1 Purpose and Scope<br />

This document is designed to allow each person in the organis<strong>at</strong>ion to understand wh<strong>at</strong> they are<br />

responsible for when planning, designing, constructing, oper<strong>at</strong>ing, maintaining or disposing of the<br />

processes covered by the standard.<br />

The assessment covers all requirements of the standard for Senior Managers, Managers, Team Leaders<br />

and Team members in the Oper<strong>at</strong>ions, Maintenance and Support functions.<br />

By using this questionnaire, each person should be able to judge whether they have adequ<strong>at</strong>e skills, tools<br />

or written procedures to safely carry out their responsibilities for <strong>Safe</strong> <strong>Working</strong> <strong>at</strong> <strong>Height</strong>. The<br />

questionnaire is compiled from the Major Hazard Standard and the audit protocol.<br />

3 Roles and Functions<br />

The questionnaire is divided into different roles and functions. The aim has been to develop questions<br />

th<strong>at</strong> explore how you are complying with the requirements of the standard while taking into account the<br />

different responsibilities and activities th<strong>at</strong> are present on each site.<br />

Each site will need to define where their management structure fits with the generic roles as listed below.<br />

When this has been achieved, all personnel on site will understand which section of the questionnaire<br />

applies to them.<br />

2 How to use the <strong>Assessment</strong><br />

The questionnaire is divided into separ<strong>at</strong>e sections.<br />

• Next: Read through each question answering with a Yes, No or N/A (Not Applicable);<br />

Functions are defined as follows;<br />
