Renowned Paediatrician Dr. Charles<br />

P Larson has been working on Zinc<br />

for many days. He is the Director of<br />

SUZY project of ICDDR,B. Dr. Larson<br />

talked with the editor of Pep<br />

Newsletter frankly regarding various<br />

topics on Zinc that is given below.<br />

Pep Newsletter: Zinc is being used<br />

for many days by the Physicians in the<br />

treatment of different diseases but<br />

recently the use of Zinc has reached<br />

to a remarkable position in the<br />

treatment of childhood diarrhea, what<br />

may be the cause behind this<br />

Dr. Charles P Larson: There are<br />

various reasons behind this. I'm just<br />

telling you about one important<br />

reason. Continuous research is going<br />

on in Bangladesh, India and Africa for<br />

the evaluation of Zinc in childhood<br />

diarrhea. All these researches and<br />

clinical data support that Zinc has<br />

wide role in the management of<br />

diarrhea.<br />

Pep Newsletter: What is the role of<br />

Zinc in the management of childhood<br />

diarrhea<br />

Dr. Charles P Larson: Zinc reduces<br />

the severity and duration of diarrhea<br />

among children less than five years of<br />

age. Besides speeding up recovery,<br />

Zinc provides immunity to the children<br />

for the prevention of diarrhea for the<br />

next 3 months.<br />

Pep Newsletter: How does Zinc<br />

reduce the severity and duration of<br />

diarrhea<br />

Dr. Charles P Larson: There are<br />

two main pathways by which Zinc<br />

reduces the severity and duration of<br />

diarrhea. First, Zinc increases the<br />

immunity of the children by activating<br />

T-cell, thus it inhibits the microorganisms<br />

responsible for diarrhea.<br />

Second, Zinc regenerates intestinal<br />

microvilli which help to absorb foods.<br />

By these ways Zinc reduces the<br />

severity and duration of diarrhea. Zinc<br />

also repairs the damaged gut mucosa.<br />

Pep Newsletter: Now we see<br />

various advertisements on Zinc in<br />

different media. As a Physician what<br />

is your comment regarding the use of<br />

Zinc in children without prescription,<br />

especially in Bangladesh<br />

Dr. Charles P Larson: We<br />

discussed with many paediatricians<br />

and other healthcare professionals.<br />

They recognized that, Zinc has no<br />

major side effect other than vomiting<br />

and Zinc could be given to children<br />

without prescription. The one<br />

exception, those being severely<br />

malnourished. The advertisements<br />

would not be harmful; rather it would<br />

help the parents to treat their children<br />

(suffering from diarrhea) immediately<br />

with ORS and Zinc. I further want to<br />

add that the Drug Administration of<br />

Bangladesh also approved us to go for<br />

minimize this scenario<br />

Dr. Charles P Larson: Diarrhea can<br />

be prevented by pursuing<br />

multisectoral efforts like improving<br />

access to clean water and safe<br />

sanitation, promoting hygiene<br />

education, exclusive breast-feeding,<br />

improved weaning practices, using<br />

sanitary latrines, keeping food and<br />

water clean, washing hands with soap<br />

(the baby's as well) before and after<br />

touching food and after using latrine.<br />

Pep Newsletter: Does Zinc play any<br />

role for the treatment of diarrhea in<br />

food. How the children can be<br />

benefited by using Zinc<br />

Dr. Charles P Larson: Although<br />

there are not enough studies but<br />

there are few studies to support that<br />

Zinc increases appetite in children.<br />

After talking to the parents, from my<br />

experience I can say that Zinc<br />

increases appetite. I have an advice<br />

to the parents that don't insist your<br />

child to eat more and more.<br />

Pep Newsletter: Suppose a child<br />

eats according to his/her need that is<br />

sufficient but his/her parents claim<br />

that the child is not eating adequately<br />

and they encourage to take Zinc to<br />

eat more, is there any problem with<br />

this<br />

Dr. Charles P Larson: I think "No",<br />

as Zinc doesn't have any major side<br />

effect. But I would discourage the<br />

parents to administer Zinc for these<br />

children and also suggest them not to<br />

push their children to eat more and<br />

more. We do not want to encourage a<br />

drug as a solution for poor appetite in<br />

children.<br />

Pep Newsletter: What are the<br />

outcomes of Zinc deficiency in human<br />

body<br />

Dr. Charles P Larson: Growth<br />

retardation, weight loss, intercurrent<br />

