ALZHEIMER DISEASE<br />

Alzheimer disease (or senile dementia) use <strong>to</strong> be called second childhood, or the person was described as<br />

eccentric. Today it is recognised as the most <strong>com</strong>mon form of dementia in the elderly, but it may start as early as the<br />

mid-fifties.<br />

It is named after the Wroclaw (Poland) neurologist Alois Alzheimer, who was born in 1864, and first described the<br />

disease in the medical literature.<br />

The c<strong>au</strong>se is a faster than normal loss of nerve cells in the brain, the exact c<strong>au</strong>se of which is unknown, but studies<br />

suggest specific genes may predispose a person <strong>to</strong> the disease, and there is a familial tendency (runs in families from<br />

one generation <strong>to</strong> the next).<br />

Initially it c<strong>au</strong>ses loss of recent memory, loss of initiative, reduced physical activity, confusion and loss of orientation<br />

(confused about place and time), then progresses <strong>to</strong> loss of speech, difficulty in swallowing which c<strong>au</strong>ses drooling, stiff<br />

muscles, incontinence of both faeces and urine, a bedridden state and eventually the patient is <strong>to</strong>tally unaware of<br />

themselves or anything that is happening around them. Some patients may not deteriorate for some time, then drop <strong>to</strong><br />

a lower level of activity quite suddenly. Admission <strong>to</strong> a nursing home or hospital is eventually necessary.<br />

Reduced brain volume and wasting may show on a CT scan, but the diagnosis is primarily a clinical one made by a<br />

doc<strong>to</strong>r after excluding all other forms of dementia by blood tests, X-rays, electroencephalogram (EEG) and sometimes<br />

taking a sample of the spinal fluid. The progress of the disease can be followed by tests of skill, general knowledge,<br />

simple maths, etc.<br />

Medication is useful for restlessness and insomnia, and a number of medications (eg. donepezil, rivastigmine,<br />

tacrine, mematine) are now being used <strong>to</strong> slow the progression of the disease. These medications should only be<br />

prescribed by, or in consultation with, clinicians, who are experienced in the diagnosis and management of Alzheimer<br />

disease. In women, hormone replacement therapy after menop<strong>au</strong>se reduces the incidence of Alzheimer disease, and<br />

slows its progress. Visits by the family general practitioner, physiotherapists, occupational therapists, home nursing<br />

care and health visi<strong>to</strong>rs are the main forms of management. Many claims have been made for various herbal<br />

remedies, but none have proved <strong>to</strong> be beneficial.<br />

There is no cure, and treatments are aimed at keeping the patient content. Medications in the anticholinesterase<br />

class (eg. donepezil) have been shown <strong>to</strong> slow the progression of the disease in some patients. Lipoic acid is used by<br />

some practitioners. From diagnosis <strong>to</strong> eventually death takes seven years on average.<br />

AMENORRHOEA<br />

Amenorrhoea is a cessation of menstrual periods for any reason from pregnancy <strong>to</strong> stress and menop<strong>au</strong>se.<br />

ANDROPAUSE<br />

The male menop<strong>au</strong>se (androp<strong>au</strong>se) is a natural event that occurs in all men. After the androp<strong>au</strong>se no male<br />

hormones are manufactured in the testes, the testes no longer produce sperm, and the man is infertile.<br />

The male sex hormone (tes<strong>to</strong>sterone) is released from the testes in<strong>to</strong> the blood in response <strong>to</strong> signals from the<br />

pituitary gland, which sits underneath the centre of the brain. These hormones effect every part of the body, but more<br />

particularly the penis, scrotum and body hair production. For an unknown reason, once a man reaches an age<br />

somewhere between the late sixties and late seventies, the pituitary gland s<strong>to</strong>ps sending messages <strong>to</strong> the testes,<br />

which results in the symp<strong>to</strong>ms of the androp<strong>au</strong>se.<br />

The man experiences the gradual onset of a loss of interest in sex (low libido), difficulty in maintaining or achieving<br />

an erection of the penis, a lack of ejaculation during sex, thinning of body and pubic hair, and shrinking of the testicles.<br />

Osteoporosis may occur, particularly if there is a family his<strong>to</strong>ry, or the androp<strong>au</strong>se occurs at an early age. These<br />

symp<strong>to</strong>ms are far more subtle, and far less distressing than those that occur in the female menop<strong>au</strong>se.<br />

Blood tests can determine the levels of tes<strong>to</strong>sterone and the stimulating hormone released by the pituitary gland.<br />

No treatment is normally necessary as it is a normal part of the ageing process, but if the androp<strong>au</strong>se occurs earlier<br />

than normal, or following an injury or surgery <strong>to</strong> the testes or pituitary gland, tes<strong>to</strong>sterone supplements may be given by<br />

tablet, injection or implant. .<br />

ANDROSTENEDIONE<br />

Androstenedione is a weak androgenic hormone secreted by the testes, ovaries and adrenal glands. The amount<br />

present in the blood may be determined in the investigation of female hirsutism (facial hair and other male<br />
