RULES AND REGULATIONS CONTRACT 2013 I have read the Fantasy Cheerleading Rules and Regulations Contract in its entirety and understand its contents. I realize that when representing Fantasy Cheerleading I must always conduct myself with class and responsibility. I understand that any athlete or parent that does not abide by the rules and regulations contained in this contract, that is consistently negative, or acts in a manner that jeopardizes the name and reputation of the Fantasy Cheerleading programme, will be subject to removal with no refund. Medical/Accident I understand as parent/guardian of the participant above, hereby give permission for her to attend and participate in any Fantasy Cheerleading NZ lesson, event or performances during the year 2013. I understand by attending and participating there are possibilities of injury and physical illness to her which I waive and release and forever discharge any and all rights and claims for damages, which may arise now or in the near future against the director/s, staff, owner or other associated representatives of Fantasy Cheerleading NZ. Furthermore I authorize the director/s of Fantasy cheerleading NZ to act for me and on behalf of me in any emergency requiring medical attention. Advertisement/Photography/Film I hereby give permission for my daughter to be used in advertisement material for Fantasy Cheerleading NZ or to promote cheerleading in general, this includes the rights to photograph and film my child during lessons, events or performances and to publish in media such as web sites, flyers, posters, news papers and videos but not limited. Fee’s 2013 Teams: Recreational Cheerleading – Open age – train 1 ¼ hours per week - $150 per term. Competitive teams: Mini Level 1b Fantasy Rockers – age 4-8 years – train 1 hour per week $110 per term – add additional 1 hour tumble class, total 2 hours training per week $200 per term. Youth Level 1 Fantasy Beatbox – age 6-11 years – train 1 ½ hours per week, 1 hour cheer ½ hour tumble per week $150 per term – add additional 1 hour tumble class, total 2 ½ hours training per week $240 per term. Elite Level 1 Fantasy MegaMix – age 6-14 years – train 3 hours per week, 2 hours cheer, 1 hour tumble per week $260 per term – add additional 1 hour tumble class, total of 4 hours training per week $300 per term. Elite Level 2/3 Fantasy Diva’s – age 8-14 years – train 3 hours per week 2 hours cheer, 1 hour tumble per week $260 per term – add additional 1 hour tumble class, total of 4 hours training per week $300 per term. Elite Level 2/3 Fantasy Dubstep – age 12 years and up – train 3 hours per week, 2 hours cheer, 1 hour tumble per week $260 per term – add additional 1 hour tumble class, total of 4 hours training per week $300 per term. Elite Level 3/4 Fantasy Vogue – open age – train 4 hours per week 3 hours cheer, 1 hour tumble per week $300 – add additional 1 hour tumble class, total of 5 hours training per week $330 per term. Elite Level 4/5 Fantasy Remix – open age – train 4 ½ hours per week, 3 hours cheer, 1 ½ hour tumble per week $320 per term – add additional 1 hour tumble class, total of 5 ½ hours training per week $340 per term.
