A checklist may be used to account for each component of item 8 (a) to (h) and their<br />
respective sub- elements. The checklist can provide for a “YES” and “NO” response in<br />
terms of the compliance of the existing system to the SMS requirements. Remarks for<br />
partial compliance or deviations should be made as well as actions required in order to<br />
meet the criteria. There should be a column for annotating existing company<br />
documentation where the requirement is addressed.<br />
Once the gap analysis is complete and fully documented, the items you have identified<br />
as missing or deficient will form the basis of your SMS project plan. Organizations may<br />
format their project plan to suit their individual needs.<br />
13. DEFINITIONS<br />
ALARP: As low as reasonably practicable<br />
Hazard: Is a condition, object or activity with the potential of causing injuries to<br />
personnel, damage to equipment or structures, loss of material, or reduction<br />
of ability to perform a prescribed function.<br />
Mitigation: Measures to eliminate the potential hazard or to reduce the risk probability<br />
or severity.<br />
Probability: The likelihood that a situation of danger might occur.<br />
Risk: Is the chance of a loss or injury, measured in terms of severity and probability.<br />
The chance that an event can happen and the consequences when it does.<br />
Risk Index: Combined value of risk probability and severity.<br />
Safety: Is the state in which the risk of harm to persons or property damage is reduced<br />
to, and maintained at or below, an acceptable level through a continuing process of<br />
hazard identification and risk management.<br />
Severity: The possible consequences of a situation of danger, taking as<br />
worst foreseeable situation.<br />

SMS: A systematic, explicit and proactive process for managing safety that integrates<br />
operations and technical systems with financial and human resource management to<br />
