Chapter 3: HIV/AIDS <strong>and</strong> <str<strong>on</strong>g>Drug</str<strong>on</strong>g>s<br />

Globally, sexual transmissi<strong>on</strong> of HIV c<strong>on</strong>tinues to be the most comm<strong>on</strong> way the virus is spread, but drug use is c<strong>on</strong>tributing<br />

to the p<strong>and</strong>emic in at least four ways. First, the most comm<strong>on</strong> <strong>and</strong> best-researched method of transmissi<strong>on</strong><br />

is via the use of c<strong>on</strong>taminated injecti<strong>on</strong> equipment between people who inject drugs. Sec<strong>on</strong>d, there is sexual<br />

transmissi<strong>on</strong> of the virus between those who inject drugs <strong>and</strong> their sexual partners. The dual transmissi<strong>on</strong> risk in<br />

the case of sex workers who also inject drugs leads to epidemics that exp<strong>and</strong> quickly <strong>and</strong> act as a bridge to the rest<br />

of the populati<strong>on</strong>. Third, n<strong>on</strong>-injecting use of drugs such as cocaine <strong>and</strong> amphetamine-type stimulants leads to highrisk<br />

sexual behaviour. And finally, HIV can be transmitted from an infected mother to her child.<br />

In the early stages of the p<strong>and</strong>emic, HIV/AIDS am<strong>on</strong>g injecting drug users was largely viewed as self-limiting, affecting<br />

injectors <strong>and</strong> their immediate sexual partners but not leading to a more generalised spread of the virus. Recent<br />

work <strong>on</strong> the Asian <strong>and</strong> Eastern European HIV/AIDS epidemics has proven this perspective to be incorrect. 3 Globally,<br />

it is estimated that 5% -10% of all HIV infecti<strong>on</strong>s are attributable to injecting drug use, mostly via the use of<br />

c<strong>on</strong>taminated injecti<strong>on</strong> equipment. 4 In many countries of Europe, Asia, the Middle East <strong>and</strong> the Southern C<strong>on</strong>e of<br />

Latin America, the use of n<strong>on</strong>-sterile injecti<strong>on</strong> equipment has remained the most important mode of HIV transmissi<strong>on</strong>,<br />

accounting for 30%-80% of all reported infecti<strong>on</strong>s.<br />
