on Zinc in Bangladesh
For health and happiness
Issue-02, September-November 2007
Up to one-fifth of the world's people lack sufficient Zinc in their diet
Renowned Paediatrician Dr. Charles
P Larson has been working on Zinc
for many days. He is the Director of
SUZY project of ICDDR,B. Dr. Larson
talked with the editor of Pep
Newsletter frankly regarding various
topics on Zinc that is given below.
Pep Newsletter: Zinc is being used
for many days by the Physicians in the
treatment of different diseases but
recently the use of Zinc has reached
to a remarkable position in the
treatment of childhood diarrhea, what
may be the cause behind this
Dr. Charles P Larson: There are
various reasons behind this. I'm just
telling you about one important
reason. Continuous research is going
on in Bangladesh, India and Africa for
the evaluation of Zinc in childhood
diarrhea. All these researches and
clinical data support that Zinc has
wide role in the management of
diarrhea.
Pep Newsletter: What is the role of
Zinc in the management of childhood
diarrhea
Dr. Charles P Larson: Zinc reduces
the severity and duration of diarrhea
among children less than five years of
age. Besides speeding up recovery,
Zinc provides immunity to the children
for the prevention of diarrhea for the
next 3 months.
Pep Newsletter: How does Zinc
reduce the severity and duration of
diarrhea
Dr. Charles P Larson: There are
two main pathways by which Zinc
reduces the severity and duration of
diarrhea. First, Zinc increases the
immunity of the children by activating
T-cell, thus it inhibits the microorganisms
responsible for diarrhea.
Second, Zinc regenerates intestinal
microvilli which help to absorb foods.
By these ways Zinc reduces the
severity and duration of diarrhea. Zinc
also repairs the damaged gut mucosa.
Pep Newsletter: Now we see
various advertisements on Zinc in
different media. As a Physician what
is your comment regarding the use of
Zinc in children without prescription,
especially in Bangladesh
Dr. Charles P Larson: We
discussed with many paediatricians
and other healthcare professionals.
They recognized that, Zinc has no
major side effect other than vomiting
and Zinc could be given to children
without prescription. The one
exception, those being severely
malnourished. The advertisements
would not be harmful; rather it would
help the parents to treat their children
(suffering from diarrhea) immediately
with ORS and Zinc. I further want to
add that the Drug Administration of
Bangladesh also approved us to go for
minimize this scenario
Dr. Charles P Larson: Diarrhea can
be prevented by pursuing
multisectoral efforts like improving
access to clean water and safe
sanitation, promoting hygiene
education, exclusive breast-feeding,
improved weaning practices, using
sanitary latrines, keeping food and
water clean, washing hands with soap
(the baby's as well) before and after
touching food and after using latrine.
Pep Newsletter: Does Zinc play any
role for the treatment of diarrhea in
food. How the children can be
benefited by using Zinc
Dr. Charles P Larson: Although
there are not enough studies but
there are few studies to support that
Zinc increases appetite in children.
After talking to the parents, from my
experience I can say that Zinc
increases appetite. I have an advice
to the parents that don't insist your
child to eat more and more.
Pep Newsletter: Suppose a child
eats according to his/her need that is
sufficient but his/her parents claim
that the child is not eating adequately
and they encourage to take Zinc to
eat more, is there any problem with
this
Dr. Charles P Larson: I think "No",
as Zinc doesn't have any major side
effect. But I would discourage the
parents to administer Zinc for these
children and also suggest them not to
push their children to eat more and
more. We do not want to encourage a
drug as a solution for poor appetite in
children.
Pep Newsletter: What are the
outcomes of Zinc deficiency in human
body
Dr. Charles P Larson: Growth
retardation, weight loss, intercurrent
infections, delayed puberty in
adolescents, rough skin, poor
appetite, delayed wound healing, poor
immune system, short stature or
dwarfism, diarrhea, pneumonia, white
spots on fingernails,
mass media campaign giving the Zinc
dispersible tablet OTC (over-thecounter)
waiver.
