10.3.3 Destruction <strong>of</strong> records including electronic records or images<br />

containing patients’ data is undertaken in a secure manner.<br />

10.4 Special Registers<br />

10.4.1 A register <strong>of</strong> patients is maintained. The registry can be in<br />

electronic or written <strong>for</strong>mat. The in<strong>for</strong>mation to be included is<br />

as follows -<br />

(i)<br />

the name, sex, date <strong>of</strong> birth, personal identifier, address <strong>and</strong><br />

telephone number <strong>of</strong> each patient;<br />

(ii) the number given to identify the medical record <strong>of</strong> that<br />

particular admission, e.g. hospital number <strong>and</strong> centre<br />

number;<br />

(iii) the date <strong>of</strong> admission; <strong>and</strong><br />

(iv) the date <strong>of</strong> discharge, transfer or death.<br />

10.4.2 A register is maintained on the details <strong>of</strong> medical devices<br />

implanted that serves critical purposes. Pacemakers are<br />

examples <strong>of</strong> medical devices used <strong>for</strong> critical purpose. The<br />

register should contain the name or identifier <strong>of</strong> the patient, the<br />

br<strong>and</strong>, model, batch number <strong>and</strong> serial number <strong>of</strong> the device <strong>and</strong><br />

the date <strong>of</strong> implant. This in<strong>for</strong>mation is to allow subsequent<br />

tracing.<br />

10.4.3 A register is maintained on the particulars <strong>of</strong> the patients<br />

receiving pharmaceutical products that are derived from human<br />

sources. Examples are plasma <strong>and</strong> its derivatives.<br />
