2011-12 Make-A-Wish ® Campaign<br />
Fundraising Checklist<br />
1. Select a project from the list of pre-approved <strong>fundraising</strong> ideas. If your<br />
team seeks to do a project not listed, please email Karen Halterman with<br />
NCA/NDA at khalterman@nationalspirit.com or Tracy Jourdan, with the<br />
Make-A-Wish Foundation ® at tjourdan@wish.org with a clear description of<br />
the proposed fundraiser for a quick response.<br />
2. Conduct <strong>fundraising</strong> project campaign.<br />
3. Collect funds. Convert all cash to a school check or money order made<br />
payable to the Make-A-Wish Foundation®.<br />
4. Submit all checks or money orders with completed Fundraising and<br />
Volunteer Tracking Form by April 30, 2012 to Karen Halterman at the<br />
NCA/NDA headquarters, 2010 Merritt Drive, Garland, TX 75041 (f) (972)<br />
840-4010.<br />
*For Project NCA/NDA, submit <strong>form</strong> and funds by November 3 rd , 2011.<br />
5. Send photos of your team having fun while raising money for the<br />
Make-A-Wish Foundation to Karen at KHalterman@nationalspirit.com!<br />
6. Select another project and repeat the process again! You can fundraise year<br />
round!<br />
