Schedule an advising appointment by choosing the appropriate advising option below: Academic Advising Students who are required to enroll in Academic Foundation (AF) courses based on COMPASS test scores are seen by a pre-admit Advisor in the Advising and Counseling Center, Room 168 Main Building. To schedule an appointment with a pre-admit advisor, students should call the Advising and Counseling Center at (513)-569-1552. Program Advising Students who have completed all (or were not required to complete) AF courses are seen by a program advisor in the Health and Public Safety Division office, Room 312 Health Professions Building. To schedule an appointment with a program advisor, students should call the Health and Public Safety Division office at (513)-569-1670. Mandatory Requirements to apply for Practical Nursing Progression Complete PNC Progression Application - have PNC declared as major to apply Attend mandatory PNC Information Sessions Cumulative GPA of 2.0 or above- must maintain GPA throughout application process Math & Science GPA of 2.0 or above-must maintain GPA throughout application process Take the WorkKeys test- score of 4 or greater need for each section State Tested Nurse Aide - STNA If you are not a STNA, you may take MCH 130 at Cincinnati State or the STNA course offered through Great Oaks, www.greatoaks.com , to prepare for the state test. The MCH 130 course offered at Cincinnati State requires the student to complete a Nurse Aide Training application located in Health Professions Building, Room 312. Submit the application to the front desk in the Health and Public Safety Division – Health Professions Building, Room 312 in order to enroll in Nurse Aide Training. Consider taking the Statement of Health form (see below) to your healthcare provider when completing the Nurse Aide training application. Take the two-part state Nurse Aide test and pass both portions (written and skills.) Verify that you are in GOOD STANDING on a state Nurse Aide registry. Obtain a print-out from your state Nurse Aide registry website showing that you are in GOOD STANDING. For Ohio, please refer to: https://odhgateway.odh.ohio.gov/nar/nar_registry_search.aspx. If you are already an STNA working outside of long-term healthcare and are no longer current on a Nurse Aide state registry, please refer to http://www.odh.ohio.gov/odhPrograms/io/nurseaide/naRequirements.aspx. If applicable, provide documentation of health care work experience on company letterhead. During the open application period, submit the letter to the Health and Public Safety Division attention Tammy Smith or e-mail copy of the letter to pncprogression@cincinnatistate.edu letter must be received by application deadline date. If applicable, during the open application period provide documentation of military service to Health and Public Safety attention Tammy Smith or e-mail a copy to pncprogression@cincinnatistate.edu by the application deadline date. PNC Progression Application Process 1. Complete an eform in order to apply for PNC Progression. The e-form application is only available the open application period. There will be an application period for each subsequent semester. Once applications are reviewed, applicants are moved forward in the progression process based upon the applicant’s PNC Progression worksheet scores. 2. Progressing applicants will be notified of these additional requirements and the deadline for submission: a) CPR Certification Student must submit a signed copy of a current Healthcare Provider or Professional Rescuer certification (for adults, infants, children and AED). Online certification with hands on demonstration is accepted. Options for CPR Training: MCH 130 is offered at Cincinnati State; CPR for the Healthcare Provider at Great Oaks CPR for the Healthcare Provider OR CPR for the Professional Rescuer at the American Red Cross or American Heart Association Be sure to sign your card on the back where it says “Card Holder” card is not valid without your signature Online CPR certifications must have hands on demonstration. b) PNC Statement of Health Form with Current Immunizations Students must submit a completed PNC Statement of Health Form with current immunization records. c) Criminal Background Check Student must complete both a BCI&I and FBI background check when selected to progress. Background checks must be for the nursing profession and care of children and adults. The background checks must be received by the program chair by the established deadline. Failure to complete paperwork in appropriate timeframe will be ground for dismissal from the progression process. Students should be aware that many facilities also require drug screening and additional background restrictions. Background checks are offered in the Cincinnati State Public Safety office in the Main Building, Room 7. The cost as of November, 2011is $60.00. Students who have background checks completed at an alternate location should direct the results to: Cincinnati State Technical and Community College Health and Public Safety Division – HPB 312 ATTN: Practical Nursing Certificate Program 3520 Central Parkway Cincinnati, OH 45223 Check your Cincinnati State e-mail daily. A PNC Admissions representative will contact you through your Cincinnati State e-mail only (please use e-mail address with domain name: @cincinnaitistate.edu) Designation into a cohort will be based on program progression criteria and completion of program requirements for each open application period. Attend mandatory orientation (if accepted into a cohort). All admitted students are sent an e-mail invitation for orientation.
