POSITION DESCRIPTION: Mental <strong>Health</strong> Coordinator<br />
POSITION PURPOSE<br />
<strong>Nelson</strong> <strong>Bays</strong> <strong>Primary</strong> <strong>Health</strong> (NBPH) leads and coordinates a range of primary health care services through<br />
its contracted General Practices and other <strong>Primary</strong> <strong>Health</strong> providers e.g. Māori Providers and NGO’s, in the<br />
delivery of the <strong>Primary</strong> <strong>Health</strong> Care Strategy. The organisation holds contracts with the <strong>Nelson</strong> Marlborough<br />
District <strong>Health</strong> Board (NMDHB) and other providers for these services. <strong>Nelson</strong> <strong>Bays</strong> <strong>Primary</strong> <strong>Health</strong> is<br />
committed to reducing health inequalities within the health system.<br />
This position will be responsible for the line management of all mental health and aligned roles within NBPH<br />
and be required to manage a caseload, within the Brief Intervention Service, for clients aged 16 years and<br />
over, presenting with mild to moderate mental health issues. This role will build on the experiences of the<br />
existing primary mental health and aligned programmes within NBPH.<br />
POSITION OBJECTIVES<br />
The objectives of this position are:<br />
• Line management of appropriate and designated mental health and aligned roles and contracts within<br />
NBPH.<br />
• Support change management and facilitate service development in relation to new and existing<br />
programmes<br />
• Caseload management for clients aged 16 years and over presenting with mild to moderate mental<br />
health issues within the Brief Intervention Service.<br />
• Active partnership with NMDHB Mental <strong>Health</strong> Directorate in the development and delivery of a primary<br />
mental health service in our region and take an active part in the development of the stepped care model<br />
for mental health.<br />
• Work with NMDHB to scope and develop a single point of entry for clients to access mental health<br />
services within the <strong>Nelson</strong> <strong>Bays</strong> region.<br />
• Ensure the provision of a comprehensive and coordinated service for individuals that present with mild to<br />
moderate mental illness and other social care related needs which can be managed in primary care.<br />
• Support general practice teams, Māori health providers and other relevant stakeholders to ensure<br />
clients’ needs are met and outcomes are achieved.<br />
• Reduce barriers and improve access to a range of services using flexible packages of care that will wrap<br />
around the individual client’s needs in a culturally appropriate manner.<br />
• Promote an integrated model of care at a primary level.<br />
• Utilise accepted best practice assessment tools and interventions.<br />
• Establish and maintain a database of current and existing services available to support the client with a<br />
package of care. Memorandum of Agreements with packages of care providers will be completed.<br />
• Ensure comprehensive workforce and systems development.<br />
• Contribute to other activity as directed and delegated by the CEO of NBPH.<br />

LOCATION<br />
This full-time position is available a person (1.0 FTE), located at the office of <strong>Nelson</strong> <strong>Bays</strong> <strong>Primary</strong> <strong>Health</strong>.<br />

QUALIFICATIONS/EXPERIENCE<br />
The NBPH Mental <strong>Health</strong> Coordinator will:<br />
• Have demonstrated leadership skills<br />
• Have line management experience<br />
• Have experience in managing client triaging processes for clinical referrals<br />
• Knowledge of Youth Development Principles<br />
• Be a registered clinician under the <strong>Health</strong> Practitioner Competency Assurance Act (2003)<br />
• Have experience in the primary mental health care field<br />
• Hold a full and current driver’s license<br />
