Sunscreen/Insect Repellent<br />

All campers should apply sunscreen in the morning prior to arriving at camp. Staff will assist<br />

campers with applying their own sunscreen after morning and afternoon swim periods. In order to<br />

comply with child abuse prevention guidelines, the application <strong>of</strong> sunscreen will take place in an<br />

open setting. We recommend that each camper keep their own bottle <strong>of</strong> sunscreen lotion in their<br />

backpack labeled with their first and last name. Because campers spend a large part <strong>of</strong> their day<br />

outside, we recommend camper's use the following precautions:<br />

• Wear a hat to protect them from the sun.<br />

• Wear approved swim shirts while participating in swim periods.<br />

• Have their own personal bottle <strong>of</strong> sunscreen available<br />

MEDICAL AND HEALTH INFORMATION<br />

The <strong>2013</strong> <strong>Day</strong> <strong>Camp</strong> Health Form (2 pages) is to be submitted the first day <strong>of</strong> camp.<br />

Medications<br />

It is preferable that prescription medications be administered at home when possible. <strong>Camp</strong>ers who<br />

require medications will need to submit additional information and meet the camp nurse.<br />

All medication must be in its original container and clearly labeled with campers first and last<br />

name. WE CANNOT ACCEPT ANY MEDICATIONS THAT ARE NOT IN THEIR ORIGINAL CONTAINER.<br />

Immunization History<br />

New York State Department <strong>of</strong> Health requires each camp to keep a current copy <strong>of</strong> every camper’s<br />

immunization history on file in order for them to attend. A complete copy <strong>of</strong> your child’s<br />

immunization history should be submitted with the Health Form. CAMPERS WITHOUT AN<br />

IMMUNIZATION HISTORY ON FILE WILL NOT BE PERMITTED TO ATTEND.<br />

Emergencies<br />

Our staff is trained in handling a variety <strong>of</strong> emergency situations. In the event <strong>of</strong> an emergency, we<br />

will make every attempt to notify parents immediately. If necessary, emergency transportation will<br />

be handled by local emergency services.<br />

Daily Snack<br />

The Y is committed to healthy living and modeling healthy eating. A healthy afternoon nut-aware<br />

snack will be provided for all campers.<br />

PAYMENT OPTIONS AND PROCEDURES<br />

Payments are due in full by June 1, <strong>2013</strong>.<br />

Easy Payment Option<br />

The Easy Payment Option (EFT) is the preferred billing method. Simply provide a credit or debit card<br />

and tuition will be automatically paid June 1, <strong>2013</strong>.<br />

Changes in Registration<br />

<strong>Parent</strong>s are responsible for informing the camp <strong>of</strong>fice in writing <strong>of</strong> all changes to camper<br />

registration and updating contact details including address and phone numbers.
