Parental Financial Agreement 1. Payments are due Friday before services are rendered. A $5.00 late fee will be added, per day, if the tuition is unpaid after 6:30 P.M. on Monday. Child/ren will not be admitted if payment is not received by Wednesday at 6:30 P.M. BE SURE TO GET A RECEIPT FOR ALL PAYMENTS. 2. There is no adjustment of fees when a child is absent from school, including holidays or school cancellations. 3. There is a $37 charge for each returned check. Two returned checks will result in your account being placed on a “certified check or money order” status. 4. If your HEAD START and/or GEORGIA PRE-K child cannot be picked up by 3:00 P.M. every day, they should be enrolled in the after care program. If you are late picking your child up your child will be placed in after care and you will be charged $20 for the day. If you have prior knowledge that your child will be in after care please notify us. 5. A late fee charge of $1 per minute is due if your child is not picked up by 6:30 P.M. If a child remains on the premises longer than 30 (thirty) minutes after closing, DFCS will be notified of the neglect. 6. A 2 (two) week’s written notice must be provided before withdrawing a child. All parents will receive an exit interview to determine if their child’s needs were met. Parental Agreement The Andrew and Walter Young Family YMCA Early Learning Academy agrees to provide care for my child: _____________________________________________________________________ Child’s Name 1. Children who must take prescribed medicine are required to have a medication form filled out and signed at the time the medication and instructions are brought to the front desk. Medications must be labeled with the child’s name, date, name of the licensed health care provider, expiration date, and original prescription label. Authorization to dispense medication shall be limited to two weeks, unless otherwise prescribed. Children are not to have any form of medication on their person or in their cubbies, book bags, diaper bags, etc. 2. My child will not be allowed to enter or leave the facility without being escorted by the parent(s), person authorized by parent(s), or facility personnel. 3. I acknowledge it is my responsibility to keep my child’s records current to reflect any significant changes as they occur, e.g. telephone numbers, work location, emergency contacts, child’s health status, etc. 4. The facility agrees to keep me informed of any incidents, including illnesses, injuries, adverse reactions to medications, and exposure to communicable diseases which include my child. 5. The Andrew and Walter Young Family YMCA Early Learning Academy agrees to obtain written authorization from me before my child participates in routine transportation, field trips, special activities away from the facility, and water related activities occurring in water that is more than two (2) feet deep.
