experienced shortfalls in stocks <strong>of</strong> essential drugs.<br />

One-fifth <strong>of</strong> health workers (20 percent Senegal; 21<br />

percent Tanzania) were absent on a given day. Most<br />

indices were worse in rural settings.<br />

An overhaul <strong>of</strong> the health care delivery system so<br />

that it is also responsive to chronic conditions <strong>and</strong><br />

NCDs, could focus on a number <strong>of</strong> areas:<br />

• Re-engineering <strong>of</strong> primary health care within a<br />

well-functioning district system, with a greater<br />

role for prevention <strong>and</strong> promotion [402]<br />

• Design <strong>of</strong> a delivery system suited to the coordinated<br />

continuity <strong>of</strong> care needed for chronic conditions<br />

• A greater role for patients in self-management<br />

• Decision support tools <strong>and</strong> clinical information<br />

systems.<br />

This would need to be underpinned by broader<br />

health system strengthening such as sustainable financing,<br />

performance management, improved procurement<br />

<strong>and</strong> supply <strong>of</strong> medicines <strong>and</strong> equipment,<br />

<strong>and</strong> task shifting.<br />

Primary Health Care<br />

Primary care is the main entry point into health<br />

services for most people. Ideally, although perhaps<br />

not <strong>of</strong>ten achieved, care is person-centered, comprehensive,<br />

<strong>and</strong> integrated, with continuity <strong>and</strong> participation<br />

<strong>of</strong> patients, families, <strong>and</strong> communities [403]<br />

(Figure 18). Apart from the management <strong>of</strong> disease,<br />

primary care opens up opportunities for disease prevention<br />

<strong>and</strong> health promotion as well as early detection<br />

<strong>of</strong> disease.<br />

Primary care helps prevent illness <strong>and</strong> death, is<br />

better value for money than its alternatives, <strong>and</strong>, in<br />

contrast to specialty care, is associated with a more<br />

equitable distribution <strong>of</strong> health in populations [403-<br />

404]. Health systems with a strong primary care orientation,<br />

emphasizing comprehensive care <strong>and</strong> the<br />

overall health <strong>of</strong> the patient, are likely to find it eas-<br />