infections, delayed puberty in<br />

adolescents, rough skin, poor<br />

appetite, delayed wound healing, poor<br />

immune system, short stature or<br />

dwarfism, diarrhea, pneumonia, white<br />

spots on fingernails,<br />

mass media campaign giving the Zinc<br />

dispersible tablet OTC (over-thecounter)<br />

waiver.<br />

Pep Newsletter: Recently WHO and<br />

UNICEF revised their<br />

recommendations on managing<br />

childhood diarrhea and added the use<br />

of Zinc to reduce the severity and<br />

duration of diarrhea. How will you<br />

evaluate the revised<br />

recommendations<br />

Dr. Charles P Larson: We evaluated<br />

the impact of Zinc supplementation<br />

on 26,000 children at ICDDR,B for the<br />

management of diarrhea and found<br />

the result very satisfactory.<br />

Considering this I can say WHO and<br />

UNICEF's revised recommendations<br />

on managing childhood diarrhea is<br />

very very logical and time demanded.<br />

Pep Newsletter: In Bangladesh a<br />

child suffers from diarrhea at least 3-4<br />

times in a year on an average. This is,<br />

of course, an alarming situation. What<br />

preventive measures can be taken to<br />

adult<br />

Dr. Charles P Larson: Zinc<br />

treatment of diarrhea in adults has<br />

not been studied.<br />

Pep Newsletter: Pneumonia is<br />

another common problem for the<br />

Bangladeshi children. Would you<br />

please tell us about the role of Zinc in<br />

the treatment of Pneumonia<br />

Dr. Charles P Larson: Early studies<br />

suggest that Zinc treatment may have<br />

a positive impact on childhood<br />

pneumonia. Two studies were<br />

conducted in India and Nepal, one<br />

study showed that Zinc had a<br />

significant role in the treatment of<br />

Community Acquired Pneumonia. In<br />

ICDDR,B a study is still going on and<br />

the result will be published very soon.<br />

We are very hopeful about this study.<br />

complain against their children that<br />

sufficient Zinc in their diet, while an<br />

estimated one-third live in thrid-world<br />

countries considered at high risk of<br />

Zinc deficiency.<br />

Pep Newsletter: Which foods<br />

contain Zinc most<br />

Dr. Charles P Larson: Zinc is<br />

mostly available in red meats, poultry<br />

products, milk and dairy products,<br />

spinach, nuts, oats, beans, and<br />

certain seafoods. Oysters contain<br />

more Zinc per serving than any other<br />

food.<br />

Pep Newsletter: What are the main<br />

causes of Zinc deficiency in<br />

Bangladeshi people<br />

Dr. Charles P Larson: Although<br />

Zinc is available in foods, the poor<br />

people of Bangladesh don't receive<br />

enough Zinc in their diets. They eat<br />

cereals, legumes-based diets,<br />

vegetables which are low in Zinc<br />

content and also inhibit Zinc<br />

absorption (as they have phytic acid<br />

Approximately 300 enzymes<br />

require Zinc for their activities. Zinc<br />

is an essential trace mineral for<br />

DNA synthesis, cell division, protein<br />

synthesis and all necessary<br />

processes for tissue regeneration &<br />

repair. Zinc deficiency has been<br />

associated with poor wound<br />

healing and decreased breaking<br />

strength of animal<br />

wounds, which can<br />

result from<br />

decreased protein<br />

and collagen<br />

synthesis during<br />

healing found in<br />

Zinc-deficient<br />

animals. Senapati<br />

and Thompson<br />

found Zinc levels<br />

were 50-percent<br />

higher in muscle and<br />

skin from abdominal<br />

wounds of rats<br />

during wound<br />

healing, but mild<br />

deficiency reduced<br />

this accumulation. Zinc demands<br />

are thought to be the highest from<br />

time of wounding throughout the<br />

early inflammatory phase.<br />

Sequential changes in Zinc<br />

concentrations were studied in the<br />

incisional wound model in the rat.<br />

Zinc levels increased from<br />

wounding and peaked on the fifth<br />

which hamper the absorption of<br />

Zinc). In their regular diet, they lack<br />

protein-based foods like red meat,<br />

milk, which are potent Zinc source.<br />

Moreover, Zinc level in the soil of this<br />

country is decreasing day by day, so<br />

we don't get enough Zinc from rice<br />

and other cereals. In case of the<br />

infants, 70% of pregnant and<br />

breastfeeding women are<br />

malnourished, so their babies are not<br />

getting enough Zinc from the<br />

mothers. Some other reasons of Zinc<br />

deficiencies are high prevalence of<br />

hookworm infestation, hot and humid<br />

