<strong>SYVPI</strong> RISK ASSESSMENT PROTOCOL for NEIGHBORHOOD<br />

NETWORKS<br />

1. The <strong>SYVPI</strong> <strong>Risk</strong> <strong>Assessment</strong> <strong>Tool</strong> will be completed according to the<br />

following schedule:<br />

a. The Initial <strong>Risk</strong> <strong>Assessment</strong> <strong>Tool</strong> will be conducted during the intake<br />

process for each youth referred to a <strong>SYVPI</strong> Network.<br />

b. The <strong>Risk</strong> <strong>Assessment</strong> <strong>Tool</strong> will be conducted every six (6) months<br />

after the original intake assessment. The 6-, 12-, 18- months <strong>Risk</strong><br />

<strong>Assessment</strong> <strong>Tool</strong>s will be used for these assessment periods.<br />

c. The Exit <strong>Risk</strong> <strong>Assessment</strong> <strong>Tool</strong> will be conducted when the youth is<br />

closed out from the Initiative.<br />

2. The <strong>SYVPI</strong> Network staff will use Motivational Interviewing to complete the<br />

<strong>SYVPI</strong> <strong>Risk</strong> <strong>Assessment</strong> <strong>Tool</strong>. It may take a few intake conversations to<br />

cover all of the domains. It is a best practice for Network staff to<br />

complete the Initial <strong>Risk</strong> <strong>Assessment</strong> within two (2) weeks after the youth<br />

has agreed to enroll in the Initiative and signed the <strong>SYVPI</strong> Disclosure of<br />

Information form. The information from the Initial <strong>Risk</strong> <strong>Assessment</strong> should<br />

help inform the development of the youth’s Service Goal Plan.<br />

3. <strong>SYVPI</strong> Network staff may collect information from collateral contacts to<br />

complete sections related to school and criminal history.<br />

4. As a best practice, the risk assessment should be entered into the <strong>SYVPI</strong><br />

database during the same week it is completed. At the very latest, the<br />

risk assessment shall be entered into the <strong>SYVPI</strong> database by the 5 th<br />

working day of the month for the previous month.<br />

5. It is a best practice to revisit the previous risk assessment when<br />

completing a follow-up risk assessment to evaluate the youth’s progress<br />

and to make appropriate changes to the youth’s Service Goal Plan.<br />

1. The <strong>SYVPI</strong> <strong>Risk</strong> <strong>Assessment</strong> <strong>Tool</strong> will be completed according to the<br />

a. The Initial <strong>Risk</strong> <strong>Assessment</strong> <strong>Tool</strong> will be conducted during the intake<br />

process for each referred youth who has agreed to participate in<br />

case management services.<br />

b. The <strong>Risk</strong> <strong>Assessment</strong> <strong>Tool</strong> will be conducted every six (6) months<br />

after the original intake assessment. The 6-, 12-, 18- months <strong>Risk</strong><br />

<strong>Assessment</strong> <strong>Tool</strong>s will be used for these assessment periods.<br />
