Completed Research Support within the last 3 years: Principal Investigator “Multilevel Model of Breast Cancer Follow-up Care” NIH R21 6/1/04 5/31/07 (one year no-cost extension) $90,000/year The goal of the proposed project is to analyze existing data of 2,800 low-income Missouri women with abnormal breast cancer screening results and to collect primary data of nearly 300 low-income Missouri women in order to test four different pathways by which low-income women from areas with higher deprivation are less likely to receive adequate diagnostic follow-up in a future R01 project. Principal Investigator “Geography of Amputations Among African Americans” NIH R21 9/30/04 8/31/07 (one year no-cost extension) $100,000/year The purpose of the proposed exploratory study is to: 1) characterize the geographic variation of the incidence of diabetes-related lower extremity amputations (LEA) at the county level among African Americans age 65-99 in 5 southern states, and 2) explore the association of selected determinants with the geographic variation in diabetes-related incidence of LEA among African Americans age 65-99 in 5 southern states. Epidemiologist “Feeling F.I.N.E.” CDC R06 9/30/02 9/29/06 The major goals of this project are to increase knowledge and skills, increase social support, increase opportunities for physical activity and healthy eating options at community events, and change the community infrastructures to make it easier to make healthy choices. Principal Investigator “Modeling Community Disparity in Breast Cancer Screening” AHRQ R03 5/1/04 4/30/06 This project will construct a conceptual model by testing the different pathways (medical infrastructure, general community infrastructure, social capital) by which women age 40 or older from metropolitan areas with adverse material and social conditions are less likely to be screened for breast cancer after controlling for individuallevel barriers. We will conduct analyses of existing National Health Interview Survey data of more than 10,000 women from 58 metropolitan areas throughout the United States merged with community data from various existing sources. Epidemiologist “Physical Frailty in Urban African Americans” NIA 9/30/99 4/30/06 The major goals of this project are to identify (1) the effects of frailty factors on falls, excess hospitalizations, progressive disability, and death in a cohort of inner-city African Americans aged 70 to 99 at baseline and (2) the timing of development, precursors, and consequences of sarcopenia and subclinical disability on the function and health services utilization of a cohort of urban African Americans aged 50 to 64 at baseline. Principal Investigator “GIS and Breast Cancer Screening in Saint Louis” NCI R21 8/1/03 1/31/06 The major goals of the project are to determine areas where breast cancer screening is underutilized in St. Louis, if women living in neighborhoods characterized by poorer socioeconomic conditions are less likely to be screened for breast cancer, and if this association depends on the geographical classification used to obtain these community indicators. Principal Investigator “Radiation Therapy in Older Breast Cancer Patients” NIH/NCI R21 9/30/01 8/31/05 This exploratory population-based study will identify reasons why rural women 65 years of age and older in Iowa are less likely than their urban counterparts to receive radiation therapy following conservative surgery for stage I-II breast cancer. Principal Investigator “GIS, Multilevel Modeling and Breast Cancer Screening” NIH R03 6/1/01 5/31/04 The major goals of this study are to describe rural-urban differences in breast cancer screening at the level of the Census tract, to determine if ecological factors are independently associated with breast cancer screening after controlling for individuallevel factors and their interactions, and to determine if rural-urban differences in screening prevalence exist while controlling for important individual and ecological factors and their interactions associated with breast cancer screening. b.