Pep Newsletter: Recently WHO and
UNICEF revised their
recommendations on managing
childhood diarrhea and added the use
of Zinc to reduce the severity and
duration of diarrhea. How will you
evaluate the revised
recommendations
Dr. Charles P Larson: We evaluated
the impact of Zinc supplementation
on 26,000 children at ICDDR,B for the
management of diarrhea and found
the result very satisfactory.
Considering this I can say WHO and
UNICEF's revised recommendations
on managing childhood diarrhea is
very very logical and time demanded.
Pep Newsletter: In Bangladesh a
child suffers from diarrhea at least 3-4
times in a year on an average. This is,
of course, an alarming situation. What
preventive measures can be taken to
adult
Dr. Charles P Larson: Zinc
treatment of diarrhea in adults has
not been studied.
Pep Newsletter: Pneumonia is
another common problem for the
Bangladeshi children. Would you
please tell us about the role of Zinc in
the treatment of Pneumonia
Dr. Charles P Larson: Early studies
suggest that Zinc treatment may have
a positive impact on childhood
pneumonia. Two studies were
conducted in India and Nepal, one
study showed that Zinc had a
significant role in the treatment of
Community Acquired Pneumonia. In
ICDDR,B a study is still going on and
the result will be published very soon.
We are very hopeful about this study.
Pep Newsletter: Most of the parents
complain against their children that
they don't eat properly and they
(children) have no interest on given
Up to one-fifth of the world's people lack sufficient Zinc in their diet
reduction in collagen turnover and
synthesis, acne, cross-linking of
collagen, hyaluronic acid
abnormalities, defective connective
tissue, cataracts, teeth with black
plaque like deposits etc.
Pep Newsletter: Would you please
provide us any information on the
prevalence of Zinc deficiency
worldwide
Dr. Charles P Larson: Up to onefifth
of the world's people lack
sufficient Zinc in their diet, while an
estimated one-third live in thrid-world
countries considered at high risk of
Zinc deficiency.
Pep Newsletter: Which foods
contain Zinc most
Dr. Charles P Larson: Zinc is
mostly available in red meats, poultry
products, milk and dairy products,
spinach, nuts, oats, beans, and
certain seafoods. Oysters contain
more Zinc per serving than any other
food.
Pep Newsletter: What are the main
causes of Zinc deficiency in
Bangladeshi people
Dr. Charles P Larson: Although
Zinc is available in foods, the poor
people of Bangladesh don't receive
enough Zinc in their diets. They eat
cereals, legumes-based diets,
vegetables which are low in Zinc
content and also inhibit Zinc
absorption (as they have phytic acid
Approximately 300 enzymes
require Zinc for their activities. Zinc
is an essential trace mineral for
DNA synthesis, cell division, protein
synthesis and all necessary
processes for tissue regeneration &
repair. Zinc deficiency has been
associated with poor wound
healing and decreased breaking
strength of animal
wounds, which can
result from
decreased protein
and collagen
synthesis during
healing found in
Zinc-deficient
animals. Senapati
and Thompson
found Zinc levels
were 50-percent
higher in muscle and
skin from abdominal
wounds of rats
during wound
healing, but mild
deficiency reduced
this accumulation. Zinc demands
are thought to be the highest from
time of wounding throughout the
early inflammatory phase.
Sequential changes in Zinc
concentrations were studied in the
incisional wound model in the rat.
Zinc levels increased from
wounding and peaked on the fifth
which hamper the absorption of
Zinc). In their regular diet, they lack
protein-based foods like red meat,
milk, which are potent Zinc source.
Moreover, Zinc level in the soil of this
country is decreasing day by day, so
we don't get enough Zinc from rice
and other cereals. In case of the
infants, 70% of pregnant and
breastfeeding women are
malnourished, so their babies are not
getting enough Zinc from the
mothers. Some other reasons of Zinc
deficiencies are high prevalence of
hookworm infestation, hot and humid
climate (increase sweating and
considerable Zinc loss).
Pep Newsletter: Mention the name
of some regions of Bangladesh that
are Zinc deficient.
Dr. Charles P Larson: Almost all
over Bangladesh.
