually. Need based transportation<br />

for those who do not have the ability to physically drive or can afford to drive to hospital and doctor's offices. Transportation <strong>Service</strong>s are<br />

provided within a 10-mile radius of the SMH. SMH contracts services through Acadian Ambulance. <strong>Service</strong> priority: (1) Hospital facilities for X-<br />

rays, Lab, Out-Patient <strong>Service</strong>s; (2) Doctor's offices at SMH; (3) Any physician within the City of Slidell or 10-mile radius of SMH; (4) Any type<br />

of medical appointment within 10-mile radius of SMH. SMH has 1 handicapped accessible vehicle to operate that can hold 5 passengers<br />

including a wheelchair.<br />

NORTHSHORE MEDICAL CENTER- NORTHSHORE CAMPUS:<br />

ELIGIBILITY: Northshore Medical Center Clients.<br />

SERVICES: Clients must schedule an appointment for service either through Doctor's Office or Hospital and not directly through Transportation<br />

Department. <strong>Service</strong> is provided only to and from Northshore Medical Center Center and the two medical clinics located on the campus. 2<br />

handicapped accessible vans operate the service. Vehicle capacity is 2 passengers with 1 wheelchair.<br />

SUNRISE TRANSPORTATION:<br />

OFFICE HOURS: 8:00 to 5:00 PM, M-F. Some weekend service provided if schedule ahead<br />

AREA SERVED: <strong>St</strong>. <strong>Tammany</strong> <strong>Parish</strong><br />

ELIGIBILITY: Northshore Medical Center Clients.<br />

FEES:<br />

$1.50 per mile fare is charged.<br />

SERVICES: Transportation services are provided to anybody who calls, but services mainly cater to the elderly. <strong>Service</strong> provided for doctor's<br />

appointments, dialysis treatments, grocery shopping and prescription pick-ups. Not handicapped accessible. However, Sunrise personnel will<br />

help handicapped people in and out of vehicles as long as people have capability to lift and move themselves. 2 vehicles, (1) 6-passenger van<br />

and (1) car.
