Those wishing to attend must return the Registration Form duly completed and accompanied<br />
by receipt of payment. Because of limited seating, registrations will be accepted on a firstcome<br />

the organizing secretariat will promptly contact those participants exceeding the threshold<br />

CERTIFICATE OF ATTENDANCE<br />
A certificate of attendance will be issued for the number of days a registrant has actually attended<br />
the congress. These certificates will be available at the registration desk and must be picked<br />
up before departure.<br />
EACCME ACCREDITATION<br />
The congress is accredited by the European Accreditation for Continuing Medical Education<br />
(EACCME) to provide the following CME activity for medical specialists. The EACCME is an<br />
institution of the European Union of Medical Specialists (UEMS), www.uems.net.<br />
The congress is designated for a maximum of 12 hours of European external CME credits.<br />
Certification of these credit units will be issued along with the certificate of attendance. It is<br />
required to fill out the evaluation form and to deliver it to the registration desk in order to get the<br />
credits. Delegates are advised to contact their own National Accreditation Council for<br />
transcription of these credits. Italian delegates can also obtain ECM points, Agenas has<br />
accredited 16 ECM point for the whole Congress.<br />

CANCELLATION<br />
Cancellations will be accepted by the organizing secretariat strictly within 15 May 2012 entitling<br />
the registrant to a 80% reimbursement of the fee paid. Although no reimbursements will be<br />
processed further to the cancellation deadline, registration may be turned over to another<br />
attendee, provided the name is communicated in advance to the organizing secretariat.<br />
