Student Nationality Foreigners -‐ Non-‐Lebanese Documents Needed • Completed Application Form • Student Writing Sample for Grade 5 and above • Final Report Card for the last 2 school years completed • Certified letter from the previous school stating dates attended and grade levels completed. This letter should be stamped by the school and the Ministry of Education • 2 Student Reference Forms (Grade 1 -‐12) • Health Record and Health Supplement Form • Vaccination Record (required by Ministry of Health) • Copy of Foreign Passport • 2 Passport size photos NOTE: Wellspring offers a bilingual program in Arabic and English. All our students will take Arabic and will receive the appropriate support to learn both languages. In cases of exemption from official exams, no report of a student’s Arabic language achievement will be submitted to the Ministry of Education. 2) Students applying from outside of Lebanon: Student Nationality Lebanese (Applying to G1-‐G6) Lebanese (Applying to G7-‐G12) N.B. In cases of exemption from official exams, no report of a student’s Arabic language achievement will be submitted to the Ministry of Education. 3) Equivalence with the Lebanese Program: *Applicable for Lebanese students OR dual nationals, currently studying abroad and applying for acceptance to Grades 7-‐12: -‐If Applying from Abroad, 3 years completed abroad are required. *The requirements listed below are for Lebanese students registering for the Lebanese program. Student Nationality Lebanese Studied Abroad OR Lebanese with Dual Nationality (Lebanese with a Foreign Nationality) Documents Needed • The Final Report card and a Certified Letter from the last school attended should be stamped from a) the current school, b) the Ministry of Education in the country of residence, c) the Lebanese Embassy or Consulate in the country of residence, and d) Ministry of Foreign Affairs from the country of residence. • These documents should be submitted to Wellspring with the application.
