Attend a <strong>PNC</strong> information session.<br />

If you are new to <strong>Cincinnati</strong> <strong>State</strong>, please attend a “Get There College Information Session". These<br />

sessions are offered every Tuesday at 9 AM and 6:30 PM in Main Building, Room 342. To register for the<br />

“Get There College Information Session”, please do so by calling the Admissions office at 513-569-1544 or<br />

online at http://www.cincinnatistate.edu/contact/get-there-session.<br />

Apply to <strong>Cincinnati</strong> <strong>State</strong> online: https://swebapps.cincinnatistate.edu/apply/page_who_are_you.aspx<br />

When you apply choose <strong>Practical</strong> <strong>Nursing</strong> <strong>Certificate</strong> as your major.<br />

If you are already a <strong>Cincinnati</strong> <strong>State</strong> student you may attend the <strong>PNC</strong> information session without<br />

registering for it. The <strong>PNC</strong> information sessions for currently enrolled students are offered on Thursday’s<br />

at 10 am in Health and Public Safety Building (HPB), Room 06 or at 5:00 pm in HPB Room 06<br />

<br />

Earned a high school diploma or GED.<br />

Request to have your official high school and college transcripts mailed to<br />

<strong>Cincinnati</strong> <strong>State</strong>’s Admissions Office by using the following online form:<br />

or by picking up a transcript request form in the Admissions office – Main Building,<br />

Room 181.<br />

Mail or bring a copy of official GED scores to <strong>Cincinnati</strong> <strong>State</strong>’s Admissions Office<br />

Financial Aid-If planning to use financial aid, meet with a financial aid counselor in the Financial Aid office – Main<br />

Building, Room 155 or you may call 513-569-1530 to set up an appointment. Be sure that your financial aid is in<br />

order before registering for any classes or your coursework may not be covered.<br />

If you have taken college level English and/or math and earned a grade “C” or better in each (college level<br />

math must be within the last 7 years for <strong>PNC</strong> program), you may submit a COMPASS waiver form for one or<br />

more sections of the Compass test by using the online COMPASS waiver form below:<br />

If you have taken the ACT achievement test within the last two years, you may be exempt from placement<br />

testing. Submit a copy of your ACT scores to the Office of Admission via www.ACTstudent.org.<br />

If you have not completed college level English or math (math must be within the last 7 years) and/or have<br />

not earned a grade “C” or better, you are required to take the Compass for full admission to <strong>Cincinnati</strong><br />

<strong>State</strong>. The Compass Lab is located in Main Building, Room 176. You do not need an appointment to take<br />

the Compass.<br />

Complete or confirm waiver (based upon Compass scores or transfer credit) from the following<br />

Academic Foundations courses:<br />

AFM 075, 090, 095<br />

AFL 070, 080, 085<br />

All required Academic Foundation courses must be completed with an earned grade of a C or greater, prior<br />

to being eligible to apply for progression into the <strong>Practical</strong> <strong>Nursing</strong> <strong>Certificate</strong> Program.<br />

Schedule an advising appointment by choosing the appropriate advising option below:<br />

Academic Advising<br />

Students who are required to enroll in Academic Foundation (AF) courses based on COMPASS test scores<br />

are seen by a pre-admit Advisor in the Advising and Counseling Center, Room 168 Main Building. To<br />

schedule an appointment with a pre-admit advisor, students should call the Advising and Counseling Center<br />

at (513)-569-1552.<br />

Program Advising<br />

Students who have completed all (or were not required to complete) AF courses are seen by a program<br />

advisor in the Health and Public Safety Division office, Room 312 Health Professions Building. To schedule<br />

an appointment with a program advisor, students should call the Health and Public Safety Division office at<br />

(513)-569-1670.<br />

Mandatory Requirements to apply for <strong>Practical</strong> <strong>Nursing</strong> Progression<br />

Complete <strong>PNC</strong> Progression Application - have <strong>PNC</strong> declared as major to apply<br />

Attend mandatory <strong>PNC</strong> Information Sessions<br />

Cumulative GPA of 2.0 or above- must maintain GPA throughout application process<br />

Math & Science GPA of 2.0 or above-must maintain GPA throughout application process<br />

