Preliminary Sign Up Form Instructions
2013 Boy Scout Resident Camp
Western Massachusetts Council, Inc. BSA
To the Scoutmaster:
To help us prepare for your week at summer camp, please fill out the accompanying form as completely
as possible as submit it no later than the second Pre-Camp meeting on June 18. The form is a fillable pdf which
will allow you to type the information in on your computer and then send the completed form via email to John
Willemain, Summer Camp Program Director, at johnwillemain@comcast.net. If you are unable to complete it
using your computer, feel free to print it, fill it in by hand and either bring it to one of the Pre-Camp meetings in
June or turn it in to either of the council service centers. If you fill it in by hand, please print it legibly so that we
will be able to create preliminary rosters of Scouts taking each merit badge and the other programs prior to
camp. On Sunday evening, immediately following dinner, each Scout will be asked to meet with the appropriate
staff members to confirm their merit badges (by turning in their signed blue cards) or participation in the other
programs. This will also be the time to make changes to these selections.
This form should be used to indicate which merit badges each Scout would like to take during camp and
at which time. For first year Scouts, they should indicate their participation in Trail’s Head. The form should
also be used to indicate a Scout’s interest in other programs such as Snorkeling BSA, Project COPE and the
ATV program. See the examples below. Submit as many copies as needed to cover all of your Scouts attending
camp. If you have any questions, please contact us at the email address above.
