Dear Parents:
Welcome to the 2012-2013 Bethany Community Church Awana club. We are excited about the upcoming year of
Awana. We are putting together a great program & cannot wait to partner with you to teach your child about
God’s love & for them to memorize Bible verses. Here is some important information for you concerning BCC
Awana club:
Registration Process: We would like for you to register before the first night. There will be a registration
table during Sunday services from August 19 through September 2. If you register & pay before September 2,
your child will receive their book on the first night of club. Also on the first night, you can go directly to your
Awana room instead of the registration area.
Registration Fees: Fees for the Awana clubs are $25 per child for the first three kids. Each child after that
is $10. You registration fee covers your dues, Start Zone & the Awana book. Additional books may be an
additional fee. If there is a financial need, the church has scholarships available to support your family as
needed. Make checks payable to Bethany Community Church.
E-mail Lists: All important Awana info will be passed onto you via the Awana email list on the Bethany
Community Church website. To create a church profile & then add yourself to the Awana list, simply click on
"myBethany" on the site homepage. Help can be provided, if you have trouble. If you cannot create a profile,
the office can add you manually. Our Awana email list is the most consistent way to get Awana information. We
will also be populating the BCC facebook page with Awana information periodically.
Shirts, Vests, & Bags: Orders for shirts, vests, & bags will be taken at the second week of club. Your clubber
earns points & awards for having on their uniform. If you volunteer you can order shirts, vests, & bags during
the training session being held the Wednesday before club starts.
Donation Jar: Throughout the year, there will be a donation jar when your child comes into their Awana room.
The money collected through the year will be used to support a local missionary.
Volunteer Sign-up: Our Awana club is growing! Please consider how you can be a part of this ministry to
children. There are several different roles that are available and most do not require teaching or preparation
time. The focus for our volunteers is to build a relationship with the children and their parents. To sign up,
contact one of the people below.

Liability Information:
I, the undersigned parent/guardian hereby consent to my child(ren), participating in activities connected with CAMP GOOD NEWS® & BETHANY
COMMUNITY CHURCH in Washington, IL, an activity sponsored by Child Evangelism Fellowship®. I certify that my child is able to participate in these
activities including sports, hiking, ropes course, Zipline, & swimming, etc. (unless otherwise indicated). I have listed any activities I do not want my child to be
involved in. I UNDERSTAND & HEREBY AGREE TO ASSUME ALL OF THE RISKS WHICH MAY BE ENCOUNTERED ON SAID ACTIVITIES,
INCLUDING ACTIVITIES PRELIMINARY & SUBSEQUENT THERETO. If my child has medical conditions, which may be relevant to a physician in the event
of an emergency, I have listed them. In the event that an emergency occurs, I may be reached at the telephone number(s) listed. If I cannot be reached within a
reasonable period of time, I hereby authorize the Camp Good News staff to make emergency medical decisions for my child. I consent to the use of my child’s
portrait or picture for purposes of trade, publicity, & or information by Camp Good News® & Child Evangelism Fellowship® & Bethany Community Church. I do
hereby agree to hold Camp Good News® & Bethany Community Church & its agents & employees, harmless from any & all liability, actions, causes of action,
claims, expenses, & damages on account if injury to my child or property, even injury resulting in death, which I now have or which may arise in the future in
connection with the activity to participation in any other associated activities. I expressly agree that this release, waiver, & indemnity agreement is intended to be
broad & inclusive as permitted by the State of Illinois law & that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue
in full legal force & effect. This release contains the entire agreement between the parties hereto, & the terms of this release & contractual & to a mere recital. I
further state that I HAVE CAREFULLY READ THE FOREGOING RELEASE & KNOW THE CONTENTS THEREOF & I SIGN THIS RELEASE AS MY
OWN FREE ACT. This is a legally binding agreement, which I have read & understand.
