Introducing the team<br />

You have been referred to the <strong>Bridgeways</strong> <strong>Day</strong><br />

<strong>Hospital</strong> which is part of the Community<br />

Assessment and Therapy Team.<br />

This is because your referrer believes that you<br />

may benefit from our assessment, support and<br />

therapy which aims to help you work towards<br />

recovery.<br />

What does the team do?<br />

<strong>Bridgeways</strong> <strong>Day</strong> <strong>Hospital</strong> helps older people to<br />

cope with their mental health problems in a<br />

supportive and caring environment. Every person<br />

who attends <strong>Bridgeways</strong> can expect individually<br />

tailored therapy and care. A care plan is agreed<br />

with the person who is experiencing the problems.<br />

They will receive a written copy of their care plan<br />

that they can refer to.<br />

Care may be provided in group or individual<br />

settings. These may be in <strong>Bridgeways</strong>, out in the<br />

community or in the person’s own neighbourhood<br />

and home. The team will regularly review<br />

progress involving carers and family where<br />

agreed. They will explore the best ways to work<br />

towards a positive future, wellbeing, safety and<br />

quality of life.<br />

Who are we?<br />

We are a team of experienced mental health<br />

workers who have many years of experience in<br />

caring and supporting people with mental health<br />

problems. These include staff involved in support<br />

time and recovery work, occupational therapy,<br />

nursing, psychology, social work, and medicine.


The process<br />

Following the initial referral from your GP or other<br />

community worker to the Community Assessment<br />

and Therapy Team, an assessment will identify<br />

your needs and whether <strong>Bridgeways</strong> is suitable<br />

for you.<br />

Once agreed, you will be welcomed to<br />

<strong>Bridgeways</strong> by a member of the team who will<br />

also introduce you to staff and show you around.<br />

Your needs will be assessed and we will discuss<br />

with you which days or sessions you may find<br />

most helpful. You will be allocated a care<br />

coordinator who will help to organise, liaise and<br />

advocate for you during your time at <strong>Bridgeways</strong>.<br />

You will asked about your current difficulties and<br />

how long you have been feeling this way. You<br />

may be asked about your social circumstances<br />

and your past medical history. You may like to<br />

write down any questions you have before your<br />

first appointment. There is space for this at the<br />

back of this leaflet. It may also be helpful to bring<br />

with you any medication you are currently taking.<br />

Any queries will be answered by staff and your<br />

care coordinator.<br />

Confidentiality<br />

All our staff are required to abide by a strict code<br />

of conduct on confidentiality. We may share some<br />

information with other staff who do not work for<br />

<strong>Oxleas</strong> <strong>NHS</strong> <strong>Foundation</strong> <strong>Trust</strong>. Examples may<br />

include your GP and social services employees.<br />

When considering who may see information about<br />

you, our staff use the following principles:<br />

only share information with those who need to<br />

know in order to provide good quality care<br />

share the minimum information necessary to<br />

ensure good quality care.