Pep Newsletter: Orion introduced
Zinc under the brand name Pep in
2000 for the first time in Bangladesh
and now Pep is the number one
brand in Zinc market of Bangladesh
with highest six different
presentations. Now we are publishing
Pep Newsletter. I think you read the
first issue. What is your comment on
it
Dr. Charles P Larson: Very well
done. I hope it will be published
regularly.
Pep Newsletter: Thank you for
Zinc in wound healing
day - at a time of high
inflammation, granulation tissue
formation, and epidermal cell
proliferation. Zinc concentrations
returned to normal by the seventh
day, when inflammation had
regressed. It has been suggested
that increased local demand for
Zinc resulting from surgery and
wounding exposes otherwise
marginal Zinc deficiencies in
humans. Perioperative Zinc
supplementation is recommended
for Zinc-depleted patients.
Data is lacking to show Zinc
supplementation improves healing
in non-deficient individuals;
however, Zinc deficiency in humans
spending your valuable time with us.
Dr. Charles P Larson: Best wishes
for Pep Newsletter and you also.
Ten tips for growing children
1. A child raised with patience will
learn about tolerance
2. A child raised with approval will
3. A child raised with encouragement
4. A child raised with laughter will live
6. A child raised with acceptance will
7. A child raised in security will know
8. A child raised with understanding
will gain self-knowledge
9. A child raised with love will love in
return
10. A child raised with compassion is
a gift to the world
is widespread and injured and
stressed individuals are more prone
to developing deficiencies. Ehrlich
et al suggest Zinc is lost in
significant amount after surgery
because of fistulas, stress, and
diarrhea. Zinc deficiencies have
also been identified in individuals
with deep partial or full-thickness
burns and chronic
venous leg ulceration.
Further research is
needed on the efficacy
of Zinc supplements for
wound healing.
Justification for
perioperative Zinc
supplementation
includes the absence of
adverse effects at
moderate doses (15-30
mg daily) and evidence
that Zinc deficiency
impairs wound healing.
Zinc supplementation of
15-30 mg daily is
recommended
perioperatively to prevent
unmasking of marginal deficiencies.
Higher levels of Zinc
supplementation may be necessary
in patients with malnutrition,
malabsorption, chronic diarrhea, or
other risk factors of Zinc deficiency.
Children with HIVinfection
benefit
Ref: www.therainbowbabies.com
from Zinc supplements
Zinc supplements are safe, simple,
and cost-effective method of
reducing illness in children infected
with HIV, concludes a randomised
trial in the latest issue of "The
Lancet". Previous studies have
shown that Zinc supplements can
reduce the incidence of diarrhea and
pneumonia in children in resourcepoor
settings.
However, the HIV virus requires Zinc
for its structure and function, and
Zinc activates lymphocytes that are
target cells for HIV-1 replication. For
these reasons the safety of Zinc
supplementation in children with
HIV-infection has been uncertain.
To study the affect of Zinc
supplementation, William Moss
(Johns Hopkins School of Public
Health, Baltimore, MD, USA) and
his colleagues recruited 96 children,
aged between 6 months to 5 years,
from Grey's Hospital in
Pietermaritzburg, South Africa. The
team randomly assigned children to
receive Zinc supplements or a
placebo daily for 6 months.
The investigators found that Zinc
supplementation did not result in an
increase in blood HIV viral load.
However, children receiving Zinc had
less diarrhea. The authors state that
Zinc supplementation should be
used as an adjunct therapy for
children with HIV-1 infection.
Dr Moss states: "Few interventions
are available to reduce morbidity in
children with HIV-1 infection in
resource-poor countries. Although
UNAIDS, WHO, and their partners
are committed to providing
antiretroviral therapy to 3 million
people by the end of 2005,
antiretroviral therapy and
prophylaxis for opportunistic
infections are not accessible for
many children. Consequently, more
than half of these children die
before the age of 3 years, most
commonly of respiratory tract
infections and diarrheal diseases.
Zinc supplementation could be a
simple and cost-effective
intervention to reduce morbidity and
mortality in children with HIV-1
infection. Programmes to enhance
Zinc intake in deficient populations
with a high prevalence of HIV-1
infection can be implemented
without concern for adverse effects
on virus replication."
I have looked through the Newsletter. It is a time
demanded publication. It is beneficial both for
