We look forward to holding our first annual Jog-A-Thon, and hope students will enjoy the opportunity to test themselves physically while raising
money for Classroom Funds and the SDCCS PSA General Fund. Students are invited to solicit pledges from parents, friends, family, and
neighbors - but please, don't approach strangers! Please gather as many pledges as you can prior to Nov. 9th. At the Jog-A-Thon, students will
be given a record of how many laps they completed, then they can start collecting the pledges. Please turn in this envelope to your
child's teacher, with all pledge payments enclosed by November 30th. Good luck jogging on November 9th!

All Pledges are Tax-Deductible, we are a non-profit organization under FEIN 33-0931009
Checks should be made payable to "SDCCS-PSA" with "Jog-A-Thon" and child's class in the memo.
All children are encouraged to participate in the Jog-A-Thon, even if they receive no pledges. IF YOU DO NOT WANT
YOUR CHILD TO RUN, JOG, OR WALK IN THE JOG-A-THON, PLEASE SIGN HERE:________________________