climate (increase sweating and<br />

considerable Zinc loss).<br />

of some regions of Bangladesh that<br />

are Zinc deficient.<br />

Dr. Charles P Larson: Almost all<br />

over Bangladesh.<br />

Dr. Charles P Larson: Very well<br />

done. I hope it will be published<br />

Pep Newsletter: Thank you for<br />

Zinc in wound healing<br />

day - at a time of high<br />

inflammation, granulation tissue<br />

formation, and epidermal cell<br />

proliferation. Zinc concentrations<br />

returned to normal by the seventh<br />

day, when inflammation had<br />

regressed. It has been suggested<br />

that increased local demand for<br />

Zinc resulting from surgery and<br />

wounding exposes otherwise<br />

marginal Zinc deficiencies in<br />

humans. Perioperative Zinc<br />

supplementation is recommended<br />

for Zinc-depleted patients.<br />

Data is lacking to show Zinc<br />

supplementation improves healing<br />

in non-deficient individuals;<br />

however, Zinc deficiency in humans<br />

spending your valuable time with us.<br />

Ten tips for growing children<br />

stressed individuals are more prone<br />

to developing deficiencies. Ehrlich<br />

et al suggest Zinc is lost in<br />

significant amount after surgery<br />

because of fistulas, stress, and<br />

diarrhea. Zinc deficiencies have<br />

also been identified in individuals<br />

with deep partial or full-thickness<br />

burns and chronic<br />

venous leg ulceration.<br />

Further research is<br />

needed on the efficacy<br />

of Zinc supplements for<br />

wound healing.<br />

Justification for<br />

perioperative Zinc<br />

supplementation<br />

includes the absence of<br />

adverse effects at<br />

moderate doses (15-30<br />

mg daily) and evidence<br />

that Zinc deficiency<br />

impairs wound healing.<br />

Zinc supplementation of<br />

15-30 mg daily is<br />

recommended<br />

perioperatively to prevent<br />

unmasking of marginal deficiencies.<br />

Higher levels of Zinc<br />

supplementation may be necessary<br />

in patients with malnutrition,<br />

malabsorption, chronic diarrhea, or<br />

other risk factors of Zinc deficiency.<br />

Children with HIVinfection<br />

Zinc supplements are safe, simple,<br />

and cost-effective method of<br />

reducing illness in children infected<br />

with HIV, concludes a randomised<br />

trial in the latest issue of "The<br />

Lancet". Previous studies have<br />

shown that Zinc supplements can<br />

reduce the incidence of diarrhea and<br />

pneumonia in children in resourcepoor<br />

settings.<br />

However, the HIV virus requires Zinc<br />

for its structure and function, and<br />

Zinc activates lymphocytes that are<br />

target cells for HIV-1 replication. For<br />

these reasons the safety of Zinc<br />

supplementation in children with<br />

HIV-infection has been uncertain.<br />

To study the affect of Zinc<br />

supplementation, William Moss<br />

(Johns Hopkins School of Public<br />

Health, Baltimore, MD, USA) and<br />

his colleagues recruited 96 children,<br />

aged between 6 months to 5 years,<br />

from Grey's Hospital in<br />

Pietermaritzburg, South Africa. The<br />

team randomly assigned children to<br />

receive Zinc supplements or a<br />

placebo daily for 6 months.<br />

The investigators found that Zinc<br />

supplementation did not result in an<br />

increase in blood HIV viral load.<br />

However, children receiving Zinc had<br />

less diarrhea. The authors state that<br />

Zinc supplementation should be<br />

used as an adjunct therapy for<br />

children with HIV-1 infection.<br />

Dr Moss states: "Few interventions<br />

are available to reduce morbidity in<br />

children with HIV-1 infection in<br />

resource-poor countries. Although<br />

UNAIDS, WHO, and their partners<br />

are committed to providing<br />

antiretroviral therapy to 3 million<br />

people by the end of 2005,<br />

antiretroviral therapy and<br />

prophylaxis for opportunistic<br />

infections are not accessible for<br />

many children. Consequently, more<br />

than half of these children die<br />

before the age of 3 years, most<br />

commonly of respiratory tract<br />

infections and diarrheal diseases.<br />

Zinc supplementation could be a<br />

simple and cost-effective<br />

intervention to reduce morbidity and<br />

mortality in children with HIV-1<br />

infection. Programmes to enhance<br />

Zinc intake in deficient populations<br />

with a high prevalence of HIV-1<br />

infection can be implemented<br />

without concern for adverse effects<br />

on virus replication."<br />