Take the WorkKeys test- score of 4 or greater need for each section<br />

<strong>State</strong> Tested Nurse Aide - STNA<br />

If you are not a STNA, you may take MCH 130 at <strong>Cincinnati</strong> <strong>State</strong> or the STNA course offered through<br />

Great Oaks, www.greatoaks.com , to prepare for the state test.<br />

The MCH 130 course offered at <strong>Cincinnati</strong> <strong>State</strong> requires the student to complete a Nurse Aide Training<br />

application located in Health Professions Building, Room 312. Submit the application to the front desk in<br />

the Health and Public Safety Division – Health Professions Building, Room 312 in order to enroll in Nurse<br />

Aide Training. Consider taking the <strong>State</strong>ment of Health form (see below) to your healthcare provider<br />

when completing the Nurse Aide training application.<br />

Take the two-part state Nurse Aide test and pass both portions (written and skills.)<br />

Verify that you are in GOOD STANDING on a state Nurse Aide registry. Obtain a print-out from your state<br />

Nurse Aide registry website showing that you are in GOOD STANDING. For Ohio, please refer to:<br />

If you are already an STNA working outside of long-term healthcare and are no longer current on a Nurse<br />

Aide state registry, please refer to<br />

If applicable, provide documentation of health care work experience on company letterhead. During the<br />

open application period, submit the letter to the Health and Public Safety Division attention Tammy Smith or<br />

e-mail copy of the letter to pncprogression@cincinnatistate.edu letter must be received by application<br />

deadline date.<br />

If applicable, during the open application period provide documentation of military service to Health and<br />

Public Safety attention Tammy Smith or e-mail a copy to pncprogression@cincinnatistate.edu by the<br />

application deadline date.<br />

<strong>PNC</strong> Progression Application Process<br />

1. Complete an eform in order to apply for <strong>PNC</strong> Progression. The e-form application is only available the open<br />

application period. There will be an application period for each subsequent semester.<br />

Once applications are reviewed, applicants are moved forward in the progression process<br />

based upon the applicant’s <strong>PNC</strong> Progression worksheet scores.<br />

2. Progressing applicants will be notified of these additional requirements and the deadline for submission:<br />

a) CPR Certification<br />

Student must submit a signed copy of a current Healthcare Provider or Professional Rescuer<br />

certification<br />

(for adults, infants, children and AED). Online certification with hands on demonstration is accepted.<br />

Options for CPR Training:<br />

MCH 130 is offered at <strong>Cincinnati</strong> <strong>State</strong>;<br />

CPR for the Healthcare Provider at Great Oaks<br />

CPR for the Healthcare Provider OR CPR for the Professional Rescuer at the American Red Cross<br />

or American Heart Association<br />

Be sure to sign your card on the back where it says “Card Holder”<br />

card is not valid without your signature<br />

Online CPR certifications must have hands on demonstration.<br />

b) <strong>PNC</strong> <strong>State</strong>ment of Health Form with Current Immunizations<br />

Students must submit a completed <strong>PNC</strong> <strong>State</strong>ment of Health Form with current immunization records.<br />

c) Criminal Background Check<br />

Student must complete both a BCI&I and FBI background check when selected to progress.<br />

Background checks must be for the nursing profession and care of children and adults. The<br />

background checks must be received by the program chair by the established deadline. Failure to<br />

complete paperwork in appropriate timeframe will be ground for dismissal from the progression<br />

process. Students should be aware that many facilities also require drug screening and additional<br />

background restrictions. Background checks are offered in the <strong>Cincinnati</strong> <strong>State</strong> Public Safety office in<br />

the Main Building, Room 7. The cost as of November, 2011is $60.00. Students who have background<br />

checks completed at an alternate location should direct the results to:<br />

Check your <strong>Cincinnati</strong> <strong>State</strong> e-mail daily. A <strong>PNC</strong> Admissions representative will contact you through your<br />

<strong>Cincinnati</strong> <strong>State</strong> e-mail only (please use e-mail address with domain name: @cincinnaitistate.edu)<br />

Designation into a cohort will be based on program progression criteria and completion of program requirements for<br />

each open application period.<br />

Attend mandatory orientation (if accepted into a cohort). All admitted students are sent an e-mail invitation for<br />

orientation. Failure to attend the orientation may result in forfeiture of your designated seat.<br />
