SEXUAL INTERCOURSE PAIN<br />

The pain experienced by a woman during sexual intercourse is called dyspareunia by doc<strong>to</strong>rs. It may occur<br />

superficially near the outside as the penis initially enters the vagina, during deep penetration of the penis, or may be a<br />

mixture of both.<br />

Infections of the vagina c<strong>au</strong>sed by a fungus (eg. thrush) or bacteria may inflame both the vagina and the vulval<br />

tissue around the outside. These infections are usually ac<strong>com</strong>panied by a discharge. Irritation of the inflamed tissue<br />

during sex will c<strong>au</strong>se pain.<br />

Deeper infections in the lower part of the belly involving the bladder (cystitis), urethra (tube that drains urine from<br />

the bladder), bowel (eg. diverticulitis), lining of the belly (peri<strong>to</strong>nitis) or an abscess in the pelvis will be aggravated by<br />

intercourse.<br />

A lack of the female hormone oestrogen after menop<strong>au</strong>se may lead <strong>to</strong> a dryness of the vagina, which makes sex<br />

difficult and painful. The natural lubrication of the vagina is maintained by glands that are stimulated by oestrogen.<br />

Some women may notice that their vagina is drier at some stages of their menstrual cycle than at others due <strong>to</strong><br />

variations in the level of oestrogen. Underactive thyroid, pituitary or adrenal glands, which affect the functioning of all<br />

cells in the body, may also c<strong>au</strong>se vaginal dryness.<br />

A prolapse (dropping) of the uterus, usually as a result of childbirth, age or obesity, may c<strong>au</strong>se a backache that is<br />

aggravated by sex.<br />

C<strong>au</strong>ses of superficial pain during sex include vulvodynia, Bartholin’s gland infection (glands near the opening of the<br />

vagina that produce its natural lubricating fluid), genital herpes infection (painful ulcers around the vulva and the lips of<br />

the vagina), an episio<strong>to</strong>my scar (cut in the back wall of the vagina performed <strong>to</strong> allow the baby’s head <strong>to</strong> emerge more<br />

easily during birth), vaginismus (strong spasm of the muscles in the vagina that prevents the penis from entering), a<br />

allergy <strong>to</strong> soaps, perfumes, detergents in underwear or other substances may c<strong>au</strong>se a swelling and inflammation of the<br />

delicate tissues of the vulva.<br />

C<strong>au</strong>ses of deep pain during sex include cervicitis, salpingitis and pelvic inflamma<strong>to</strong>ry disease (infections of the<br />

fallopian tubes and other organs in the pelvis), endometriosis, fibroids (hard balls of fibrous tissue that form in the<br />

muscular wall of the uterus), an ec<strong>to</strong>pic pregnancy (a foetus in the Fallopian tubes instead of the uterus) and tumours<br />

or cancer of the cervix, uterus or ovaries.<br />

STILBOESTROL<br />

The synthetic sex hormone stilboestrol (diethylstilboestrol, or in the USA stilbestrol) may be used as a vaginal<br />

pessary for female hormone replacement in menop<strong>au</strong>se and vaginal dryness after the menop<strong>au</strong>se. One or two<br />

pessaries are inserted high in<strong>to</strong> the vagina at night.<br />

It is not <strong>to</strong> be used in pregnancy, breastfeeding or children but accidental usage in these situations unlikely <strong>to</strong> be<br />

harmful. Use stilboestrol with c<strong>au</strong>tion in epilepsy, migraine, heart failure, high blood pressure, kidney disease, diabetes,<br />

porphyria or uterine disease. Do not take it if suffering from liver disease, breast or genital cancer, or blood clots.<br />

Side effects may include abnormal uterine bleeding, vaginal thrush, fluid retention, weight gain and breast<br />

tenderness. Un<strong>com</strong>monly a rash, bloating and intestinal cramps may occur. Severe but rare side effects include blood<br />

clots, calf or chest pain and yellow skin (j<strong>au</strong>ndice). Smoking increases the risk of serious side effects. Interactions<br />

occur with other sex hormones.<br />

It is now considered <strong>to</strong> be a rather old-fashioned form of hormone replacement therapy as other better suited<br />

hormones are now available, and its use is steadily declining.<br />

SWEATING EXCESS<br />

Increased sweating (hyperhidrosis or diaphoresis) is obviously associated with hot conditions, but may also occur<br />

with anxiety and fear, eating hot (spicy hot or heat hot) foods, significant pain from any c<strong>au</strong>se, and drinking excess<br />

alcohol. There are a number of medical conditions, which may also be responsible.<br />

Any bacterial or viral infection that c<strong>au</strong>ses a fever will have excessive sweating associated with it. Some infections,<br />

such as malaria, AIDS and tuberculosis, may result in sweating out of proportion <strong>to</strong> the level of fever.<br />

The menop<strong>au</strong>se occurs in the late forties and early fifties in most women. The symp<strong>to</strong>ms of menop<strong>au</strong>se include<br />

irregular menstrual periods, hot flushes, sweats, headaches, irritability, personality changes, breast tenderness,<br />

tiredness and pelvic dis<strong>com</strong>fort.<br />

The thyroid gland in the front of the neck produces the hormone thyroxine, which acts as an accelera<strong>to</strong>r for every<br />

cell in the body. Excess thyroxine will c<strong>au</strong>se sweating, weight loss, diarrhoea, poor absorption of food, nervousness,<br />
